        • Efficacy of LDL-C lowering therapy in patients with non-ST-segment elevation acute coronary syndrome: a meta-analysis

          Objective To investigate the efficacy of LDL-C lowering treatment on NSTE-ACS, and to analyze the target LDL-C level for clinical treatment. Methods PubMed, EMbase, the Cochrane Central Register of Controlled Trials, Web of Science databases were searched up to January 2016 for randomized controlled trials assessing the effects of LDL-C lowering therapy on major adverse cardiac events (MACE) in patients with NSTE-ACS. Two reviewers independently screened litertures, extracted data and assessed the risk of bias of included studies, and then meta-analysis was performed by using Stata12.0 and RevMan 5.3 software. Result A total of 12 RCT including 4 702 individuals with NATE-ACS were included. The results of meta-analysis showed that, compared with the control group, the statin group could significantly reduced the risk of MACE (RR=0.68, 95% CI 0.549 to 0.834,P=0.000). With 18.68 months of follow-up, patients in target LDL-C level from over 70 mg/dL to less than 100 mg/dL group had lower risk of MACE than other LDL-C level group. When LDL-C lower 20% to 40% than baseline with 28.99 months follow-up, patients in target of LDL-C level from over 70 mg/dL to less than 100 mg/dL group had lowest risk of MACE (RR=20.143, 95% CI 6.946 to 58.414,P=0.000). Conclusion LDL-C lower treatment can lower the risk of MACE in patients with NSTE-ACS. Patients in target LDL-C level from over 70 mg/dL to less than 100 mg/dL group have relatively low risk of MACE, in which patients who lower 20% to 40% LDL-C than baseline will get more benefits from LDL-C lowering therapy.

        • Recruitment-inflation ratio-based analysis of prognostic risk factors and nomogram prediction model for acute respiratory distress syndrome

          Objective To establish and validate a recruitment-to-inflation (R/I) ratio-oriented nomogram prediction model for mortality risk and assess its prognostic value in patients with moderate to severe acute respiratory distress syndrome (ARDS). Methods A prospective observational study was conducted. Patients diagnosed with moderate to severe ARDS receiving mechanical ventilation in the ICU of the First Affiliated Hospital of Jinzhou Medical University between January 2024 and July 2025 were enrolled. They were divided into a training set and a validation set in a 7:3 ratio. Baseline data and respiratory mechanics parameters, including R/I ratio, best positive end-expiratory pressure (Best PEEP), stress index (SI), driving pressure (DP) , tidal volume (VT), respiratory rate (RR), mechanical power (MP), and oxygenation index (P/F ratio) measured 4 hours after mechanical ventilation. Based on 28-day clinical outcomes, the patients were categorized into a survival group and a non-survival group. Least absolute shrinkage and selection operator (Lasso) regression and multivariate logistic regression were used to identify risk factors for 28-day prognosis in ARDS patients, and a nomogram prediction model was constructed. The predictive value of the model for 28-day mortality was evaluated in the validation set using receiver operating characteristic (ROC) curves and calibration curves (discrimination and accuracy) for internal validation. Decision curve analysis (DCA) was applied to assess the clinical utility of the nomogram. Results A total of 207 patients with moderate to severe ARDS were included, with 145 in the training set and 62 in the validation set. In the training and validation sets, 44 (30.34%) and 21 (32.26%) patients died, respectively. Compared with the survival group, the non-survival group had higher Best PEEP, DP, RR, and MP, and lower R/I ratio, VT, and P/F ratio (all P<0.05). Lasso regression identified 7 potential predictors for 28-day prognosis from 8 clinical variables in the training set: R/I ratio, Best PEEP, DP, VT, RR, MP, and P/F ratio. Logistic regression results showed that R/I ratio (OR=0.014, 95%CI 0.000-0.924, P=0.014), Best PEEP (OR=1.171, 95%CI 1.001-1.370, P=0.048), DP (OR=1.199, 95%CI 1.009-1.426, P=0.039), VT (OR=0.991, 95%CI 0.982-1.000, P=0.046), RR (OR=1.461, 95%CI 1.222-1.745, P=0.009), MP (OR=1.150, 95%CI 1.005-1.315, P=0.042), and P/F ratio (OR=0.968, 95%CI 0.945-0.992, P= 0.012) were all independent risk factors for 28-day prognosis in the ARDS patients. A nomogram model was constructed using these variables. In both the training and validation sets, ROC curves showed that the area under the curve for predicting 28-day mortality was 0.884 (95%CI 0.828-0.939) and 0.814 (95%CI 0.691-0.937), respectively. Calibration curves demonstrated good consistency between predicted and actual mortality, indicating satisfactory accuracy. DCA results indicated that the nomogram prediction model provided greater net benefit than the "treat all" or "treat none" strategies. Conclusion The R/I ratio-oriented nomogram prediction model for mortality risk in patients with moderate to severe ARDS demonstrates good prognostic value.

