        • Disease Spectrum and Cost Constitution of Patients Aged over 65 Years with Respiratory Diseases in A Tertiary Hospital from 2010 to 2014

          ObjectiveTo investigate the disease spectrum and cost constitution of patients aged over 65 years with respiratory diseases in Sichuan Provincial People's Hospital from 2010 to 2014. MethodsThe inpatients' clinical data and costs were collected from hospital information system. The diseases were classified and the cost constitution were analyzed. The data were analyzed using SPSS 18.0 software. ResultsThe total number of the inpatients aged over 65 years was 17600 from 2010 to 2014, with more males (64.0%) than females (36.0%). The patients aged 76.9 years on average and were mainly in the 65-89 age group which accounted for 94.4%. The patients were mainly distributed in geriatric department, respiratory department and emergency department, accounting for 78.2% of the total. The top three respiratory diseases were pneumonia, chronic obstructive pulmonary disease (COPD) and pulmonary tumor accounting for 86.5% of the total with more males than females. There was an upward trend in the proportion of pulmonary tumor, bronchiectasis and pulmonary tuberculosis, and a downward trend in the proportion of asthma, COPD and pneumothorax. The average length of hospital stay decreased from 17.6 days to 16.0 days since 2010, and the average cost per capita increased from ¥20162.6 yuan to ¥30015.1 yuan since 2010. The proportion of drug cost to inpatients hospitalization cost decreased from 57.2% to 48.8% since 2010. ConclusionsPneumonia, COPD and pulmonary tumor are main respiratory diseases of inpatients over 65 years in Sichuan Provincial People's Hospital from 2010 to 2014. The male inpatients are more than female inpatients. The incidence of pulmonary tumor and pulmonary tuberculosis rises, and of asthma and COPD goes down. The incidence of lung tumor is increasing in younger age groups. The average length of hospital stay and the proportion of drug cost have declined in recent five years, while the average cost per capita has increased.

        • Analysis of the Muscle Fatigue Based on Band Spectrum Entropy of Multi-channel Surface Electromyography

          Exercise-induced muscle fatigue is a phenomenon that the maximum voluntary contraction force or power output of muscle is temporarily reduced due to muscular movement. If the fatigue is not treated properly, it will bring about a severe injury to the human body. With multi-channel collection of lower limb surface electromyography signals, this article analyzes the muscle fatigue by adoption of band spectrum entropy method which combined electromyographic signal spectral analysis and nonlinear dynamics. The experimental result indicated that with the increase of muscle fatigue, muscle signal spectrum began to move to low frequency, the energy concentrated, the system complexity came down, and the band spectrum entropy which reflected the complexity was also reduced. By monitoring the entropy, we can measure the degree of muscle fatigue, and provide an indicator to judge fatigue degree for the sports training and clinical rehabilitation training.

        • Association between folic acid supplementation during pregnancy and the risk of autism spectrum disorder in the offspring: a meta-analysis

          ObjectiveTo systematically review the association between maternal folate supplementation during pregnancy and the risk of autism spectrum disorder (ASD) in the offspring.MethodsPubMed, EMbase, Web of Science, The Cochrane Library, Scopus, CNKI, WanFang Data, VIP and CBM databases were electronically searched to collect case-control and cohort studies on the association between maternal folate supplementation during pregnancy and the risk of ASD in the offspring from inception to December 2020. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies. Meta-analysis was then performed using RevMan 5.3 and Stata 14.0 software.ResultsA total of 17 studies involving 10 812 cases and 876 241 controls were included. The results of meta-analysis showed that there was no significant association between maternal folate supplementation during pregnancy and the risk of ASD in the offspring in the total population. The subgroup analysis revealed that maternal folate supplementation during pregnancy was statistically associated with a reduced risk of ASD in the offspring in the Asian population (OR=0.71, 95%CI 0.53 to 0.96, P=0.03). However, there were no statistical correlations in European and American populations.ConclusionsCurrent evidence shows that maternal folate supplementation during pregnancy may reduce the risk of ASD in the offspring in the Asian population. Due to limited quality and quantity of the included studies, more high-quality studies are required to verify the above conclusions.

