        • Methodological Evaluation on Domestic Clinical Trials on Traditional Chinese Medicine Nursing from 2006 to 2011

          Objective To evaluate the methodological quality of clinical trials on traditional Chinese medicine (TCM) nursing in recent six years.Methods Such databases as CNKI, VIP, WanFang Data and CBM were searched for collecting clinical trials on TCM nursing published from January 2006 to September 2011, and domestic primary nursing journals were also searched from January 2010 and September 2011. Methodological quality of included studies was assessed using quality assessment criteria of the Cochrane systematic review guideline. Results A total of 854 clinical trials were retrieved, including 706 (82.7%) randomized controlled trials (RCTs), 108 (12.6%) quasi-randomized controlled trials and 40 (4.7%) non-randomized controlled trials. In the methodological quality analysis, the comparability of baseline was mentioned in 784 trials (91.8%), a total of 498 (58.3%) reported definite diagnosis criteria. 178 (20.8%) reported exclusive criteria. 831 studies (97.3%) applied relevant statistical methods properly. However, only 55 trials (6.4%) mentioned the method of randomization sequence. 10 studies (1.2%) described the method of randomiztion assignment. Blinding was mentioned in 22 studies (2.6%). 98 trials (11.5%) did prospective follow-up. 93 trials (10.9%) had safety description. 20 trials (2.3%) reported lost and with drawl cases, but only 2 conducted intention-to-treat analysis. It was hard to determine whether there was selective reporting bias or not because all the studies did not have protocols. Only 21 studies (2.5%) mentioned the lack of outcome indicators which could be the evidence for existing of bias. By annual analysis, there were 81 trials which conformed to at least 2 low risk criteria. 10 trials (12.3%) was published in 2009, 26 trials (32.1%) published in 2010, and 27 trials published by September 2011, indicated an uptrend. Conclusions According to the Cochrane Collaboration’s tool for assessing risk of bias, the overall quality of clinical trials on TCM nursing is low with defects in different degrees, but it rises gradually over years.

        • Actuality and Quality Analysis of Published EBN Related Articles in China

          Objective To explore the characteristics of articles related to Evidence-based Nursing (EBN) and to identify the problems and difficulties encountered by clinical nurses when they were engaged in evidence-based practice in China.Methods We searched for articles published in 6 nursing journals in China from 2000 to early 2006, and classified, investigated and analyzed the retrieved articles. Results Research on EBN has been changing from explaining theories and methods to applying research results in nursing practice. Among articles on clinical application, 90.9% did not describe the methods and search strategy used; 63.6% did not specify the sources of the evidence applied in practice; 93.9% did not describe the methodological quality of the evidence; only 6.1% used the results of systematic reviews or randomized trials to guide clinical practice; and 72.7% did not use effective evaluation methods to identify the nursing outcomes after applying evidences in clinical practice. Conclusions The quality of EBN related articles needs to improve further. Nurses should learn more about the knowledge and skills associated with EBN so as to improve the quality of nursing practice and of academic articles on EBN

        • Influence of Drug Repercussion by Pediatric Hospital Inpatients on Nursing Work

          ObjectiveTo explore the appropriate intervention measures to reduce the influence of drug repercussion by pediatric hospital inpatients on nursing work. MethodBetween March 1st and 28th, 2014, statistical analysis on the characteristics of pediatric drug-return by drug repercussion questionnaires was carried out. ResultsEach drug repercussion took much time of the nurses (median of 5.00 minutes per time). The frequency of drug repercussion in the internal medicine department was more than that in the surgical department; the most drug repercussions were found in the respiratory medicine department, reaching 26.84%. The main category of drug repercussion was aerosolized medication (39.32%). The drug repercussion mainly resulted from lack of patients' education and doctor-related administration, which had a proportion of 31.44% and 27.19%, respectively. ConclusionsThe wards which have more drug repercussions should be under the supervision according to the analysis of drug repercussion. Meanwhile, improving patients' education and training of medical staff can reduce the pediatric hospital inpatients' drug repercussion and also may reduce the bad effects on nursing work.