        • Effectiveness of transverse double “8”-shaped tension band technique for Lawrence zoneⅠ fracture of the fifth metatarsal base

          Objective To explore the effectiveness of transverse double “8”-shaped tension band technique in the treatment of Lawrence zoneⅠfracture of the 5th metatarsal base. Methods Between February 2019 and October 2021, 15 patients with Lawrence zoneⅠfracture of the 5th metatarsal base were treated with transverse double “8”-shaped tension band technique. There were 8 males and 7 females, with a median age of 40 years (range, 23-59 years). The fractures were caused by sprains. The time from injury to operation was 3-7 days (mean, 4.1 days). X-ray films were taken to observe the fracture healing and the anchor looseness and detachment. The foot function was evaluated by American Orthopaedic Foot and Ankle Society (AOFAS) score, visual analogue scale (VAS) score, and the eversion angle of the calcaneal talus joint. Results The incisions healed by first intention after operation in 14 cases and the incision healed poorly in 1 case. All patients were followed up 8-12 months (median, 10 months). The imaging examination showed that all fractures healed well, with a healing time of 10-14 weeks (mean, 11.7 weeks). At last follow-up, AOFAS score was 82-100 (median, 98); 13 cases were excellent and 2 cases were good, with an excellent and good rate of 100%. VAS score was 0-3 (median, 1). Three cases had mild limited ankle joint range of motion, while 12 cases had normal range of motion. The eversion angle of the calcaneal talus joint was 25°-32° (median, 30°). Conclusion The application of transverse double “8”-shaped tension band technique for Lawrence zone Ⅰ fracture of the 5th metatarsal base has advantages such as simple operation, avoidance of secondary operation, and reduction of foreign body sensation, with definite effectiveness.

        • Clinical application of ultrasound guided Fogarty balloon catheter in arterial crisis

          Objective To explore the effectiveness of arterial crisis after replantation of limb treated by ultrasound guided Fogarty balloon catheter. Methods Between January 2012 and July 2016, 27 patients suffered from arterial crisis after replantation of limb were treated with ultrasound guided Fogarty balloon catheter combined with thrombolytic anticoagulant. There were 18 males and 9 females with the age of 19-51 years (mean, 32 years). The limb mutilation position was at knee joint in 3 cases, lower limb in 9 cases, ankle joint in 6 cases, elbow joint in 2 cases, forearm in 4 cases, and wrist joint in 3 cases. The arterial crisis happened at 2.5-18 hours (mean, 7.5 hours) after limb replantation surgery. Color doppler ultrasonography was used to diagnose the arterial thrombosis, finally the anastomotic thrombosis were found in 16 cases, non-anastomotic thrombosis in 7 cases, and combined thrombosis in 4 cases. All the thrombosis were deteced in the arteries with the length of 0.8-3.9 cm. Results No complication such as vascular perforation, rupture, air embolism, thromboembolism, wound infection, or sepsis happened after operation. Arterial crisis occurred again in 3 cases at 1.5-13.5 hours after limb replantation and treated by arterial exploration, 1 case was treated successfully; 2 cases had arterial occlusion and partial necrosis of limb, and got amputation treatment at last. The rest 24 cases survived with the incision healing by first stage. In the 24 cases, 1 case suffered from acute myonephropathic metabolic syndrome and corrected after hemodialysis; 1 case suffered from acute liver functional damage and corrected by comprehensive treatment of internal medicine. The 24 patients were followed up 7-38 months (mean, 11 months). At last follow-up, blood supply of the limb was good with normal skin temperature and improved sense of feeling, activity, and swelling. According to Chinese Medical Association of hand surgery to the upper extremity function assessment standard, the results were excellent in 12 cases, good in 8 cases, and fair in 4 cases with an excellent and good rate of 83.3%. Conclusion Ultrasound guided Fogarty balloon catheter treatment of posterior replantation of arterial crisis can accurately locate the thrombosis, get the thrombus fast and invasive minimally to avoid the blind and repeated thrombectomy, and obtain certain effectiveness.

