Objective To study the advances in clinical application of hernioplasty by high molecular material. Methods The literature in the recent years on the advances of hernioplasty by high molecular material was reviewed. Results At present time many operative techniques of hernioplasty by high molecular material have been developed. The representative techniques were ①Rives-Stoppa′s mesh inlay hernioplasty; ②Lichtenstein′s tesion-free herniorrhaphy; ③mesh plug hernioplasty; ④Gilbert′s sutureless hernioplasty; ⑤laparoscopic inguinal hernioplasty. The reparing high molecular material was divided into absorbable and unabsorbable material, the former included polyglycolic-acid and polyglaction, the later consists of polypropylene polyester and expanded polytetrafluoroethylene.Conclusion The clinical application of henioplasty by high molecular material is increasing. According to the hernia type and patient condition, excellent outcome will be achieved by the application of proper repairing method and repair material.
【Abstract】ObjectiveTo evaluate the therapeutic benefits of three styles of hernioplasties such as the traditional hernioplasty, the mesh only hernioplasty and the plug amp; mesh hernioplasty. MethodsThe traditional hernioplasty in 534 cases(583 sides), the mesh only hernioplasty in 57 cases(60 sides) and the plug amp; mesh hernioplasty in 51 cases(54 sides) were performed. The comparing studies on the operative time, the postoperative complications, the recurrent rate and so on were analyzed. ResultsThe average operative time of the traditional styles group was (34.26±4.56) min, which was significantly shorter than the mesh only group 〔(40.35±6.24) min, P<0.05〕 and the plug amp; mesh group 〔(49.12±8.69) min, P<0.01〕 respectively. This significant difference between the mesh only group and the plug amp; mesh group was also identified (P<0.05). Postoperative complications in the traditional styles group, the mesh only group and the plug amp; mesh group were 1.12%, 1.75% and zero,respectively (Pgt;0.05). Recurrent rate in the traditional styles group was 5.99%(32/534), which was significantly higher than that of the mesh only and the plug amp; mesh group (no recurrence). The average hospitalizing time in the traditional styles group, the mesh only group and the plug amp; mesh group was (7.11±3.06) days,(5.38±2.53) days and (6.19±3.61) days, respectively, in which there was no significant difference among groups. The activityrecovering time in the traditional styles group was (16.98±4.35) days, which was significantly longer than (7.26±2.46) days in the mesh only group and (8.02±3.35) days in the plug amp; mesh group. Conclusion The mesh only hernioplasty or the plug amp; mesh hernioplasty have a lower recurrent rate comparing with that in the traditional hernioplasty, which should be much more popularly applied in the treatment of inguinal hernia.
Objective To investigate the value of laparoscopy in resetting and repairing of inguinal incarcerated hernia. Methods Clinical data of 11 patients with acute inguinal incarcerated hernia treated with laparoscopy from March 2007 to February 2008 were reviewed, including 3 cases of direct inguinal hernia and 8 cases of oblique inguinal hernia. Results There were 7 patients with incarcerated hernia who were reset automatically after general anaesthesia, and 4 patients who were reset with laparoscopy. All the hernias were repaired with the transabdominal preperitoneal (TAPP) method after observed or improved the blood cycle of the incarcerated intestinal tract under laparoscopy. Average follow-up time was 6.6 months (2-12 months) after operation. Good recover and no recurrence were showed in all patients. Conclusion Laparoscopy is a safe technology in resetting and repairing of inguinal incarcerated hernia
Objective To explore the important role of preperitoneal space in laparoscopic transabdominal preperitoneal (TAPP) and totally extraperitoneal (TEP) hernia repair. Methods The clinical data of 66 patients (78 sides) performed laparoscopic TAPP and TEP hernia repair from January 2008 to April 2011 in this hospital were analyzed retrospectively. Results TAPP hernia repair were performed in 16 cases (20 sides),TEP hernia repair were performed in 50 cases (58 sides). Three cases of TEP hernia repair transferred to TAPP hernia repair. The unilateral operation time was (86.92±36.38) min,intraoperative bleeding was (6.08±3.43) ml. Postoperative complication rate was 16.7% (11/66),including 3 cases of postoperative serum swelling,3 cases of temporary paraesthesia of nerve feeling in the repair area,2 cases of scrotum emphysema,2 cases of urinary retention,and 1 case of intestinal obstruction. There were 2 cases of recurrence. The hospital stay was (4.52±0.99) d. The return to activities and working time was (10.32±1.86) d after discharge. Sixty-six cases were followed up for (18.56±1.96) months (range 1-38 months),the patch infection,chronic pain,and testicular atrophy complications were not been observed. Conclusions Acquainting and mastering laparoscopic preperitoneal space and its important structure are the key to avoid intraoperative and postoperative complications of laparoscopic inguinal hernia repair.
