Objective To explore the efficiency of artificial intelligence algorithm model using preoperative blood indexes on the prediction of deep vein thrombosis (DVT) in patients with lower limb fracture before operation. Methods Patients with lower limb fracture treated in the Department of Orthopedics of Deyang People’s Hospital between January 2018 and December 2022 were retrospectively selected. Their basic and clinical data such as age, gender, height and weight, and laboratory examination indicators at admission were collected, then the neutrophi to lymphocyte ratio (NLR), monocyte to lymphocyte ratio (MLR), and platelet to lymphocyte ratio (PLR) were calculated. According to color Doppler ultrasound indication of DVT in lower extremities at admission, the patients were divided into DVT group and non-DVT group. After data preprocessing, grey relational analysis (GRA) was used to screen the combination model of important predictive features of DVT, and BP neural network prediction model was established using the selected features. Finally, the accuracy of BP neural network prediction model was evaluated, and was compared with those of different models in clinical prediction of DVT. Results A total of 4033 patients with lower limb fracture were enrolled, including 3127 cases in the DVT group and 906 cases in the non-DVT group. GRA selected seven important predictive features: absolute lymphocyte value, NLR, MLR, PLR, plasma D-dimer, direct bilirubin, and total bilirubin. The accuracies of logistic regression analysis, random forest, decision tree, BP neural network and GRA-BP neural network combination model were 74%, 76%, 75%, 84% and 87%, respectively. The GRA-BP neural network combination model had the highest accuracy. Conclusion The GRA-BP neural network selected in this paper has the highest accuracy in preoperative DVT risk prediction in patients with lower limb fracture, which can provide a reference for the formulation of DVT prevention strategies.
Objective To analyze the influence of the body mass index (BMI) and age on deep vein thrombosis (DVT) after the total hip arthroplasty(THA) or total knee arthroplasty(TKA) in Chinese patients who received prophylactic treatment for DVT. Methods We used a randomized clinical case-control study. From April 2004 to August 2004, weperformed THA and TKA for 95 patients (128 hips and knees). There were 27 men and 68 women with an average age of 60 years (range, 23-78) at surgery, and withan average BMI of 25.88 kg/m2 (range, 14.34-40.39) before surgery. All the patients were given low-molecular-weight heparin for 7-10 days pre-and postoperatively to prevent DVT. Color Doppler ultrasonography was used to detect DVT of bilateral lower extremities in all the patients before operation and 7-10 days after operation.The patients were divided into 4 groups according to their BMI:the non-obese group (BMI≤25.00 kg/m2), the overweight group (BMI, 25.01-27.00 kg/m2), the obese group (BMI, 27.01-30.00 kg/m2), and the morbidly obese group (BMIgt;30.00 kg/m2); and they were also divided into 4 groups according to their ages (≤40, 4160, 61.70, gt;70 yr) so as to analyze the influence of the BMI and age on DVT. Results The total incidence ofDVT in all the patients was 47.4% (45/95) and the incidence of proximal DVT was 3.2%. Forty-five patients had DVT and 50 patients had no DVT. The average BMI of the patients with DVT was significantly higher than that of the patients with no DVT (Plt;005). The overweight, obese, and morbidly obese patients had an odds ratio of 7.04, 4.8,and 9.6 for DVT compared with the nonobese patients (P<0.05); but the obese group had a less risk than the other 2 groups.The 41-60, 61-70, and gt;70-year-old patients had an odds ratio of 24.0, 38.2, and 24.4 for DVT compared with the ≤40year-old patients (P<0.05). Conclusion Obesity (BMIgt;25 kg/m2) and an increasing age (gt;40 yr) are identified as statistically significant risk factors for DVT after THA or TKA, and patients aged 61-70 years are more likely tohave DVT than the other patients. We should pay more attention to those obesity and aged patients when they are undergoing the total joint replacement, and we should give them enough prophylaxis and closely observe the symptoms in their bilateral lower extremities after operation, taking ultrasonography or venography to check DVT if necessary, so that we can give them prompt treatment and prevent fatal pulmonary thromboembolism.
