        • Clinical Outcomes and Predictive Factor Analysis of Mild-to-moderate or Moderate Functional Mitral Regurgitationafter Aortic Valve Replacement in Patients with Severe Aortic Stenosis

          Objective To evaluate clinical outcomes of mild-to-moderate or moderate functional mitral regurgitation(FMR)after aortic valve replacement (AVR) in patients with severe aortic stenosis (AS),and analyze prognostic factors of these patients with mild-to-moderate or moderate FMR (2+to 3+). Methods From September 2008 to December 2011,a total of 156 patients with severe AS (peak aortic gradient (PAG)≥50 mm Hg) as well as FMR (2+to 3+) underwent surgical treatment in Zhongshan Hospital. There were 95 male and 61 female patients with their average age of 59.2±10.5 years. Detailed perioperative clinical data were collected,and postoperative patients were followed up. The ratio of FMRpreoperative/FMR postoperative was calculated. Patient age,gender,body weight,history of hypertension,ventricular arrhythmia,atrial fibrillation (AF),left ventricular ejection fraction (LVEF),left ventricular end-diastolic diameter (LVEDD),left atrial diameter (LAD),pulmonary artery hypertension (PAH),PAG were assessed by logistic multivariate regression analysis. Results Six patients died postoperatively,including 4 patients with low cardiac output syndrome and 2 patients with refractory ventricular arrhythmia. Perioperative mortality was 3.8%. The average follow-up time was 20.3±8.5 months and follow-up rate was 85.3% (133/156). Eight patients died during follow-up,including 3 patients with heart failure,2 patients with ventricular arrhythmia,and 3 patients with anticoagulation-related cerebrovascular accident. Multivariate regression analysis showed that FMR preoperative/FMR postoperative ratio was not correlated with age≥55 years,male gender,body weight≥80 kg,LVEDD≥55 mm,LVEF≤50%,history of hypertension or ventricular arrhythmia. However,LAD≥50 mm,PAH≥50 mm Hg,PAG≤75 mm Hg and preoperative AF were negatively correlated with postoperative FMR improvement. Conclusions Risk factors including LAD≥50 mm,PAH≥50 mm Hg,PAG≤75 mm Hg and preoperative AF are negatively correlated with postoperative improvement of FMR (2+to 3+). Patients with severe AS and above risk factors should receive concomitant surgical treatment for their FMR during AVR,since preoperative FMR(2+to 3+)usually does not improve or even aggravate after AVR.

        • APPLICATION OF ARTIFICIAL HUMERAL HEAD IN THE TREATMENT OF TUMOR OF UPPER END OF HUMERUS

          In order to restore the function of shoulder joint in patient with tumor of upper end of humerus, artificial humeral head replacement was performed. The materials included resinene, nylon-6 and large molecular polyethylene from 1978 to 1993, 14 patients were treated. The tumors involved in this group were giant cell tumor, synviosarcoma, bone cyst, osteochondroma, osteoblastoma, osteofibrosarcoma, osteosarcoma, chondrosarcoma, parosteosarcoma and malignant giant cell tumor. After resection of the tumor, the artificial prosthesis was implanted with bone cement. After 1 to 16 years follow-up, functions of the shoulder joint were reserved in 86% of the patients.

        • Surgical Treatment of Aortic Arch Diseases with Four Branches Aortic Graft

          Objective To summarize the methods and experiences of surgical treatment of aortic arch diseases with four branches aortic graft under deep hypothermia circulatory arrest (DHCA) and antegrade selective cerebral perfusion (ASCP). Methods In 2004 from September to December, surgical treatment of 12 patients with 7 aortic aneurysm(4 cases with ascending aorta and aortic arch aneurysm, 3 cases with aneurysm of aortic isthmus) and 5 aortic dissection(DeBakey Ⅰ 1 case, DeBakey Ⅱ 3 cases, DeBakey Ⅲ 1 case) were collected in Gunma Prefectural Cardiovascular Center. All operations were carried out under DHCA and ASCP, and four branches aortic graft were used to replace the aortic arch. The Bentall procedure, total and partial arch replacement and elephant trunk technique were undertaken in different patients. Results Total 12 patients recovered from the great vessel diseases smoothly without severe cerebral and other systematic complications, the time of operation was 5.5±1.7 h, the period of DHCA was 42.2±12.9min, 4 cases with no blood transfusion, the time of hospitalization was 22.3±7.2d. Conclusion ASCP is a safe. and effective method of cerebral protection during circulation arrest, and four branches aortic graft may shorten the time of DHCA and simplify the procedure of aortic arch replacement.

