This study aims to explore the differences of event related potential (ERP) between attention deficit hyperactivity disorder (ADHD) and normal children, so that these differences provide scientific basis for the diagnosis of ADHD. Eight children were identified to be ADHD group by the diagnostic criteria of DSM IV (diagnostic and statistical manual of mental disorders IV), and the control group also consisted of 8 normal children. Modified visual continuous performance test (CPT) was used as the experiment paradigm. The experiment included two major conditions, i.e. Go and NoGo. All the 16 subjects participated in the study. A high density EEG acquisition instrument was used to record the EEG signal and processed these EEG data by means of ERP and spectrum analysis. P2 N2 peak peak value and spectral peak around 11 Hz were analyzed between ADHD subjects and those in the control group, and then statistical tests were applied to these two groups. Results showed that: ① Under the condition of Go, ADHD group had a significant lower P2 N2 peak peak value than the values in the control group ( P< 0.05); but under the condition of NoGo there was no significant difference in between. ② Compared with the control group, the ADHD group had significant lower spectral amplitude around 11 Hz under the condition of NoGo ( P< 0.05). However, under the condition of Go the difference was insignificant. In conclusion, there is certain cognitive dysfunction in ADHD children. P2-N2 peak-peak value and spectral peak around 11 Hz could be considered as clinical evaluation indexes of ADHD children′s cognitive function. These two objective indexes provide an early diagnosis and effective treatment of ADHD .
Autism spectrum disorder (ASD) is a neurodevelopmental disorder characterized by deficits in social communication and repetitive behaviors. With the rapid development of computer vision, visual behavior analysis aided diagnosis of ASD has got more and more attention. This paper reviews the research on visual behavior analysis aided diagnosis of ASD. First, the core symptoms and clinical diagnostic criteria of ASD are introduced briefly. Secondly, according to clinical diagnostic criteria, the interaction scenes are classified and introduced. Then, the existing relevant datasets are discussed. Finally, we analyze and compare the advantages and disadvantages of visual behavior analysis aided diagnosis methods for ASD in different interactive scenarios. The challenges in this research field are summarized and the prospects of related research are presented to promote the clinical application of visual behavior analysis in ASD diagnosis.
Seizures during sleep increase the probability of complication and sudden death. Effective prediction of seizures in sleep allows doctors and patients to take timely treatments to reduce the aforementioned probability. Most of the existing methods make use of electroencephalogram (EEG) to predict seizures, which are not specific developed for the sleep. However, EEG during sleep has its characteristics compared with EEG during other states. Therefore, in order to improve the sensitivity and reduce the false alarm rate, this paper utilized the characteristics of EEG to predict seizures during sleep. We firstly constructed the feature vector including the absolute power spectrum, the relative power spectrum and the power spectrum ratio in different frequencies. Secondly, the separation criterion and branch-and-bound method were applied to select features. Finally, support vector machine classifier were trained, which is then employed for online prediction. Compared with the existing method that do not consider the characteristics of sleeping EEG (sensitivity 91.67%, false alarm rate 9.19%), the proposed method was superior in terms of sensitivity (100%) and false alarm rate (2.11%). This method can improve the existing epilepsy prediction methods and has important clinical value.
Objective To summarize the principle and application of functional MR imaging of pancreatic carcinoma and chronic mass-forming type pancreatitis. Methods Articles about diffusion-weighted imaging (DWI), magnetic resonance spectrum imaging (MRSI) and dynamic contrast-enhanced MR imaging of pancreatic carcinoma and chronic pancreatitis were reviewed and analyzed. Results Functional MR imaging could reflected the differences in molecules diffusion, metabolism and tissue perfusion between pancreatic carcinoma and chronic pancreatitis. Conclusion As a non-invasive protocol, functional MR imaging can provide useful information in differential diagnosis between chronic mass-forming type pancreatitis and pancreatic carcinoma.
ObjectiveTo investigate the feasibility of electroencephalography (EEG) power spectrum analysis monitoring noninvasive intracranial pressure (ICP). MethodsBetween September 2008 and May 2009, the EEG signals were recorded in 62 patients (70 cases/times) with central nervous system (CNS). By using self-designed software, EEG power spectrum analysis was conducted and pressure index (PI) was calculated automatically. ICP was measured by lumbar puncture (LP). ResultsThe mean ICP was (239.74±116.25) mm H2O (70-500 mm H2O, 1 mm H2O=0.009 8 kPa), and 52.9% of patients had increased ICP. The mean PI was 0.29±0.20 (0.02-0.85). The Spearman rank test showed that there was a significant negative correlation between PI and ICP (rs=-0.849, P<0.01). The data from the patients with diffuse lesions of CNS and focal lesions were analyzed separately; the results showed there were significant negative correlations between PI and ICP in both groups (rs=-0.815, -0.912; P<0.01). ConclusionThe PI obtained from EEG analysis is correlated with ICP. Analysis of specific parameters from EEG power spectrum might reflect the ICP. Further research should be carried out.
