ObjectiveTo investigate the economic burden of the disposal of medical personnel after occupational exposure in medical institution.MethodA retrospective survey method was used to investigate and analyze the costs of inspection, treatment, and vaccination of 112 medical personnel after occupational exposure from January 2014 to December 2016.ResultsAfter the occupational exposure, the per capita economic burden on the medical institution for the exposed personnel was (331.80±66.16) yuan; the main expenditure item was the inspection expenses (93.53%); and the expenditure after the exposure of hepatitis B virus accounted for 75.40%, which ranked the top one in the expenditures of blood-derived infectious diseases. Occupational exposure was most likely to occur when dealing with sharps, and the cost of hospital spending accounted for 47.26%. After strengthening interventions such as standard prevention, the per capita cost of occupational exposures in the hospital from 2015 to 2016 showed a downward trend (P<0.05).ConclusionsOccupational exposure of medical personnel may cause a certain economic burden on medical institution. It is necessary to strengthen occupational protection, raise awareness of standard prevention, and reduce occupational exposure.
Objective To investigate the construction and services of the community health service system in Shifang, as well as the satisfaction of community residents with the community health services and the post-disaster emergency response capability of the community hospital, so as to provide decision-making suggestions on better reconstruction of the community health service system and improvement of its post-disaster emergency response capability. Methods There were 4‰ of community residents in Fangting town were selected by convenience sampling for a face-to-face interview using a questionnaire. Logistic regression was used to identify the influencing factors of residents’ satisfaction with community health services. Results A total of 250 questionnaires were conducted for face-to-face interviews, and 246 ones were retrieved (response rate 98.4%). Residents’ understanding and satisfactory degree of the community health service were 41.1% and 36.6%, respectively. Health education, medical expenses and medical insurance were the main factors influencing the residents’ satisfactory degree of community health services (P=0.050, 0.001, and 0.001). The proportions of disaster / disaster prevention education, exercises of post-disaster contingency plans, and psychological intervention as well as rehabilitation for residents were 37.4%, 10.6%, and 12.6%, respectively. Conclusion Community health services have not been widely accepted by community residents, and the satisfactory degree is low. The residents’ understanding and adaptation can be improved by strengthening the community health service propaganda. Strengthening health education, improving the quality of services, controlling costs, and introducing medical insurance reimbursement mechanism can increase the residents’ satisfactory degree. Including disaster emergencies into basic tasks can strengthen the emergency response capability and then provide guarantee for the residents’ health.
ObjectiveTo evaluate the burden of carbapenem-resistant Klebsiella pneumoniae (CRKPN) and carbapenem-resistant Escherichia coli (CRECO), two types of carbapenem-resistant Enterobacteriaceae (CRE), in pediatric patients in Jiangxi Province.MethodsA retrospective investigation was carried out for the distribution of CRKPN/CRECO in pediatric (neonatal group and non-neonatal group) and adult patients in 30 hospitals in Jiangxi Province from January 2016 to December 2018, and the changing trends and detection situations of different patients and types of hospitals were compared and analyzed.ResultsFrom 2016 to 2018, the annual resistance rates of Klebsiella pneumoniae and Escherichia coli to carbapenem in pediatric patients were 5.89%, 4.03%, and 4.24%, respectively, showed a downward trend (χ2trend=5.568, P=0.018). The resistance rate of Klebsiellae pneumoniae and Escherichia coli to carbapenem in neonatal group was higher than that in non-neonatal group (8.44% vs. 3.40%; χ2=63.155, P<0.001) and adult group (8.44% vs. 3.45%; χ2=97.633, P<0.001). In pediatric patients, the 3-year carbapenem resistance rate of Klebsiella pneumoniae was higher than that of Escherichia coli (9.10% vs. 2.48%; χ2=128.177, P<0.001). In non-neonatal pediatric patients, the 3-year resistance rate of Klebsiella pneumoniae and Escherichia coli to carbapenem in maternity and children hospitals was higher than that in general hospitals (4.35% vs. 1.36%; χ2=25.930, P<0.001). CRKPN/CRECO detected in pediatrics were mainly isolated from sputum (31.64%), blood (24.36%), urine (13.82%), and pus (8.36%).ConclusionAlthough the overall resistance rate of Klebsiella pneumoniae and Escherichia coli to carbapenem in pediatric patients showed a downward trend, that in neonatal patients was still high, and the monitoring and prevention and control measures of CRE should be strengthened in neonatal patients.