        • Effects of Hearing Reconstruction by Teflon Prothesis in Chronic Otitis Media

          【摘要】 目的 評價聚四氟乙烯人工聽骨在慢性中耳炎聽力重建中的應用效果。 方法 回顧性分析2008年8月〖CD3/5〗2009年12月慢性中耳炎手術中接受聚四氟乙烯部分/全人工聽骨贗復物(Partial/Total ossicular replacement prostheses,PORP/TORP,美國美敦力公司)行聽骨鏈重建的50例患者50只耳的臨床資料及隨訪結果。比較聽力學資料完整的39例39只耳手術前及手術后3~6個月語言頻率(05、1.0、2.0 kHz)氣導平均聽力及氣骨導差(airbonecap,ABG)。 結果 隨訪3~16個月,50只耳手術后1~3個月均干耳,手術后1個月鼓膜愈合良好。9只耳手術中人工聽骨表面未置入耳屏軟骨,其中1只耳手術后3個月人工聽骨脫出,1只耳手術后2個月鼓膜內陷,人工聽骨外端盤面外突。39只耳手術前氣導聽力為3667~9500 dB,平均為(5970±1433) dB;骨導聽力為330~5170 dB,平均為(2278±1161) dB;手術后氣導聽力平均為(4660±1703) dB,與手術前比較,差異有統計學意義(Plt;001)。手術前ABG為10~60 dB,平均為(3700±1212) dB。手術后ABG平均為(2620±1156) dB,與手術前比較,差異有統計學意義(Plt;001)。手術后氣導聽力提高1000~3000 dB者(手術后聽力提高有效)共24只耳,有效率為615%(24/39)。 結論 聚四氟乙烯人工聽骨是慢性中耳炎體重建聽骨的理想材料,手術后取得良好的聽力結果,遠期療效需進一步觀察。【Abstract】 Objective To evaluate the effects of hearing reconstruction in chronic otitis meadia with prothesis. Methods Retrospective reviews were performed in 50 patients(50 ears)who had undergone Teflon Partial/Total ossicular replacement prostheses (PORP/TORP) implants from August 2008 to December 2009. Audiometric measurements (the average hearing threshold at 05, 10, 20 kHz) were taken in 39 patients (39 ears) preoperatively and postoperatively. Comparisons of preoperative and postoperative mean air condition and air bone gap (ABG) were performed. Results All patients were followed up for three to 16 months, and the surgery resulted in a dry ear and a healing drum without perforation in all 50 patients. Of the nine patients without tragus cartilage interposed between the drum and the prosthesis, one patient occured prothesis extrusion, and one patient had retraction of the eardrum. The mean air conduction of the 39 patients before and after surgery were (5970±1433) dB and (4660±1703) dB. The mean air bone gap (ABG) was (3700±1212) dB before operation, while it was elevated to (2620±1156) dB when followed up (Plt;001). The surgical treatments were found to be successful (postoperative mean air condition improved 10.0030.00 dB) in 24 ears (615%). The effectiveness was 615%. Conclution Teflon could be an ideal material for ossicular reconstruction in chronic otitis media, and the hearing improvement is obtained after operation, but the problem of extrusion has not been solved at present, and the longterm effects should be surveyed.

        • Application of shared decision-making in the extended care of artificial joint replacement

          Objective To evaluate the effectiveness of the shared decision-making scheme in postoperative out-of-hospital extended care for patients with total hip/knee arthroplasty (THA/TKA). Methods Patients who underwent THA/TKA in the Department of Orthopedic Surgery at West China Hospital of Sichuan University between October 2023 and April 2024 were included using convenience sampling. Patients were divided into the control group (odd-numbered dates) and the intervention group (even-numbered dates) based on the surgical dates. The intervention group was received care guided by a shared decision-making protocol, whereas the control group was followed the standard post-arthroplasty follow-up procedures. Differences between the two groups were compared in terms of decision-making capacity, decision satisfaction, and hip/knee function at the following time points: on the day of discharge, 3 weeks after discharge, 2 months after discharge, and 3 months after discharge. Results A total of 118 patients were included, with 59 cases in each group. There were no significant differences in demographic and clinical characteristics between the two groups (P>0.05). In terms of decision-making, compared with the control group, the experimental group had stronger decision-making ability, lower decision-making conflict, and more satisfaction with the decision-making process (P<0.05). In terms of joint function, the experimental group showed better joint function than the control group at 3 weeks, 2 months, and 3 months after surgery (P<0.05). There was no significant difference in the pain dimension of the Western Ontario and McMaster Universities Osteoarthritis index between the two groups (P=0.199). Conclusions Compared with the traditional follow-up protocol, the shared decision-making protocol can enhance patient engagement in medical decision-making, reduce decisional conflict, improve satisfaction with the decision-making process, and simultaneously promote joint functional recovery and expedite the rehabilitation process.

        • Progress in Perioperative Treatments of Cardiac Valve Replacement during Pregnancy

          Abstract:  There is a 1% - 4% incidence of cardiac disease in pregnancy, and rheumatic heart disease is the most common diagnosis. On the condition that medical treatment is inefficacious, an open heart surgery should be performed. Because of the changes in physiological functions, there is a major difference between pregnant patients and the general population in perioperative management of valve replacement. Now , the pregnancy is not an absolute contraindication for cardiopulmonary bypass, though the incidence of mortality and birth defects remains high. It is helpful in decreasing complications and increasing survival rate by improving themethods of cardiopulmonary bypass, postoperative monitoring, anticoagulation etc. In this article, the progress in t reatments of perioperative period of cardiac valve replacement during pregnancy is reviewed.