        • The impact of disease-related group payment methods on the diagnosis and treatment of inpatient medical insurance patients with neuromyelitis optica spectrum disorders in Xi'an and its improvement strategy

          ObjectiveTo preliminary investigate the impact of the diagnosis-related groups (DRG) payment method reform on the diagnosis and treatment of inpatient medical insurance patients with neuromyelitis optica spectrum disorders (NMOSD), and to propose potential improvement strategies. MethodsA single-center, retrospective study. From October 1, 2020, to September 30, 2022, 44 hospitalized medical insurance patients with acute-phase NMOSD diagnosed and treated at the First Affiliated Hospital of Northwest University (Xi'an First Hospital) were included in the study. Among them, there were 11 males and 33 females, with an average age of (40.8±20.2) years. According to the implementation time of DRG payment, patients were divided into two groups: group A, which consists of cases one year before the implementation of DRG payment from October 1, 2020 to September 30, 2021, and group B, which consists of cases one year after the implementation of DRG payment from October 1, 2021 to September 30, 2022, with 20 and 24 cases, respectively. Detailed information such as hospitalization duration, treatment methods, and hospitalization costs of the two groups of patients was collected. Comparative analysis was conducted on hospitalization costs and treatment methods between the two groups. For intergroup comparison, t-test was used for normally distributed data, and Mann-Whitney U test was used for skewed distributed data. ResultsAmong the 44 patients, 5 cases (5/24, 20.8%) received plasma exchange (PE) treatment, all of whom were in group B. The numbers of patients who received and did not receive intravenous immunoglobulin (IVIG) treatment were 9 and 11 in group A, respectively, and 7 and 12 in group B (except for 5 cases who received PE treatment), respectively. Compared with group A, there was no significant decrease in hospitalization duration (t=0.004) and total hospitalization costs (Z=0.036), as well as costs for western medicine (Z=0.036), examinations (Z=0.011), laboratory tests (Z=0.040), treatments (Z=0.017), and nursing (Z=3.131) in group B, and the differences were not statistically significant (P>0.05). For patients receiving PE treatment, except for the cost of western medicine (Z=0.062, P=0.804), the other costs (Z=8.288, 5.013, 11.400, 10.925, 9.126) were significantly higher than those of patients not receiving PE treatment, and the hospitalization duration (t=20.474) was significantly prolonged, with statistically significant differences (P<0.05). The total hospitalization costs of patients receiving IVIG treatment were significantly higher than those not receiving IVIG treatment in both group A and group B, with statistically significant differences (Z=7.690, 10.314; P<0.05). There was no statistically significant difference in the comparison of total hospitalization costs between patients receiving IVIG treatment in group A and group B (Z=0.137, P>0.05). ConclusionsThere is no significant decrease in various hospitalization costs of NMOSD medical insurance patients in Xi'an after the implementation of DRG payment, especially for patients receiving PE treatment. It is suggested to optimize the rate stratification of NMOSD patients when implementing DRG payment methods.

        • Prediction of recurrence of paroxysmal atrial fibrillation based on RR interval

          Atrial fibrillation (AF) is one of the most common arrhythmias, which does great harm to patients. Effective methods were urgently required to prevent the recurrence of AF. Four methods were used to analyze RR sequence in this paper, and differences between Pre-AF (preceding an episode of AF) and Normal period (far away from episodes of AF) were analyzed to find discriminative criterion. These methods are: power spectral analysis, approximate entropy (ApEn) and sample entropy (SpEn) analysis, recurrence analysis and time series symbolization. The RR sequence data used in this research were downloaded from the Paroxysmal Atrial Fibrillation Prediction Database. Supporting vector machine (SVM) classification was used to evaluate the methods by calculating sensitivity, specificity and accuracy rate. The results showed that the comprehensive utilization of recurrence analysis parameters reached the highest accuracy rate (95%); power spectrum analysis took second place (90%); while the results of entropy analyses and time sequence symbolization were not satisfactory, whose accuracy were both only 70%. In conclusion, the recurrence analysis and power spectrum could be adopted to evaluate the atrial chaotic state effectively, thus having certain reference value for prediction of AF recurrence.

          摘要:目的: 探討基層醫院急診“120”院前救治水平的影響。 方法 :分析301例死因,年齡及百分比。 結果 :急診致死原因疾病病譜前9位分別是交通事故,猝死,溺水,意外傷害,自殺,電擊傷,刀傷,一氧化碳中毒及呼吸道梗塞,“無名氏”群體86例占285%,這部分給臨床和社會帶來了新問題,這仍有待今后繼續探討。 結論 :完善基層醫院“120”體系,提高救治水平。Abstract: Objective: To explore the factors relate to prehospital“120”Emergency Medical Services cases death. Methods : Analysis the cause for death, ages and percentage of the age in 301 cases. Results : Traffic accident, sudden death, drowns, accident, suicide, electric burn, knife trauma, anthracemia and respiratory tract obstruction were the leading cases in the diseases spectrum of the cause for death in emergency cases. 86 cases were anonym, which brought new problems to clinic treatment and provoked social issue, accounting for 285% in total cases. These remain us to study henceforth. Conclusion : Consummate the “120” Emergency MedicalServices system in grassroots hospital and enhance the level of cure.