ObjectiveTo understand the current situation of hospital infection management related to the hospital maternity ward, learn the risks of infection, discover problems existing in hospital infection management, and seek appropriate solutions for hospital infection. MethodsBetween January 2011 and December 2012, using uniform questionnaire for prospective survey and field interviews, we investigated the hospital infection situation in medical health care workers, pregnant women, and live newborns. ResultsA total of 2 225 questionnaires were retreated with a retreat rate of 100%. Hospital infection occurred in 23 cases, of which 15 cases were maternal infection (1.35%) and 8 cases were neonate infection (0.72%). Maternal infection was mainly focused on reproductive tract, surgery incision and urinary tract. Neonate infection was mainly focused on lower respiratory tract and skin. The management system of hospital infection in the maternity ward was basically strengthened, but the infection monitoring work was still not timely, and staff training in infection control knowledge was not in place. The overall environment, sterilization, disinfection and isolation should be strengthened, and there were also some other safety hazards. ConclusionThe management of infection in hospital maternity ward is the key to control the infection in maternity ward. The infection management seems perfect, but there are still some hidden dangers. The management system needs to be improved and the management should be implemented strictly according to the system, in order to avoid the occurrence of infection in maternity ward and ensure the safety of patients.
ObjectiveTo investigate scientific research situation of postgraduates in the West China School of Medicine of Sichuan University. MethodsA self-designed questionnaire was used in an investigation concerning scientific research situation among 352 medical postgraduates in the West China Medical School of Sichuan University. Then SPSS 15.1 software was used to analyze the data. ResultsMost experiments, that the postgraduates took, involved multiple technologies, of which, molecular techniques were the most widely-used (reaching 35.5%). Sixty-eight percent students maintained that clinical practice, teaching/learning and scientific research were very important. The most frequently-encountered problem were research design, experimental techniques and methods, the contact with laboratory, and statistical analysis. When encountering the problems, 51.1% postgraduates turned to tutors, and 40.1% dealt with it by themselves. ConclusionIt is suggested that, cultivating scientific research ability and awareness of medical postgraduates should be strengthened, and platforms should be set up for medical postgraduates to carry out research.
ObjectiveTo investigate on preoperative hand-washing for analyzing the underlying problems, in purpose of preventing operation-related infection via strengthening the standardization of surgical hand-washing and hand-disinfection. MethodsFrom July to September 2014, surveillance video of pre-operative hand-washing was studied and analyzed. We randomly selected 100 subjects, and assessment was performed based on the self-made surgical hand-washing and hand-disinfection protocol. Grading was done strictly. The passing score was 60 or higher; a score of 70-79 meant good; and a score of 80-100 indicated excellence. ResultsIn the survey, 91 in the 100 subjects were qualified. Among the qualified subjects, there were 53% of excellence and 20% of good. The major problems included erroneous methods, step omissions, time lacking, pollution, wrong flushing method or dry-hand method, and insufficient washing time. ConclusionBased on the survey, the administration department is advised to strengthen the regulatory supervision and education training based on standard surgical hand-washing and hand-disinfection criteria, especially training interns and new staff, in purpose of reducing the incidence of operation-related infection and improving hospitalization management qualities.
ObjectiveTo investigate the current situation of ketogenic diet treatment centers in China, including the target, quantity, indication diseases, staffing and regional distribution of ketogenic diet therapy centers.MethodsMembers from the China Association Against Epilepsy (CAAE) received an investigation on the development situation of ketogenic diet therapy in their own epilepsy centers through an online survey. ResultsAt present, there are 91 centers for ketogenic diet therapy in China, and among them, there are 74 centers (81.32%) only treat pediatric patients. 59 centers of them (64.83%) were established between 2010 and 2020; patients of 63 centers (69.23%) have been treated with ketogenic therapy for status epilepticus (SE), among which only 9 centers (14.29%) have 5 or more epileptic patients on averge per month. 35 centers (38.46%) are offering ketogenic diet treatment for indications other than epilepsy. The main diseases are autism spectrum disorder, encephalitis, febrile infection related epilepsy syndrome (FIRES), rare diseases and glycolipid metabolic diseases. All ketogenic diet treatment centers are equipped with high-quality professionals, mainly neurologists and pediatric physicians; There are significant differences in regional distribution, with centers in the eastern region accounting for more than half (53.85%). ConclusionsThe development of ketogenic diet therapy in China has achieved initial results, but there are still some problems such as small number of patients treated and uneven geographical distribution.
