        • Current Situation of Hospital Infection Management in the Delivery Room in A Hospital

          ObjectiveTo understand the current situation of hospital infection management related to the hospital maternity ward, learn the risks of infection, discover problems existing in hospital infection management, and seek appropriate solutions for hospital infection. MethodsBetween January 2011 and December 2012, using uniform questionnaire for prospective survey and field interviews, we investigated the hospital infection situation in medical health care workers, pregnant women, and live newborns. ResultsA total of 2 225 questionnaires were retreated with a retreat rate of 100%. Hospital infection occurred in 23 cases, of which 15 cases were maternal infection (1.35%) and 8 cases were neonate infection (0.72%). Maternal infection was mainly focused on reproductive tract, surgery incision and urinary tract. Neonate infection was mainly focused on lower respiratory tract and skin. The management system of hospital infection in the maternity ward was basically strengthened, but the infection monitoring work was still not timely, and staff training in infection control knowledge was not in place. The overall environment, sterilization, disinfection and isolation should be strengthened, and there were also some other safety hazards. ConclusionThe management of infection in hospital maternity ward is the key to control the infection in maternity ward. The infection management seems perfect, but there are still some hidden dangers. The management system needs to be improved and the management should be implemented strictly according to the system, in order to avoid the occurrence of infection in maternity ward and ensure the safety of patients.

        • Investigation and analysis of the economic burden of disinfection supply and feasibility discussion on the regional centralized management mode of 263 medical institutions in Yibin City

          ObjectiveTo understand the economic burden of disinfection supply to medical institutions in Yibin City, and explore the feasibility of establishing a regional centralized management model of disinfection supply center in Yibin City.MethodsFrom April to May 2018, 263 medical institutions in the eight counties and two districts of Yibin City were investigated by means of mobile phone application-designed questionnaire, to obtain the information of cost accounting and economic burden of disinfection supply.ResultsThere were 263 medical institutions involved in the survey, in which 61 (23.19%) had set up the central sterile supply department (CSSD), including 43 public hospitals and 18 private hospitals; 202 medical institutions were without CSSD, which were mainly secondary hospitals [195 (74.14%), including 120 public hospitals and 75 private hospitals]. The higher the hospital level was, the larger the average area of the CSSD was; the difference was statistically significant (χ2=40.009, P<0.001). The higher the hospital level was, the more full-time personnel were employed, and the difference was statistically significant (χ2=31.862, P<0.001), and the care staff were the majority (66.23%). The cost burden of CSSD was more than 1 million yuan in the tertiary hospitals, which was 100 000 yuan or above in 61.90% of the secondary hospitals, and was below 100 000 yuan in hospitals below secondary level. The higher the hospital level was, the higher the total cost burden became; the difference was statistically significant (χ2=37.995, P<0.001). ConclusionIn view of the heavy economic burden of CSSD in medical institutions and the unbalanced setting up of medical institutions below secondary level, the establishment of a regional CSSD centralized management model is a new direction, new trend, and new model for future development, which is conducive to improving the quality of disinfection and sterilization, reducing medical care costs, making rational use of health resources, effectively preventing hospital infections, and ensuring the medical safety.

        • Attitude Investigation in Henan Rural Residents on Partial Usage of New Rural Co-operative Medical System Funds to Disease Control and Prevention

          Objective To investigate the attitude and its influencing factors of Henan provincial rural residents towards the partial usage of funds from the New Rural Co-operative Medical System (NRCMS) for the disease control and prevention, in order to provide evidence for policy making. Methods In Henan province, 1 117 rural residents were randomly sampled with questionnaire from the 156 villages distributing in 44 townships of 19 counties (cities, districts). The frequency analyses, the multiple linear regression analysis and the one-way analysis of variance were conducted. Results Among all the respondents, only 3.4% of the rural residents absolutely disagreed (Zero agreement degree score), 34.4% fully agreed (10 agreement degree scores), and 90.6% had agreement degree scores equal to or more than five. The agreement degree was direct proportional to NRCMS satisfaction degree. The agreement degrees from residents who were relieved from medical financial burdens by NRCMS were higher than the others. 13.2% of rural residents believed that NRCMS did not alleviate their medical financial burden. For the men and women who believed that their village general practitioner was timely at vaccination, their agreement degree was higher than the others who considered vaccination time was late or common. The “timely group” was alone a subset. The “late group” and the “common group” were homogeneous subsets. The proportion of those who answered that the vaccination timeliness at late or common reached 40.6%. Conclusion The proposition to pay funds from the existing NRCMS for disease control and prevention is in line with the will of the majority of rural residents. The combination of disease control and prevention and NRCMS is a strategy in rural healthcare management. A bettering NRCMS and disease control and prevention are the basis of this policy in the future. More attention should be paid to the timeliness of the village general practitioners’ disease control work. It is necessary to perfect the NRCMS policy focusing on residents who hasn’t been relieved from medical financial burdens, so that more population will be benefited.