Atrial fibrillation (AF) is one of the most common arrhythmias, which does great harm to patients. Effective methods were urgently required to prevent the recurrence of AF. Four methods were used to analyze RR sequence in this paper, and differences between Pre-AF (preceding an episode of AF) and Normal period (far away from episodes of AF) were analyzed to find discriminative criterion. These methods are: power spectral analysis, approximate entropy (ApEn) and sample entropy (SpEn) analysis, recurrence analysis and time series symbolization. The RR sequence data used in this research were downloaded from the Paroxysmal Atrial Fibrillation Prediction Database. Supporting vector machine (SVM) classification was used to evaluate the methods by calculating sensitivity, specificity and accuracy rate. The results showed that the comprehensive utilization of recurrence analysis parameters reached the highest accuracy rate (95%); power spectrum analysis took second place (90%); while the results of entropy analyses and time sequence symbolization were not satisfactory, whose accuracy were both only 70%. In conclusion, the recurrence analysis and power spectrum could be adopted to evaluate the atrial chaotic state effectively, thus having certain reference value for prediction of AF recurrence.
Objective To observe the effect of intravenous methylprednisolone (IVMP) pulse therapy on the best corrected visual acuity (BCVA) and the number of relapses in patients with neuromyelitis optica spectrum disorder-related optic neuritis (NMOSD-ON) after total IVMP dose. MethodsA retrospective clinical study. From March 2020 to February 2023, 23 patients of 27 eyes with NMOSD-ON in Shanxi Eye Hospital were included in the study. BCVA examinations were performed on all affected eyes using the international standard visual acuity chart, which was statistically converted into logMAR visual acuity. Serum aquaporin-4 antibody (AQP4-IgG) was detected by indirect immunofluorescence assay based on cell detection technology in all patients. According to Guideline for the diagnosis and treatment of NMOSD spectrum disorders in China (2021 edition), patients were given IVMP impact therapy. Among them, 18 and 5 patients received 1 000 and 500 mg/d IVMP pulse therapy respectively for 3-5 consecutive days, followed by a reduction to 500 or 250 mg/d for 2-3 consecutive days. The average total IVMP dose during the treatment was 4 500 mg (1 500-5 250 mg). The changes in BCVA at 1 week, 1 month, 3 months, and 6 months after treatment were observed for the initial and post-treatment BCVA of ≤0.1, >0.1-<0.5, and ≥0.5. The changes of BCVA at 1 week and 1, 3 and 6 months after treatment were observed. The comparison of BCVA between different age, disease duration, and IVMP total dose conditions was performed using the Mann-Whitney U test. The comparison of BCVA between different relapse times was performed using the Kruskal-Wallis test. The influence of IVMP total dose on the number of relapses during the 6-month follow-up was analyzed using χ2 test. The factors affecting BCVA ≥0.5 after 6 months of IVMP treatment were analyzed by logistic regression, and the correlation between ΔlogMAR BCVA and IVMP pulse total dose was analyzed by Spearman correlation. ResultsIn 23 cases with 27 eyes, there were 3 males and 20 females. The median age was 35 years. The median duration of illness was 5 days. There were 21 (91.30%, 21/23) positive and 2 (8.70%, 2/23) negative cases of AQP4-IgG, respectively. There were 3 cases (13.04%, 3/23) with the first course of disease and 4 eyes (14.81%, 4/27). There were 20 cases (86.96%, 20/23) with recurrence course and 23 eyes (85.19%, 23/27). The median time from initial onset to the initiation of corticosteroid treatment was 7 days. During the 6-month follow-up after treatment, 5 patients (21.74%, 5/23) relapsed in 6 eyes (22.22%, 6/27), all of which were patients with initial relapse course. Among them, recurred 1 or ≥2 times in 4 (66.67%, 4/6) and 2 (33.33%, 2/6) eyes respectively. BCVA≤0.1, >0.1-<0.5, ≥0.5 in 20, 4, 3 eyes and 3, 13, 11 eyes at the beginning and 6 months after treatment, respectively. There was significant difference in the number of eyes with BCVA≤0.1, >0.1-<0.5 and ≥0.5 at different time after treatment (χ2=40.772, P<0.001). The treatment effect of female patients was better than that of male patients. The patients with initial BCVA≥0.1 had more increased eye number of BCVA than those with BCVA<0.1, the patients with first course of disease had more increased eye number of BCVA than those with recurrent course of disease, and the patients with total dose of IVMP >4 500 mg had less increased eye number of BCVA than those with total dose ≤4 500 mg. The differences were statistically significant (Z=?2.449, ?2.904, ?2.485, ?2.286; P=0.014, 0.004, 0.013, 0.022). Logistic regression analysis showed that the higher the initial BCVA≤0.1 and the total impact dose of IVMP, the lower the possibility of obtaining BCVA≥0.5 after treatment (odds ratio=0.069, 0.899; 95% confidence interval 0.010-0.463, 0.798-0.998; P=0.006, 0.020). Spearman correlation analysis showed that ΔlogMAR BCVA was negatively correlated with total impact dose of IVMP (rs=?0.472, P=0.013). There was no significant difference in the number of recurrence after different total doses of IVMP (P>0.05). ConclusionsIVMP total dose ≤4 500 mg can achieve better BCVA prognosis compared with IVMP total dose >4 500 mg. IVMP total dose has no effect on the number of recurrences after treatment.