ObjectivesTo analyze patients’ values and preferences on individualized medication of high-dose methotrexate so as to support the development of the practice guideline for clinical medication of high-dose methotrexate.MethodsA multicenter cross-sectional study involving patients with osteosarcoma or hematological malignancy in 7 hospitals was conducted by questionnaires to evaluate the perception and willingness on detection of gene polymorphisms (MTHFR C677T, MTHFR A1298C, ABCB1 C3435T and RFC1 G80A) related to methotrexate (MTX) and therapeutic drug monitoring (TDM) of MTX. SPSS24.0 software was used to analyze the data.ResultsA total of 124 patients were involved, including 40 (32.26%) with osteosarcoma and 84 (67.74%) with hematological malignancy. 106 (85.48%) and 117 (94.35%) patients agreed on detection of gene polymorphisms and TDM, respectively. There was a significant difference on preference towards TDM between patients with risk factors for MTX and patients in which risk factors for MTX were not discovered (76.19% vs. 95.08%, P=0.003). The ranking of factors that contributed to the two decision-making was consistent (P<0.01), and specific orders of factors were identical. The clinical efficacy was the primary factor (mean rank 3.45 for detection of genetic polymorphisms and 3.52 for TDM), followed by safety (mean rank 3.01 and 3.16, respectively) and comfort (mean rank 1.73 and 1.79, respectively). Cost (mean rank 1.39 and 1.31, respectively) was the least important factor.ConclusionsThe preferences of patients toward detection of gene polymorphisms and TDM were generally similar, with well acceptance. No significant differences were found on the preferences toward detection of gene polymorphisms. However, patients with or without risk factors for MTX may differ significantly when making decisions on TDM, which may impact on clinical decision-making of clinicians and clinical pharmacists. The perception and willingness of patients should be considered adequately during the development of clinical practice guidelines and clinical practice.
ObjectiveTo understand the current status of nosocomial infections in a municipal medical unit, to find problems in daily monitoring, and to provide the evidence for the prevention and control of nosocomial infection in high-risk groups and procedures. MethodsAccording to the 2013 survey requirements made by the nosocomial infection control center of Chengdu, we made a cross-sectional survey about nosocomial infection among all the inpatients on July 25th, 2013 and statistically analyzed the results. ResultsTotally, 1 301 cases were actually investigated within 1 307 inpatients of the time (the actual investigation rate was 99.54%). The prevalence rate of nosocomial infection was 3.38% (44 cases). The top five departments of infection rate were Respiratory Intensive Care Unit (RICU), Center of Intensive Care Unit (CICU), Department of Neurosurgery, Department of Thoracic Surgery and Department of General Surgery. The main site of infection was respiratory tract, which took possession of 62.25%. In univariate analyses, age≥60, length of hospital stay >2 weeks, invasive operation, history of diabetes, operation, radiotherapy/chemotherapy, utilization of antibiotics were found to be risk factors for infections. Multivariate analysis showed that length of hospital stay (OR=3.115, P=0.001), invasive operation (OR=14.930, P<0.001), diabetes mellitus (OR=2.157, P=0.046), radiotherapy/chemotherapy (OR=7.497, P<0.001) were independent risk factors for nosocomial infections. The utilization rate of antibiotics was 45.73%. Among them, there was 85.21% using single antibiotics, and 82.18% of them were used therapeutically. ConclusionUnderstanding the current status of nosocomial infection in municipal hospitals helps us grasp the key and difficulty of infection control, make out prevention-control measures for high-risk groups and high-risk departments, and guide and supervise its implementation in clinical departments, which points out the direction to further reduce the incidence of nosocomial infection.