        • Investigation and Analysis of the Prevalence Rate of Inpatient Nosocomial Infections in A Municipal Medical Unit and Its Enlightenment on Prevention and Control

          ObjectiveTo understand the current status of nosocomial infections in a municipal medical unit, to find problems in daily monitoring, and to provide the evidence for the prevention and control of nosocomial infection in high-risk groups and procedures. MethodsAccording to the 2013 survey requirements made by the nosocomial infection control center of Chengdu, we made a cross-sectional survey about nosocomial infection among all the inpatients on July 25th, 2013 and statistically analyzed the results. ResultsTotally, 1 301 cases were actually investigated within 1 307 inpatients of the time (the actual investigation rate was 99.54%). The prevalence rate of nosocomial infection was 3.38% (44 cases). The top five departments of infection rate were Respiratory Intensive Care Unit (RICU), Center of Intensive Care Unit (CICU), Department of Neurosurgery, Department of Thoracic Surgery and Department of General Surgery. The main site of infection was respiratory tract, which took possession of 62.25%. In univariate analyses, age≥60, length of hospital stay >2 weeks, invasive operation, history of diabetes, operation, radiotherapy/chemotherapy, utilization of antibiotics were found to be risk factors for infections. Multivariate analysis showed that length of hospital stay (OR=3.115, P=0.001), invasive operation (OR=14.930, P<0.001), diabetes mellitus (OR=2.157, P=0.046), radiotherapy/chemotherapy (OR=7.497, P<0.001) were independent risk factors for nosocomial infections. The utilization rate of antibiotics was 45.73%. Among them, there was 85.21% using single antibiotics, and 82.18% of them were used therapeutically. ConclusionUnderstanding the current status of nosocomial infection in municipal hospitals helps us grasp the key and difficulty of infection control, make out prevention-control measures for high-risk groups and high-risk departments, and guide and supervise its implementation in clinical departments, which points out the direction to further reduce the incidence of nosocomial infection.

        • Survey on the Antidepressant Use in In-patients with Depression

          ObjectiveTo investigate the use of traditional and new-type antidepressants in the in-patients with depressive disorder. MethodsThe antidepressant use of 165 in-patients with depressive disorders from March 1st to April 30th, 2013 were investigated. ResultsThe new-type antidepressants became the best drug in clinic, and single drug use was carried out mostly. The most used drug was atypical antipsychotics. ConclusionThe use of antidepressant drugs in the in-patients with depressive disorder in our hospital is regular and reasonable.

        • Investigation on the Course of Evidence-Based Clinical Decision Making in China by Evidence-Based Case Reports

          Objective To investigate evidence retrieval, appraisal, and reevaluation during evidence-based clinical decision making in China. Also, to analyze the related factors, so as to find the problems in the course of evidence-based clinical decision making and put forward corresponding solutions. Methods We searched Chinese Biomedical Literature Disc (CBM) and China Journal Full-text Database (Medical sciences) of the China National Knowledge Infrastructure (CNKI) to collect clinical evidence-based case reports. Relevant information was extracted from these reports by a selfdesigned investigation form.Then statistical analyses were performed. Results The search tools used in the course of evidence-based clinical decision making varied. The most frequently used were MEDLINE/PubMed (82.08%) and The Cochrane Library (60.38%). 30.63% of evidence-based case reports described the search strategy in detail, and 9.01% described how they modified their search strategy. All doctors evaluated the association between evidence and disease, but few of them integrated patient factors and relevant external factors when evaluating evidence. The scientific nature and validity of the evidence was evaluated in 74 evidence-based case reports (66.67%), and such evaluation was mainly based on the criteria of evidence grading (50.00%). Reevaluation was mentioned in 85.59% of evidence-based case reports. Conclusion  In China, the application of evidence-based decision making varied in different clinical departments. Problems existed in the course of evidence retrieval, appraisal, and reevaluation. This revealed the low information diathesis level of doctors and their lack of evidence-based medicine knowledge. It is suggested that information education and evidence-based medicine education should be strengthened to improve doctors’ ability to use evidence-based clinical decision making. It is also recommended that the search tools, relevant search strategy, the modification of search strategy, and reevaluation on practice results of each case should be mentioned in evidence-based case reports.