Objective To investigate the application of transverse fascia in inguinal hernia repair. Methods In this study, 617 patients underwent inguinal hernia repair between January 1990 and December 2005 in our hospital were included, which were divided into two groups according to different operative ways: transverse fascia method group (n=337) and Bassini method group (n=280). Then intraoperative results, postoperative complications, and rehabilitated results of patients in two groups were compared. Results Compared with Bassini method group, the patients in transverse fascia method group did not show significant difference in operative time and blood loss during operation (Pgt;0.05). The differences of severe postoperative pain, testicular swelling, the time of the body’s restore for normal activities, and recurrence rate of patients between two groups were significant (Plt;0.05), while the difference of hematoma of scrotum and infection of incisional wound (Pgt;0.05). Conclusion The strengthening of posterior wall by transverse fascia and reconstruction of inner ring is a simple and effective method for inguinal hernia repair.
ObjectiveTo retrospectively analyze the risk factors of recurrent groin hernia. MethodsData came from all hernia repairs recorded in our hospital between 1991 and 2001. A total of 1 082 cases of groin hernia was analyzed through Cox proportional hazard model to determine the relative risks between recurrence and mode of admission, types of groin hernia, repair methods (traditional/tensionfree hernioplasty), and postoperative complications.ResultsFrom 1991 to 2001,1 082 groin hernia operations were performed in our hospital, 88.9% for primary hernia and 11.2% for recurrent hernia. As compared with traditional hernia repair methods, Lichtenstein’s tensionfree hernioplasty technique had less relative risk for recurrent hernia repair than that for primary hernia repair. Postoperative complications and indirect hernia were linked to an increased relative risk for recurrence. ConclusionThere are many factors affecting the recurrence of inguinal hernia, but the use of tensionfree hernia repair may decrease rerecurrence of recurrent hernia.
ObjectiveTo investigate the clinical value of veress needle with thread for laparoscopic high ligation of hernia sac in children. MethodsThe clinical data of 56 cases of pediatric inguinal hernia using laparoscopic high ligation of hernia sac with veress needle from May 2013 to May 2014 were analyzed retrospectively. ResultsAll patients were successfully carried out operation by laparoscopic high ligation of hernia sac.Forty-one cases underwent unilateral surgery, and operative time was 6-18 min (mean 9.6 min); 15 cases underwent bilateral surgery, and operative time was 10-32 min (mean 14.3 min).Nine cases of contralateral recessive hernia were found and ligated during operation.There was no complications, such as scrotal edema, scrotal gas or incision infection after operation, and all patients were discharged in l-2 days after operation.No recurrence or testicular dysplasia was found during 3 to 15 months (mean 10.8 months) follow-up. ConclusionsThere are many advantages in laparoscopic high ligation of hernia sac with veress needle, such as less damage, hidden incision, faster recovery, shorter hospital stay, simple operation, and better efficacy.We can find contralateral recessive hernia during operation and avoid second surgery with high clinical value, therefore it is worthy of clinical application.