Objective To investigate the risk factors, clinical characteristics and prognostic factors of venous thrombosis (and pulmonary embolism) in patients with idiopathic hypereosinophilia (IHE) so as to provide a theoretical basis for clinical prevention of venous thrombosis and improve prognosis.Methods Thirty-nine patients with IHE admitted to West China Hospital of Sichuan University from January 2010 to January 2022 were collected in this retrospective case-control study to explore the risk factors of venous thrombosis (including pulmonary embolism) and thrombosis recurrence after treatment. Results There were 17 (43.5%) patients combined with venous thrombosis of 39 patients with IHE. In the patients with vascular involvement, pulmonary embolism was the initial expression of IHE accounted for 29% (5/17). patients of IHE with pulmonary embolism were younger [44 (24.5 - 51.0) vs. 56 (46.3 - 67.8) year, P=0.035] and had higher peak absolute eosinophil counts [11.7 (7.2 - 26.5)×109/L vs. 3.8 (2.9 - 6.7)×109/L, P=0.020] than those without pulmonary embolism. After a mean follow-up of 13 months (2 - 21 months), thrombosis recurred in 35.3% (6/17) of patients. Persistent increasing in eosinophils (>0.5×109/L) was an independent risk factor for thrombus recurrence (odds ratio 13.33, 95% confidential interval 1.069 - 166.374). Conclusions Thrombosis is a common vascular impaired complication in IHE , and increased eosinophilia is a risk factor for thrombosis and thrombus recurrence after therapy. Controlling and monitoring the eosinophilic cell levels in patients with IHE may avoid severe comorbidities.
目的探討經腘靜脈置溶栓導管灌注溶栓治療急性下肢深靜脈血栓形成(DVT)的臨床應用價值。方法對28例急性DVT患者(其中2例合并肺動脈栓塞)在超聲引導下經患肢腘靜脈穿刺置入溶栓導管至髂、股靜脈血栓中進行溶栓治療,并對溶栓效果進行分析。 結果全組患者溶栓后癥狀明顯改善,下肢腫脹消退,肺部癥狀緩解。溶栓后患、健側大腿周徑差〔(1.72±1.23) cm〕明顯小于溶栓前〔(5.47±1.29) cm〕,差異有統計學意義(t=12.14,Plt;0.01), 患肢大腿消腫率為91.58%; 溶栓后患、健側小腿周徑差〔(1.55±0.77) cm〕也明顯小于溶栓前〔(5.04±1.32) cm〕,差異有統計學意義(t=13.81,Plt;0.01),患肢小腿消腫率為84.92%。溶栓后靜脈通暢評分〔(4.34±3.55)分〕明顯低于溶栓前〔(15.23±4.64) 分〕,差異有統計學意義(t=6.42,Plt;0.01 ),溶栓后靜脈平均通暢率為87.43%。 2例合并肺動脈栓塞患者中,1例栓子完全溶解,1例大部分溶解。 所有患者均無嚴重并發癥。 25例患者獲隨訪,隨訪1~12個月(平均7.16個月),無一例出現血栓再發。結論經腘靜脈置管溶栓治療急性DVT是一種安全、有效的治療方法。
ObjectiveTo investigate the clinical efficacy of catheter directed thrombolysis (CDT) through three different approaches combined with iliac venous endovascular therapy for acute deep venous thrombosis (DVT) complicated with Cockett syndrome of the lower extremities. MethodThe clinical data of 87 patients with CDT through three different approaches (small saphenous vein group, popliteal vein group, and posterior tibial vein group) combined with iliac venous endovascular therapy for DVT complicated with Cockett syndrome of the lower extremities were analyzed retrospectively. ResultsThe lower extremity swelling of all the patients were disappeared obviously within 72 h after surgery, there was no death related surgery and pulmonary embolism. The limb edema reduction rates had no significant differences among the small saphenous vein group, popliteal vein group, and posterior tibial vein group﹝(77±13)% versus (82±12)% versus (77±18)%, P > 0.05﹞. The recanalization rates of thrombolysis had no significant differences among the above three groups﹝(86.5±10.6)% versus (92.0±7.7)% versus (87.3±7.8)%, P > 0.05﹞. The time required for the cannulation in the posterior tibial vein group was significantly shorter than that of the small saphenous vein group or popliteal vein group﹝(15.14±3.62) min versus (32.62±9.36) min or (42.79±13.30) min, P < 0.01﹞. All the patients were performed by balloon dilatation and iliac vein stenting. Eighty-seven cases were followed-up for 1-24 months, the primary patency rate of iliac venous was 100%. ConclusionsCDT with iliac venous endovascular therapy is an effective method in treatment of acute DVT with Cockett syndrome. CDT through posterior tibial vein is an easier and effective method with less complications and time. This way could be acceptable in basal hospital.