        • Analysis of Day Surgery Indication Expansion and Countermeasures for Nursing Risk Factors in the Department of Ophthalmology

          ObjectiveTo analyze the increased risks of nursing due to expansion of ophthalmic day surgery indications, and the countermeasures. MethodsWe collected the information in the last three years from January 2012 to December 2014 in the Department of Ophthalmology, including the number of operations, the proportion of cataract patients, patients aged over 70 and under 12 years old, patients with high-risk fall, the number of general anesthesia operations, adverse events, and the data from the satisfaction survey of the patients. All the data were analyzed by statistical method. ResultsDuring the last three years, the relaxation of ophthalmic day surgery indications led to an increased admission rate of high-risk patients, and caused more nursing risk factors. Through the efforts of prevention and care, during the last three years, there were no adverse events, and patients had a satisfaction rate over 90%. ConclusionAlthough the ophthalmic day surgery indication has been relaxed, through the establishment of nursing risk response system by pre-hospital guidance, admission assessment, peri-operative education and follow-up visit, with the continuous improvement of nursing management system and convenient workflow, we can not only improve the work efficiency, but also ensure nursing safety.

        • Current Status and Analysis on Clinical Nursing Risks to Nursing Students

          Objective To investigate the current status of clinical nursing risks to nursing students, so as to provide scientific references for medical universities and policy makers. Methods The self-designed questionnaires were distributed to 360 clinical nursing students in four Third-Level hospitals. The group leaders were trained as investigators regularly. All the other nursing students were asked to fill in the questionnaire alone within 30 minutes. The questions issued were the following four aspects: general information of nursing students, clinical intern operating situation, nursing defects and awareness of nursing risks. Results Among the total 400 questionnaires distributed, 360 valid ones were retrieved (90%). The analysis showed all 21 nursing operations could be performed by students alone, and 80% of the nursing students believed that they were able to do 11 routine operations alone, such as bed-making and vital sign monitoring. The incidence rate of eight nursing defects was between 0.56% and 12.22%. As for the four questions about nursing risks, 49.44% of the nursing students operated alone before getting themselves ready because of their eagerness for such experiences. According to the nursing students, the departments with the highest nursing risks ranked as emergency, operating theatre, surgery, pediatrics, internal medicine, and gynecology and obstetrics. Conclusion The current situation of clinical nursing risks to nursing interns is still far from being satisfactory, so it is necessary to improve the training at university, regulate the clinical education, and ensure the intern safety for nursing students.

        • Management and Nursing Pattern for Ambulatory Surgery Ward

          Ambulatory surgery ward is a fast and effective way of treating, nursing and patients’ education. We report the practice of ambulatory surgery ward in West China Hospital of Sichuan University about patients’ reception, treatment and nursing, medical safety, health education and post-operative follow-up. Results show that ambulatory surgery ward would guarantee patients’ safety, shorten hospitalization days and decrease medical cost, with more than 98% of patients’ satisfaction.

        • Study on Effects of Patient Safety Education for Undergraduate Nursing Students

          ObjectiveTo explore the effect of patient safety education course on the undergraduate nursing students in terms of knowledge, attitude and skills about patient safety. MethodsWe enrolled five-year undergraduates admitted to Wannan Medical College in 2009. Eighty two nursing students who took part in the elective course of patient safety education course at the first semester of the seniors were selected as the observation group; while the other 359 nursing students who did not took part in the course were selected as the control group. The questionnaire method was performed to investigate the knowledge, attitude and skills about patient safety before and after the course. ResultsThere were significant higher scores in the observation group than in the control group in the following domains:"Knowledge of medical errors" and "What should I do after medical errors occur?". ConclusionSetting up "patient safety education" course could significantly improve the knowledge, attitude and skills about patient safety for undergraduate nursing students.