        • PROGRESS OF RELATIONSHIP BETWEEN BIOFILM AND PROSTHETIC JOINT INFECTION

          Objective To summarize the effect of biofilm (BF) on the occurrence of prosthetic joint infection (PJI). Methods The domestic and abroad original l iterature in recent years about the relationship between BF and PJI was reviewed. Results Infection is a critical compl ication for prosthetic joint replacement. Basic research showes one of the reasons for PJI is BF. After adherence of the bacteria to the surface of prosthetic joint, BF forms through a series of regulation andcontrol system. And it lead to the occurrence of PJI. Recently a lot of progress have been made in the research fields of BF related PJI, which have covered aetiology, diagnosis, treatment, and prevention. Different studies show that BF has close relationship with PJI. Conclusion BF is proved to have close relationship with PJI. It is important on cl inical significances to diagnose, treat, and prevent PJI.

        • A Randomized Controlled Trial of Concomitant Tricuspid Annuloplasty for Patients Underwent Mitral Valve Replacement: UCG Evaluation 2 Years After Surgery

          Objective To evaluate the right ventricular function of the patients 2 years after surgery by ultrasonic cardiography (UCG) who underwent mitral valve replacement (MVR) concomitant tricuspid annuloplasty (TAP). Method We finally identified 36 patients required MVR with tricuspid valve annular dilation concomitant merely mild tricuspid regurgitaion (TR) based on preoperative UCG in our hospital between April and November 2012 year. All patients were randomly divided into two groups by digital table including a tricuspid annuloplasty group (a TAP group, n=18, 7 males and 11 females at age of 45.67±12.49 years) and a no-tricuspid annuloplasty group (a NTAP group, n=18, 6 males and 12 females at age of 45.44±10.48 years). General clinical data and extracorporeal circulation data were recorded. UCG evaluation was practiced preoperation, alone with 1 week, 6 months, and 2 years after surgery. Results Two years postoperative maximal long-axis of RA (RAmla), mid-RA minor distance (RAmmd), right ventricle dimension-1(RVD1) , right ventricular fractional area change (RVFAC), 3D RV end-systolic volume (3DRVESV), tricuspid valve annular end-diastolic dimension (TVAEDD), tricuspid valve annular end-systolic dimension (TVAESD) of patients were all smaller in the TAP group than those in the NTAP group. Yet right ventricular ejection fraction (RVEF), percent shorting of tricuspid valve annulus (PSTVA) were greater in the TAP group than those in the NTAP group, although there was no statistical difference between the two groups in two years postoperative 3D RV end-diastolic volume (3DRVEDV). The patients in the TAP group had a superior trend than that of the NTAP group. Moreover, the patients' TR constituent ratio in the TAP group was much better than that of the NTAP group in 2 year after operation. Conclusions Concomitant tricuspid annuloplasty for patients with tricuspid valve annulus dilation and mild TR underwent MVR is favorable for the recovery of their 2 years postoperative function of tricuspid valve and right ventricle. It is benefit to reduce patient's long term postoperative TR residues and exacerbation.

        • SHORT-TERM EFFECTIVENESS OF Swanson ARTIFICIAL JOINT REPLACEMENT IN TREATING POSTTRAUMATIC METACARPOPHALANGEAL JOINT STIFFNESS

          Objective To investigate the short-term effectiveness of Swanson artificial joint replacement in treating post-traumatic metacarpophalangeal joint stiffness. Methods Between August 2007 and May 2010, 11 cases (13 fingers) of metacarpophalangeal joint stiffness with soft tissue defects underwent Swanson artificial joint replacement. There were 7 males (9 fingers) and 4 females (4 fingers), aged 43 to 65 years with an average of 49 years. The involved fingers included 4 thumbs, 4 index fingers, 3 middle fingers, and 2 ring fingers. The types of injury included open and crush injury in 8 fingers, fracture of the metacarpophalangeal joint in 3 fingers, metacarpophalangeal joint severing in 2 fingers. The time from joint stiffness to hospitalization was 12 to 48 weeks (mean, 24 weeks). The joint activity was (136.82 ± 28.96)°. According to total active motion (TAM) assessment, included good in 1 finger, fair in 6 fingers, and poor in 6 fingers before operation. The activities of daily living were assessed by Sollerman score, which was 45.64 ± 11.04. The X-ray films and CT scan showed traumatic arthritis of the metacarpophalangeal joint. Results The incision healed by first intention. All patients were followed up 12 to 34 months (mean, 24.1 months). At last follow-up, the joint activity was (194.64 ± 28.86)°, showing significant difference when compared with preoperative value (t=25.214, P=0.000). According to TAM assessment, including excellent in 1 finger, good in 4 fingers, fair in 7 fingers, and poor in 1 finger. The Sollerman score was 67.45 ± 8.20 postoperatively, showing significant difference when compared with the preoperative score (t=10.470, P=0.000). X-ray examination showed no prosthesis fracture, periprosthetic fracture, or joint dislocation occurred at last follow-up. Conclusion Swanson artificial joint replacement can be appl ied to treat posttraumatic metacarpophalangeal joint stiffness, which can improve the joint activity and has satisfactory short-term effectiveness.