        • Research on algorithms of uterine contraction curve analysis and its real-time status identification

          Identification of real-time uterine contraction status is very significant to labor analgesia, but the traditional uterine contraction analysis algorithms and systems cannot meet the requirement. According to the situations mentioned above, this paper designs a set of algorithms for the real-time analysis of uterine contraction status. The algorithms include uterine contraction signal preprocessing, uterine contraction baseline extraction based on histogram and linear iteration and an algorithm for the real-time analysis of uterine contraction status based on finite state machines theory. It uses the last uterine status and a series of state transfer conditions to identify the current uterine contraction status, as well as a buffer mechanism to avoid false status transitions. To evaluate the performance of the algorithm, we compare it with an existing uterine contraction analysis algorithm used in the electronic fetal monitor. The experiments show that our algorithm can analyze the uterine contraction status while monitoring the uterine contraction signal in a real-time. Its sensitivity reaches 0.939 9 and its positive predictive value is 0.869 3, suggesting that the algorithm has high accuracy and meets the need of clinical monitoring.

        • EFFECT OF GLUCOCORTICOID ON EXPRESSION LEVELS OF OSTEOPROTEGERIN/RECEPTOR ACTIVATOR OF NUCLEAR FACTOR KAPPA B LIGAND-MATRIX METALLOPROTEINASE TISSUE INHIBITOR OF MATRIX METALLOPROTEINASE SYSTEM IN BONE TISSUES OF FEMORAL HEAD OF RATS

            Objective?To investigate the effect of glucocorticoid on the expression levels of osteoprotegerin (OPG)/receptor activator of nuclear factor kappa B ligand (RANKL)-matrix metalloproteinases (MMP)/tissue inhibitor of matrix metalloproteinase (TIMP) system in bone tissues of femoral head of rats, and to discuss its interrelated action mechanism in glucocorticoid-induced avascular necrosis of femoral head (ANFH).?Methods?Forty adult Sprague Dawley rats, weighing 250-300 g, half males and half females, were randomly divided into 4 groups: high dose glucocorticoid group (HD, n=10), medium dose glucocorticoid group (MD, n=10), low dose glucocorticoid group (LD, n=10), and control group (n=10). The rats in HD group, MD group, and LD group were intramuscularly injected with 25.0, 12.5, and 7.0 mg/kg of prednisolone respectively, and the rats in the control group were injected with physiological saline. After 4 weeks intervention, the osteonecrosis of left femoral heads was observed by HE staining, total RNA was extracted from the right femoral head bone tissue and the mRNA expression levels of OPG, RANKL, MMP-2, MMP-9, TIMP-1, and TIMP-2 were detected by RT-PCR.?Results?After injection of prednisolone, 4 rats of HD group and 1 rat of MD group died of systemic failure caused by the decreased food and weight culminating in cachexia. HE staining showed that the integrity of bone trabecula and osteon was destroyed at different levels, discontinuous bone chips formed, and osteocytes were replaced by granulation tissue in some lacunae in HD, MD, and LD groups; the integrated osteon was observed, the lamellar structure formed concentric circles around the blood vessel and osteocytes were seen in the lacunae in the control group. The necrosis rates of femoral head were 83.3% (5/6), 66.7% (6/9), 30.0% (3/10), and 0 (0/10) in HD, MD, LD, and control groups. The results of RT-PCR showed: the mRNA expression levels of the OPG, TIMP-1, TIMP-2 in HD, MD, and LD groups were lower than those in the control group, showing significant differences (P lt; 0.05) and there was negative correlation with the hormone dosage. The difference in OPG expression was significant between the hormone groups (P lt; 0.05); the differences in the TIMP-1 and TIMP-2 expressions were not significant between the LD group and MD group (P gt; 0.05), but there were significant differences when compared with HD group (P lt; 0.05). The RANKL, MMP-2, and MMP-9 mRNA expression levels in HD, MD, and LD groups were higher than those in the control group and there was a positive correlation with the hormone dosage, showing significant differences when compared MD and HD groups with control group (P lt; 0.05); there was no significant difference in RANKL expression between HD group and MD group (P gt; 0.05), but there was significant difference when compared HD and MD groups with LD group (P gt; 0.05); no significant difference was observed in the MMP-2 and MMP-9 expression between MD group and LD group (P gt; 0.05), but the differences were significant when compared with HD group (P lt; 0.05).?Conclusion?Glucocorticoid-induced ANFH may be related to the expression levels of OPG/RANKL-MMP/TIMP mRNA regulated by glucocorticoid.