        • Development and Application of Self-Made Visual Teaching Aids for Fetal Intrauterine Condition

          Objective To explore the application value of self-made visual teaching aids in gynecological and obstetrical nursing education. Methods A total of 240 nursing students in grade 2009 from Fujian Medical University and Fujian Health College were selected by cluster sampling and divided by simple randomization into 2 groups (the trial group and the control group). Besides the multimedia combined with traditional teaching adopted in both groups, the visual teaching aids for fetal intrauterine condition was also adopted in the trial group rather than the control group. Questionnaire survey and focus group interview were adopted to appraise the satisfactory degree of all nursing students and the teaching effects evaluation of students in the trial group. Results There were no significant differences between the two groups in education background, and intelligibility evaluation of theoretical study on both the fetal intrauterine condition and the complications in pregnancy and delivery periods (Pgt;0.05), while the difference was statistically significant in the satisfactory degree between different teaching methods (Plt;0.05). 85.0% of nursing students in the trial group thought that visual model could help them to better understand the complications in pregnancy and delivery periods, and the intrauterine condition, 99.17% of students thought that the teaching effect of visual model was better than traditional teaching, and 95.83% of students considered that visual model was favorable for course study. Conclusion The application of self-made visual teaching aids for fetal intrauterine condition makes gynecological and obstetrical nursing education more visual, facilitates students to better understand fetal intrauterine situation and part of the mechanism of pregnancy complications, arouses students’ learning interests, and lays a theory and practice foundation for follow-up internship, so as to enhance the quality of nursing teaching.

        • Nursing to Avoid Rupture of Intracranial Aneurysm during Induction of Anesthesia

          ObjectiveTo explore the nursing method to avoid rupture of intracranial aneurysm during induction of anesthesia. MethodWe retrospectively analyzed the nursing method for 428 patients with aneurysm during the induction of anesthesia between October 2012 and October 2013. According to the causes of rupture of intracranial aneurysm (anxiety, tension, excitement, sudden elevation of blood pressure, physical labor), we adopted nursing methods to avoid those causes, and implemented targeted nursing methods during induction of anesthesia. ResultsNo intracranial aneurysm rupture occurred in these 428 aneurysm patients during induction of anesthesia. Two patients' absolute value of systolic blood pressure was below 80 mm Hg (1 mm Hg=0.133 kPa) during induction of anesthesia, and the vital signs of other patients kept normal. The number of intraoperative rupture cases was 3. When discharged from hospital, there were 385 patients with good prognosis, 39 patients with bad prognosis, and 4 death cases. ConclusionsTargeted nursing method based on patients' particular situation during induction of anesthesia can effectively control patients' emotion, stabilize fluctuations in hemodynamic indexes, decrease the incidence of aneurysm rupture, improve surgery treatment effect of intracranial aneurysm clipping, decrease complications, and improve patients' prognosis.

        • Investigation and analysis of research capacity and demand of neurological nurses in Sichuan Province

          Objective To investigate the ways of scientific research knowledge acquirement, and the situation of research capacity and demand of neurological nurses in Sichuan Province. Methods From May to June 2015, 153 neurological nurses from 7 hospitals of Sichuan Province were enrolled by convenient stratified sampling method. The ways of acquiring scientific research knowledge, research capacity, and research demand of the nurses were investigated by questionnaire. Results Among the 153 nurses, there were 50.3% with low level, 47.7% with moderate level, and 2.0% with high level of scientific research capacity, respectively. The total scores of nursing research capacity ranged from 4 to 101. The dimension of " capacity of generating the research ideas” was the highest, and the dimensions of " capacity of designing research protocol” and " capacity of analyzing research data and material” were the lowest. Of the respondents, 66.2% had never attended any research training program, 51.3% acquired scientific research knowledge by themselves, and 65.6% were willing to participate research training courses. The respondents tended to study on their own by internet. There were significant differences in scientific research capacity among neurological nurses with different initial education, attending the college entrance examination or not, having attended research training and self-study or not (P<0.05). Conclusions Most nurses acquire scientific research knowledge by themselves, and insufficient research training couldn’t satisfy the research demand of neurological nurses. The initial education degree and research study intention are related to research capacity. However, due to the limited amount of samples, the positive influence of self-study and research training on the research capacity still need to be further explored.
