Objective To detect the expressions of osteopontin (OPN), breast tumor kinase (Brk), and vascular endothelial growth factor (VEGF) in the breast cancer tissue, the adjacent (2cm) normal breast tissue, and the distal(>5cm) normal breast tissue, and analyze their clinical significances. Method The immunohistochemical method was used to detect the expressions of OPN, Brk, and VEGF in the breast cancer tissue, the adjacent (2cm) normal breast tissue, and the distal (>5cm) normal breast tissue from 40 cases of breast cancer. Results ① The expressions of OPN,Brk, and VEGF in the breast cancer tissue were significantly higher than those of the adjacent (2cm) normal breast tissue and the distal (>5cm) normal breast tissue (P<0.01), the expression of Brk in the adjacent (2cm) normal breast tissue was significantly higher than that of the distal (>5 cm) normal breast tissue (P<0.05). ② In the breast cancer tissue, the OPN and Brk protein expressions were not associated with age, tumor diameter, and histological grade (P>0.05),were associated with lymph node metastasis and TNM stage (P<0.05). The VEGF protein expression was not associated with age and tumor diameter (P>0.05), but was associated with histological grade, lymph node metastasis, and TNM staging (P<0.05). ③ In the breast cancer tissue, OPN, Brk, and VEGF had positive correlation with each other (P<0.05), but not in the adjacent (2cm) normal breast tissue and the distal (>5 cm) normal breast tissue (P>0.05). Conclusions The expressions of OPN and Brk from the same signal pathway increase by turns in the distal (>5 cm) normal breast tissue, adjacent (2cm) normal breast tissue, and breast cancer tissue. OPN induced the adhesion and migration of endothelial cells to accelerate vascular repair through VEGF and Brk has correlation with the progress of tumor invasion and metastasis through participating in tumor vascularization.
ObjectiveTo investigate clinical characteristics of patients with malignant cardiac tumors, and summarize our diagnostic methods and surgical treatment experience. MethodsClinical data of 16 patients with mali-gnant cardiac tumors who were admitted to Department of Cardiovascular Surgery, Second Affiliated Hospital of Harbin Medical University between January 2005 and February 2012 were retrospectively analyzed. There were 8 male and 8 female patients with their age of 35-64 (47.8±10.9) years and disease duration of 15 days to 48 months (11.8±10.9) months. Among the 16 patients, 13 patients underwent tumor resection under cardiopulmonary bypass (CPB), including 1 patient who received complete resection of the tumor and pedicle surrounding tissue and other 12 patients who only received partial tumor resection. One patient underwent concomitant tricuspid valve replacement. ResultsAll the operations were performed successfully. One patient died of low cardiac output syndrome and multiple organ dysfunction syndrome on the 6th postoperative day. All the other patients were successfully discharged. Average operation time was 181.2±59.5 minutes, average CPB time was 68.8±20.8 minutes, and average length of hospital stay was 20.4±7.4 days. Postoperative pathological examination showed primary cardiac malignant tumors in 9 patients, metastatic malignant tumors in 6 patients, and benign tumor with malignant growth in 1 patient. Three patients were followed up for a short time (≥3 months) and were still alive. Nine patients died in 1-14 months after discharge. One patient with benign cardiac tumor but malignant growth was still alive. ConclusionMalignant cardiac tumors are rare but highly malignant with a high rate of misdiagnosis. Surgical outcomes are comparatively satisfactory, but these patients' prognosis is usually poor.
Objective To study the advances in the relationship between the number of infiltrating dendritic cells and the postoperative prognosis of digestive malignant tumor. MethodsThe literature in recent years on the relationship between the number of infiltrating dendritic cells and the postoperative prognosis of digestive malignant tumor was reviewed.ResultsThe number of infiltrating dendritic cells among esophageal cancer,and gastric carcinoma,colonic cancer and pancreatic cancer was associated with a better prognosis.Conclusion The population density of dendritic cells among the malignant tissue could be regarded as an independent indicator in estimating the postoperative prognosis of malignant tumor.