Objective To study the influence of the immersed time by hydrogen dioxide on the characteristics of bovine cancellous bone granules in various periods. Methods Ten 24-month-old Qinchuan bovine, male or female, weighing 150-170 g, were selected. Cancellous bone granules from metaphysic of bovine long bone were cut into cubes of 5 mm × 5 mm ×5 mm and immersed by 8.8 mol/L hydrogen dioxide for 0, 12, 24, 36, 48, 60 and 72 hours separately. Determination of ash, scanning electron microscope, X-ray energy spectrum and micro CT were used to investigate the changes of composition, structure and qual ity of bone. Results With the immersed time increasing, the contents of organics in the bone cancellous were reduced gradually, and obviously decreased during the periods of 0 to 24 hours and 60 to 72 hours (P lt; 0.05). The contents of calcium and phosphorus decreased gradually, they could not be detected almost after 60 days (P lt; 0.05). Bone mineral density and bone mineral content were decreased obviously after 60 hours (P lt; 0.05). The bone trabecula became sl immer and trabecular spacing became larger. Conclusion Hydrogen dioxide can be used to remove the antigen in xenogeneic bone; however as the time increasing (more than 60 hours) the composition and structure will be damaged. Thus it is important to control the immersed time for maintaining the biological characteristics of xenogeneic bone substitute as well as el iminating antigen by hydrogen dioxide.
ObjectiveTo investigate the epidemiological characteristics of patients in the emergency observation room of Beijing Shijitan Hospital during the last 5 years, to enhance the local and regional construction of education discipline and improve the quality of medical services.MethodsA retrospective study was carried out on selected patients admitted to the emergency observation room of the Capital Medical University Affiliated Beijing Shijitan Hospital between January 1st 2014 and December 31st 2018. Statistical analysis was performed to identify associated epidemiological features in these patients.ResultsAmong the 19 518 patients admitted to the emergency observation room, 49.08% were males and 50.92% were females, with a male to female ratio of 1:1.04. The 46-65 age group had the lowest proportion of females of 47.28%, while this proportion reached to 55.38% in the over-80 age group, which was higher than that in the 46-65 and 66-80 age groups, showing statistical significance (P<0.05). During the last 5 years, 59.66% of the patients were aged above 65; the proportion of over 65 years old patients admitted to the emergency observation room of the hospital each year was 53.86%, 53.19%, 57.00%, 65.40%, and 68.79%, respectively, showing an increasing trend (P=0.011). Among the patients diagnosed, 24.20% had respiratory system diseases, 23.67% had circulation system diseases, 14.47% had digestive system diseases, 12.75% had neurological system diseases, and 7.08% had endocrine system diseases. According to the primary diagnosis, the top 10 most predominant diseases were bacterial pneumonia (14.72%), acute and chronic heart failure (7.86%), hypertension (5.00%), arrhythmia (4.02%), coronary atherosclerotic heart disease (3.92%), chronic obstructive pulmonary disease (3.01%), cerebrovascular disease (2.68%), gastrointestinal hemorrhage (2.66%), acute diabetes (2.43%), and celiac infection (1.73%), which accounted for 48.02% of the total patients admitted. In the winters (December to February) of these 5 years, the total number of patients admitted was 6 145, which contributed to 31.48% of the total admission number, and hence making winter the season with the highest number of patients seeking for medical services.ConclusionsAccording to the spectrum of the disease features, there is a need to enhance the training of specialty and emergency clinics associated with respiratory, circulation, neurological, digestive, and endocrine diseases. Special attention should be paid for treatment and health care targeted at old-aged patients. During winter, the peak hospital visiting time, appropriate medical care service, which addresses the plan of working time, should be provided in order to optimize diagnosis and treatment processes.
摘要:目的: 探討基層醫院急診“120”院前救治水平的影響。 方法 :分析301例死因,年齡及百分比。 結果 :急診致死原因疾病病譜前9位分別是交通事故,猝死,溺水,意外傷害,自殺,電擊傷,刀傷,一氧化碳中毒及呼吸道梗塞,“無名氏”群體86例占285%,這部分給臨床和社會帶來了新問題,這仍有待今后繼續探討。 結論 :完善基層醫院“120”體系,提高救治水平。Abstract: Objective: To explore the factors relate to prehospital“120”Emergency Medical Services cases death. Methods : Analysis the cause for death, ages and percentage of the age in 301 cases. Results : Traffic accident, sudden death, drowns, accident, suicide, electric burn, knife trauma, anthracemia and respiratory tract obstruction were the leading cases in the diseases spectrum of the cause for death in emergency cases. 86 cases were anonym, which brought new problems to clinic treatment and provoked social issue, accounting for 285% in total cases. These remain us to study henceforth. Conclusion : Consummate the “120” Emergency MedicalServices system in grassroots hospital and enhance the level of cure.