        • Gravity Frequency and Its Monitoring Application of EEG Spectrum in the Vigilance Operation

          It is an important means to study the electrical activity of the brain's nerve cells by exploring physiological information of the EEGs from the frequency domain. The gravity frequency is one of the global parameters with using this method. We used the multitaper spectrum method (MTM) spectrum estimation method of good performance to calculate the EEG spectrum and its gravity frequency of subjects under vigilance and vigilance decrement state. The results showed that the gravity frequency of vigilance state was higher than that of vigilance decrement state, the gravity frequency became smaller along with the vigilance decrement, and the location of the gravity frequency shifted to the left in the spectrum. Finally, the monitoring curve of the gravity frequency was acquired by designing an algorithm, and it was used to online monitoring vigilance operators.

        • Feasibility Study of Electroencephalogram Power Spectrum Analysis Monitoring Noninvasive Intracranial Pressure

          ObjectiveTo investigate the feasibility of electroencephalography (EEG) power spectrum analysis monitoring noninvasive intracranial pressure (ICP). MethodsBetween September 2008 and May 2009, the EEG signals were recorded in 62 patients (70 cases/times) with central nervous system (CNS). By using self-designed software, EEG power spectrum analysis was conducted and pressure index (PI) was calculated automatically. ICP was measured by lumbar puncture (LP). ResultsThe mean ICP was (239.74±116.25) mm H2O (70-500 mm H2O, 1 mm H2O=0.009 8 kPa), and 52.9% of patients had increased ICP. The mean PI was 0.29±0.20 (0.02-0.85). The Spearman rank test showed that there was a significant negative correlation between PI and ICP (rs=-0.849, P<0.01). The data from the patients with diffuse lesions of CNS and focal lesions were analyzed separately; the results showed there were significant negative correlations between PI and ICP in both groups (rs=-0.815, -0.912; P<0.01). ConclusionThe PI obtained from EEG analysis is correlated with ICP. Analysis of specific parameters from EEG power spectrum might reflect the ICP. Further research should be carried out.

        • Study of the Algorithm for Inversion of Low Field Nuclear Magnetic Resonance Relaxation Distribution

          It is difficult to reflect the properties of samples from the signal directly collected by the low field nuclear magnetic resonance (NMR) analyzer. People must obtain the relationship between the relaxation time and the original signal amplitude of every relaxation component by inversion algorithm. Consequently, the technology of T2 spectrum inversion is crucial to the application of NMR data. This study optimized the regularization factor selection method and presented the regularization algorithm for inversion of low field NMR relaxation distribution, which is based on the regularization theory of ill-posed inverse problem. The results of numerical simulation experiments by Matlab7.0 showed that this method could effectively analyze and process the NMR relaxation data.

        • Risk Factors and Genotyping of Nosocomial Pneumonia Due to Extended Spectrum β-Lactamase Producing EscherichiaColi and KlebsiellaPneumoniae

          Objective To analyze the current drug resistance and risk factors of hospital acquired pneumonia( HAP) due to extended spectrumβ-lactamase ( ESBLs) producing Escherichia coli and Klebsiella pneumoniae, and to estimate the prevalence trend of ESBLs producing strains. Methods FromApril 2007 to January 2008, 140 patients of Xinhua Hospital with HAP due to E. coli and K. pnermoniae were enrolled.Among them, 88 patients were with ESBLs producing strains and 52 patients were with non-ESBLs producing strains. Risk factors were analyzed by comparing between these patients. The rate of drug resistance was determined by antibiotic sensitive test. Fifty-three ESBLs producing strains were genotyped by random amplified polymorphic DNA ( RAPD) . Results The rate of drug resistance of ESBLs producing strains washigher than that of non-ESBLs producing strains. ICU stay, use of third- and forth-generation cehpalosporin were found to be the independent risk factors by multivariate analysis with logistic regression. By RAPD, 37 ESBLs producing E. coli strains were divided into 27 types and 16 ESBLs producing K. pneumoniae strains were divided into 13 types. Conclusions ICU stay, use of third-generation and forth-generation cehpalosporin remain as major risk factors in the HAP due to ESBLs producing E. coli and K. pneumoniae.RAPD is an economic, quick and credible method for epidemic analysis