Objective To introduce the experience and comprehensionto reconstruct nasal defect after tumor excision. Methods FromApril 1996 to April 2006, based on the aesthetic subunit principle and according to the size, shape, location of nasal defect and the conditions of surrounding skin, homologous local flap was selected to cover the nasal defect in 428 cases which nasal tumors were removed. Among 428 cases, there were 273 men and 155 women, with a median age of 52 years (12-78 years); including 146 cases of basal cell carcinoma, 83 cases of squamous cell carcinoma, 54 cases of epidermal cyst, and 145 cases of pigmented naevus.The clinical stage of malignant tumor was 0-Ⅰstage, the course of disease was 1 week to 3 months. The locations were nasal tip in 51 cases, nasal ala in 102 cases, dorsum of nose in 138 cases, and nasal side in 137 cases, across 2 nasal subunits in 83 cases. The area of thedefect ranged between 0.6 cm×0.6 cm and 3.0 cm×4.0 cm. The origin of flaps was frontonasal flap in 58 cases, bilobed flap in 67 cases, reforming rhomboid flap in 152 cases, nasolabial flap in 118 cses, forehead falp in 33 cases. The secondary defect of donor site was directly sutured. Results Among 428 cases, 423 cases acquired complete recovery; 3 cases which had epiderm necrosis over the far end of the flap achieved healing by the first intention and 2 cases which had suffered low-grade infection of incision achieved healing by the second intention after regional change dressings.The nasal defect was successfully repaired in all patients,and the all flaps survived. A total of 385 patients were available forfollow-up of 1 to 60 months, no tumor recurrence occurred, and the repaired tissue were good match with surrounding tissue, good nasal contour was obtained, the cosmetic results were satisfactory. Conclusion Based on the nasal aesthetic subunit principle, the local flap can reconstruct the nasal above medial defect, and a good color, contour and texture match with the surrounding skin can be obtained, the cosmetic results are satisfactory.
Objective To evaluate the role of contrastenhanced CT (CECT) scanning in the diagnosis and treatment planning of some rare liver tumors. MethodsWe retrospectively reviewed the CECT imaging features of 10 cases with rare tumors of the liver proved by surgical pathology in 8 cases or by liver biopsy in 2 cases, with correlation of relevant clinical manifestations, laboratory results and surgical findings. ResultsThree cases were histopathologically proved to be benign, and 7 cases were malignant. On CT images, the liver lesions were mixed cysticsolid in 5 cases, totally solid in the other 5. Eight cases demonstrated heterogeneous enhancement, while 2 cases of liver lymphoma had no enhancement. The anatomic relationship of tumors to intrahepatic vasculature, the compression and infiltration of neighboring abdominal structures were accurately delineated by CT as compared with findings at operation. Clinical manifestations and laboratory findings were not useful for the qualitative diagnosis of rare liver tumors, except for hepatocellular carcinoma. ConclusionCECT is very useful for the detection of rare liver tumors and the fine depiction of local extent of these tumors. When correlated with clinical and laboratory information, it helps to differentiate rare liver tumors from hepatocellular carcinoma. But its role in the characterization of rare liver tumors is limited.
Purpose To investigate nucleoside diphosphate kinase (NDPK ) expression of tumor metastasis suppressor gene nm23 in heterotransplanted model of retinoblastoma(RB) in nude mice,and analyse the correlation between the expression of nm23 gene and the formation and progression of heterotran splanted RB. Methods SP immunohistochemical method was used to detect the expression of nm23 gene product NDPK in 20 tumors of heter otransplanted RB model and normal retinal tissue. Results The negative staining of nm23/ NDPK was found in normal retinal tissue , whereas 100% expression rate in RB tumors with positive number of 48.73plusmn;2.37. No statistical significance of the expression of nm23/ NDPK was observed between the intraocular growth phase (I~Ⅲ grade) and invasive phase ( Ⅳ~Ⅴ grade)in heterotransplantedRB tumors. Conclusion The function of nm23 gene as a tumor metastasis suppressor in heterotra nsplanted RB tumors was less prominent ,but it may play a role in carcinogen esis and progrssion of RB and may predict poor prognosis. ObjectiveTo explore feasibility of mRNA vaccines as a novel strategy for individualized precision treatment of pancreatic cancer (PC). MethodThe recent domestic and international literature on vaccine research in PC was reviewed. ResultsThe heterogeneity between and within the pancreatic tumors had limited the efficacy of traditional vaccines based on cells, exosomes, proteins, peptides, or DNA for PC. The mRNA vaccine was considered as a promising alternative therapy due to its precise targeting, low toxicity, and ability to induce long-lasting immune memory. Breakthroughs in the tumor antigen recognition, immune subtype differentiation, and mRNA vaccine construction, the development strategy of PC mRNA vaccine would further facilitate the development of personalized precision medicine. The existing mRNA vaccines usually need to be combined with other immunotherapy methods to improve efficacy, while the development of preventive vaccines is still exploraing. ConclusionsmRNA vaccines, as an innovative and promising platform, offer a new hope for the development of PC vaccines. However, the heterogeneity of PC has resulted in poor efficacy with traditional vaccines. Although the limitations of traditional vaccines and the heterogeneity of PC itself, the more challenges of vaccine research of PC is facing, the advantages of mRNA vaccine still make it possible to treat PC. In the face of the challenge of complex characteristics of PC, more research is needed to support the transformation and application of mRNA vaccine in clinical therapy.