        • A Survey of Residents’ Satisfaction with Community Health Services in Shifang

          Objective To investigate the construction and services of the community health service system in Shifang, as well as the satisfaction of community residents with the community health services and the post-disaster emergency response capability of the community hospital, so as to provide decision-making suggestions on better reconstruction of the community health service system and improvement of its post-disaster emergency response capability. Methods There were 4‰ of community residents in Fangting town were selected by convenience sampling for a face-to-face interview using a questionnaire. Logistic regression was used to identify the influencing factors of residents’ satisfaction with community health services. Results A total of 250 questionnaires were conducted for face-to-face interviews, and 246 ones were retrieved (response rate 98.4%). Residents’ understanding and satisfactory degree of the community health service were 41.1% and 36.6%, respectively. Health education, medical expenses and medical insurance were the main factors influencing the residents’ satisfactory degree of community health services (P=0.050, 0.001, and 0.001). The proportions of disaster / disaster prevention education, exercises of post-disaster contingency plans, and psychological intervention as well as rehabilitation for residents were 37.4%, 10.6%, and 12.6%, respectively. Conclusion Community health services have not been widely accepted by community residents, and the satisfactory degree is low. The residents’ understanding and adaptation can be improved by strengthening the community health service propaganda. Strengthening health education, improving the quality of services, controlling costs, and introducing medical insurance reimbursement mechanism can increase the residents’ satisfactory degree. Including disaster emergencies into basic tasks can strengthen the emergency response capability and then provide guarantee for the residents’ health.

        • Investigation on the economic burden of occupational exposure of medical personnel in medical institution

          ObjectiveTo investigate the economic burden of the disposal of medical personnel after occupational exposure in medical institution.MethodA retrospective survey method was used to investigate and analyze the costs of inspection, treatment, and vaccination of 112 medical personnel after occupational exposure from January 2014 to December 2016.ResultsAfter the occupational exposure, the per capita economic burden on the medical institution for the exposed personnel was (331.80±66.16) yuan; the main expenditure item was the inspection expenses (93.53%); and the expenditure after the exposure of hepatitis B virus accounted for 75.40%, which ranked the top one in the expenditures of blood-derived infectious diseases. Occupational exposure was most likely to occur when dealing with sharps, and the cost of hospital spending accounted for 47.26%. After strengthening interventions such as standard prevention, the per capita cost of occupational exposures in the hospital from 2015 to 2016 showed a downward trend (P<0.05).ConclusionsOccupational exposure of medical personnel may cause a certain economic burden on medical institution. It is necessary to strengthen occupational protection, raise awareness of standard prevention, and reduce occupational exposure.

        • Investigation on the Incidence of Urolithiasis in Infants Fed by Milk Powder Tainted with Melamine in the Middle Area of Anhui Province

          Objective To investigate the incidence of urolithiasis in infants aged 0-3 years old fed by milk powder tainted with melamine in the middle area of the Anhui province, as well as its relationship to gender, age, milk powder consumption and drinking water. Methods Questionnaires were distributed to 1079 infants who were screened for urolithiasis by ultrasound examination from September 25, 2008 to October 17, 2008. Data was collected by Epidata and analyzed by SPSS 13.0 software. Results A total of 646 (59.87%) male infants and 433 (40.13%) female infants underwent ultrasound examination in Anhui Provincial Hospital. Of these, 86 infants were diagnosed with urolithiasis with an incidence of 7.97%, including 62 males (72.09%) and 24 females (27.91%). The mean age of those infants with urolithiasis was 1.85±0.77, and all of calculus was located in kidney. The relationship between the incidence of urolithiasis and gender, age, drinking water, feeding bottle sanitation, birth status, as well as the amount of milk powder intake was assessed by using the Pearson Chi-square test. Results showed that significant differences were noted in the incidence of urolithiasis among infants of different genders or with different drinking water sources (Plt;0.05). The result of multiple logistic regression analyses indicated that gender was related to the incidence of urolithiasis (Plt;0.05). The incidence of urolithiasis in female infants was only 58.7% of that in male infants (OR 0.587, 95%CI 0.359 to 0.959). Conclusion  The incidence of urolithiasis in infants aged 0-3 years of old in the middle area of Anhui province is relatively high and has anatomical specificity. Further data during the follow-up of these cases should be collected.