Objective To compare the feasibility, safety, and efficiency of laparoscopic total extraperitoneal (TEP)hernia repair surgery and laparoscopic transabdominal preperitoneal (TAPP) hernia repair surgery. Methods The clinical data of 95 patients with inguinal hernia who underwent laparoscopic TEP hernia repair surgery (TEP group) and TAPP hernia repair surgery (TAPP group) from Mar. 2010 to Oct. 2013 in our hospital were retrospectively analyzed, and clinical parameters including operation time, intraoperative blood loss, postoperative hospital stay, postoperative comp-lication, and operation cost of 2 groups were compared. Results All the procedures were successful, none of them was converted to open surgery. There was no significant difference between TEP group and TAPP group when considering operation time 〔(65±16) min vs.(68±17) min〕, intraoperative blood loss 〔(7.0±1.2) mL vs. (8.0±1.4) mL〕, visual pain analogue scale 〔(2.0±1.1) score vs. (1.8±1.1) score〕, postoperative hospital stay 〔(3.1±1.4) d vs. (3.3±1.2) d〕,and time to release to regular activities 〔(4.2±1.0) d vs. (4.5±1.2) d〕, P>0.05. But the operation cost of TEP group was significantly lower than that of TAPP group 〔(8 033±536) yuan vs. (9 632±643) yuan, P=0.007〕. There were 6 atients (6.3%, 6/95) suffered complications, 3 cases in TEP group and 3 cases in TAPP group, including 3 cases of seroma or hematoma in scrotum, 1 case of transient neurapraxia, and 3 cases of urinary retention. There was no signi-ficant difference in incidence rate of postoperative complication between the 2 groups (P=1.000). All patients were followed-up for 1-35 months 〔(20.0±10.2) months〕 without recurrence and chronic pain. Conclusions TEP and TAPP hernia repair surgery are feasible, safe, effective, and minimally invasive technique for inguinal hernioplasty. There are advantages and disadvantages of both TAPP and TEP hernia repair surgery, but there is no statistically significant difference regarding intraoperative and postoperative complications.
Objective To evaluate the clinical experience and skills of laparoscopic totally extraperitoneal hernia repair with non-stapling of mesh and without using a balloon dissection in order to spread and popularize this kind of operation. Methods Under general anesthesia, 32 patients (42 sides) with inguinal hernias were repaired by laparoscopic totally extraperitoneal hernia repair with non-stapling of mesh and without using a dissection balloon from August 2005 to December 2007. Results All of operations were successfully performed. The operative time was 60-120 minutes, the blood loss was 20-60 ml, and the length of postoperative hospitalization was 3 days. All cases were followed up for 8 to 18 months and found no recurrence. Conclusion Laparoscopic totally extraperitoneal hernia repair with non-stapling of mesh and without using a balloon dissection is feasible, reliable and effective, offering a low recurrence rate, while its price is higher than tension-free herniorrhaphy by traditional method.
ObjectiveTo survey the feasibility of laparoscopic high ligation for pediatric inguinal hernias by performing the surgery on rabbit models. MethodsLaparoscopic high ligation was operated on 32 healthy New-Zealand male rabbits. Eight random rabbits were observed under laparoscope on the 7th, 15th, 30th, and 60th days after operation, and the anti-tension strength at the instant when hernial inner ring cracked was measured. Repair regions were resected. After HE-staining, the syzygial status of the repair regions were checked. ResultsNone of the rabbits died during the research with no such complications as ankylenteron intestinal obstruction or hernia relapses after surgery. The anti-tension strength by the inguinal regions of the experimental rabbits after surgery on the 7th, 15th, 30th, and 60th days was respectively (42.69±6.98), (69.31±6.52), (102.64±7.91), and (106.53±7.54) mm Hg (1 mm Hg=0.133 kPa). As for the pathological section observation, the agglutination of the repair region was consistent with reparative process of chronic nonbacterial inflammation. ConclusionThe operation of laparoscopic high ligation for repairing inguinal hernia on rabbits is safe and reliable.