Objective To analyze the effectiveness of free vascularized fibula grafts (FVFG) for extensive bone defects after resection of lower limb malignant bone tumors. Methods Between November 2015 and July 2018, 15 cases of lower limb malignant bone tumors were treated. There were 12 males and 3 females with an average age of 12.3 years (range, 9-21 years). There were 11 cases of osteosarcoma and 4 cases of Ewing’s sarcoma. The tumor located at middle femur in 8 cases, lower femur in 4 cases, and middle tibia in 3 cases. The disease duration was 2-6 months (mean, 3.2 months). The tumor was completely removed, and the length of the bone defect was 8-23 cm (mean, 17.7 cm). The bone defect was repaired by FVFG, and combined inactivated tumor bone was used in 8 cases of femoral bone defect. Results The average operation time was 280 minutes (range, 180-390 minutes). The average blood loss was 310 mL (range, 200-480 mL). All incisions healed by first intention. All patients were followed up 2-24 months with an average of 14.5 months. Bone healing achieved in all patients at 9-18 months (mean, 12.3 months) after operation except 1 patient which was followed up only 2 months. The fibula grafts had active metabolism and the average bone metabolism score was 184 (range, 111-257) in effected side and 193 (range, 127-259) in contralateral side. There was no difference between bilateral sides. The average Enneking score was 24.6 (range, 20-30) at last follow-up. No ankle instability or paralysis of common peroneal nerve occurred. Conclusion FVFG appeared very efficient in repair of extensive bone defect after resection of lower limb malignant bone tumor.
ObjectiveTo analyze the electro-clinical characteristics and surgical outcome of low-grade developmental tumors in temporal lobe. MethodsThe onset age, seizure duration, seizure types, electroencephalogram and surgical outcome of 49 patients with low-grade developmental tumor of temporal lobe were analyzed retrospectively. ResultsTwo groups of the seizure types were divided. The first group was spasm, the other was focal onset. There were 12 cases in spasm group, with an average onset age of (1.00±0.59) years. The discharge was extensive and multi-brain-area locaded, especially in the temporal montages and the ipsilateral posterior montages. There were 37 cases in second group, with an average onset age of (8.90±8.84) years, mainly including autonomic seizure, tonic seizure and automotor seizure. In this group, the discharge was mainly recorded in the temporal montages, which could spread to the frontal montages and less locaded in posterior montages. The difference of onset age between the two groups was statistically significant (P<0.01). The average follow-up of spasm group was (2.80±1.57) years, and the surgical outcome of all patients in this group were all Engel I (100.00%, 12/12). The focal onset group was followed up for an average of (6.50±4.78) years, and the rate of Engel I was 91.80% (34/37). There was no significant difference between the two groups (P>0.05). ConclusionsFor low-grade developmental tumors in temporal lobe, there are two seizure types, including spasm and focal onset. The onset age of spasm is earlier, while patients with focal onset mostly start at childhood or older, rare in infancy. Surgery has a good effect on the treatment of temporal lobe developmental tumor epilepsy.
ObjectiveTo study the clinical manifestations, pathologic characteristics, imaging features, diagnosis and treatment of adenomas of extrahepatic bile duct.MethodsTwo cases of adenomas of extrahepatic bile duct in our hospital and 14 cases reported in the literatures were analyzed retrospectively.ResultsThe patients’(male 5, female 11) mean age was 58.4 years (range 21-85). The main manifestations included jaundice (n=11), abdominal pain (n=8),fever (n=6),dyspepsia (n=4),body weight loss (n=3) and claycolored stool (n=1). The locations of tumors were in the left hepatic duct (n=1), right hepatic duct (n=3), hepatic common bile duct(n=3),the junction of cystic duct and common bile duct (n=1),distal common bile duct (n=8). The pathologic types were tubular adenomas (n=5), papillary (villous) adenomas (n=10),and mucous adenoma (n=1). All the patients underwent surgical therapy. The tumors were identified by postoperative histopathologic examination.ConclusionIt is difficult to correctly diagnose adenomas of extrahepatic bile duct before operation, because the clinical manifestations are usually atypical. The definite diagnosis should depend on histopathologic examination. It is the key to completely resect the tumors. Postoperative followup should be done on regular basis.