        • Performance evaluation of a wearable steady-state visual evoked potential based brain-computer interface in real-life scenario

          Brain-computer interface (BCI) has high application value in the field of healthcare. However, in practical clinical applications, convenience and system performance should be considered in the use of BCI. Wearable BCIs are generally with high convenience, but their performance in real-life scenario needs to be evaluated. This study proposed a wearable steady-state visual evoked potential (SSVEP)-based BCI system equipped with a small-sized electroencephalogram (EEG) collector and a high-performance training-free decoding algorithm. Ten healthy subjects participated in the test of BCI system under simplified experimental preparation. The results showed that the average classification accuracy of this BCI was 94.10% for 40 targets, and there was no significant difference compared to the dataset collected under the laboratory condition. The system achieved a maximum information transfer rate (ITR) of 115.25 bit/min with 8-channel signal and 98.49 bit/min with 4-channel signal, indicating that the 4-channel solution can be used as an option for the few-channel BCI. Overall, this wearable SSVEP-BCI can achieve good performance in real-life scenario, which helps to promote BCI technology in clinical practice.

        • Quality assessment of clinical practice guidelines for benign prostatic hyperplasia

          Objectives To evaluate the methodological quality of published clinical practice guidelines for benign prostatic hyperplasia. Methods PubMed, EMbase, CNKI, WanFang Data, VIP and CBM databases, website of Yimaitong, and international authoritative guide platforms were electronically searched to collect the relevant clinical practice guidelines or consensus for benign prostate hyperplasia. The retrieval covered the time up to December 13th, 2016. Literatures were independently screened by 2 reviewers. After data extraction, the methodological quality of included guidelines was evaluated by 4 reviewers using the AGREE Ⅱ. Each domain score was calculated and the intraclass correlation coefficient was used to evaluate the consistency among the reviewers. Results A total of 15 clinical practice guidelines were included. The mean scores for the six domains in AGREE Ⅱ were: 72%, 38%, 30%, 58%, 16%, and 40%, respectively. The intraclass correlation coefficient was larger than 0.87, which indicated the total consistency was well. Conclusions The quality of clinical practice guidelines for benign prostatic hyperplasia is not satisfactory as expected. They are heterogeneous in quality and some requires improvement. Guidelines are required to be further developed in methodology in future, especially in three domains, including participants, preciseness and applicability of the design.

        • Examining the false-negative results of meta-analyses of Cochrane urology group using trial sequential analysis

          Objective To detect the false-negative results of cumulative meta-analyses of Cochrane Urology Group with the trial sequential analysis (TSA). Methods The Urology Group of The Cochrane Library (Issue 6, 2016) was searched to collect meta-analyses with negative results. Two researchers independently screened literature and extracted data of included meta-analyses. Then, TSA was performed using TSA software version 0.9 beta. Results A total of 11 papers involving 12 meta-analyses were included. The results of TSA showed that, four (33%) out of 12 meta-analyses were potentially false-negative results for failing to surpass the trial sequential monitoring boundary and to reach the required information size. Conclusion Some of the negative results of systematic reviews from Cochrane Urology Group was false-negative. TSA can help researchers to identify the false-negative results of meta-analyses.

        • REPAIR OF ACROMIOCLAVICULAR DISLOCATION WITH CLAVICULAR HOOK PLATE INTERNAL FIXATION AND CORACOACROMIAL LIGAMENT TRANSPOSITION

          To explore the operative method and the cl inical outcomes of repairing acromioclavicular dislocation by clavicular hook plate internal fixation and coracoacromial l igament transposition. Methods From August 2004 to December 2007, 12 cases of acromioclavicular dislocation were repaired with the internal fixation of clavicular hook plate and the transposition of coracoacromial l igament. There were 9 males and 3 females aged 22-56 years old (average 32 yearsold). Causes of injury: 6 cases from fall ing injury, 4 cases from crush injury and 2 cases from traffic accident. There were 5 cases of the left acromioclavicular dislocation, and 7 cases of the right. According to acromioclavicular dislocation classification set by WANG Yicong, 8 cases were graded as type III, 3 cases as type IV, and 1 case as type V. The time from injury to operation was 3-28 days (average 6 days). The injured arm was hung after operation, and the function training was started 3-5 days after operation. Results All wounds healed by first intention, and the X-ray films showed complete reposition of acromioclavicular joints was achieved in all cases 1 week after operation. Over the follow-up period of 12-30 months, no plate and screw loosening, hook break and acromion fracture occurred. At 2 months after operation, 2 patients had sl ight pain when moving the shoulder, and the symptom disappeared when removing the plate. No re-dislocation was observed in all cases after removing the plate at 6-10 months after operation. The function of shoulder joint was assessed by Karlsson evaluation standard 1 year after operation, 11 cases were graded as excellent and 1 case was good. Conclusion For the repair of acromioclavicular dislocation, the method of combining clavicular hook plate internal fixation with coracoacromial l igament transposition has the advantages of minor wound, easy operation, l ittle influence on the function of shoulder joints, and rel iable restoration of the stabil ity of shoulder joint.