As a risk factor for vascular diseases and inflammatory diseases, fibrinogen has received more and more attention. Hyperfibrinogenemia is associated with the occurrence, development, and poor outcome of artery-venous ischemic stroke (acute ischemic stroke, transient ischemic attack and cerebral venous thrombosis). Therefore, fibrinogen may be a potential therapeutic target for the prevention and management of artery-venous ischemic stroke. However, there has been controversy regarding the defibrinogen therapy in artery-venous ischemic stroke. Therefore, this paper introduces the efficacy and safety of defibrinogen therapy alone, combined with antiplatelet or combined with anticoagulant in prevention and management of artery-venous ischemic stroke in detail, in order to re-understand the role of defibrinogen therapy in the prevention and management of artery-venous ischemic stroke.
Ankle pump exercise (APE) is one of the basic measures to prevent the formation of deep vein thrombosis, which has been widely recognized for its advantages of simplicity, safety, and ease of perform. However, there is still controversy regarding the frequency, duration, angle, position, and adjunctive exercise of APE. This article will review the hemodynamic and hemorheological effects of APE for the prevention of DVT, the current status of clinical application, and new advances in adjunctive APE, in order to provide methods and guidance for clinical staff.
Objective To explore the influencing factors of muscle calf vein thrombosis (MCVT) after hip and knee arthroplasty. Methods Convenience sampling method was used to select patients who underwent hip or knee arthroplasty in 4 hospitals in Beijing, Jiangsu and Hainan between March 2021 and May 2022 as the research subjects. They were divided into MCVT group and non-MCVT group according to whether the lower limb MCVT occurred on the 3rd postoperative day. Generalized linear mixed model was used to analyze the influencing factors of MCVT. Results A total of 278 patients were included, including 47 patients in the MCVT group, accounting for 16.91%, and 231 patients in the non-MCVT group, accounting for 83.09%. Generalized linear mixed model analysis showed that the ambulation on the first postoperative day [odds ratio=0.364, 95% confidence interval (0.159, 0.832), P=0.017] and preoperative activated partial thromboplastin time [odds ratio=0.884, 95% confidence interval (0.785, 0.994), P=0.040] were protective factors for MCVT after hip and knee arthroplasty. Conclusions The ambulation on the first postoperative day and preoperative activated partial thromboplastin time are protective factors for MCVT after hip and knee arthroplasty. The relationship between hypertension, drainage and MCVT needs to be further explored. Clinical medical staff should pay close attention to the postoperative ambulation of patients undergoing hip and knee arthroplasty, encourage and guide patients to get out of bed early, comprehensively assess the patients’ condition and the risk of postoperative MCVT, and flexibly formulate individualized prevention and treatment plans to avoid the occurrence of MCVT and improve the prognosis.
