        • 新改良LeFort Ⅰ型截骨線在鼻旁凹陷畸形矯治中的應用

          目的 總結新改良LeFort Ⅰ型截骨線在鼻旁凹陷畸形矯治中的應用。 方法 2008 年4 月-2009 年9 月,采用新改良LeFort Ⅰ型截骨線矯治3 例面中部發育不良鼻旁凹陷畸形女性患者。年齡18 ~ 26 歲。均表現為上頜后縮伴明顯鼻旁區凹陷及下頜前凸,為Angle Ⅲ類錯頜畸形。術前經正畸治療后,修正SNA 平均為73.6°,SNB 平均為82.7°。 結果 術中出血量400 ~ 600 mL,平均350 mL。術后切口均Ⅰ期愈合,無骨塊壞死等并發癥發生。3 例均獲隨訪,隨訪時間6 ~ 23 個月,平均15 個月。畸形無復發,面型穩定無變化,咬調整為Angle Ⅰ類咬。 結論 采用新改良LeFort Ⅰ型截骨線矯治鼻旁凹陷畸形效果理想。

        • Application of Intraoperative Ultrasonography in the Resection of Intracranial Arteriovenous Malformation

          【摘要】 目的 探討顱內動靜脈畸形切除術中彩色多普勒超聲檢查的應用價值與預后的關系。 方法 在顯微外科手術的基礎上,將65例動腦靜脈畸形患者隨機分為A、B兩組, A組34例動靜脈畸形患者在術中接受術中超聲檢查,確定其畸形血管團的數目、位置、深度、大小、范圍及其與周邊組織結構的關系,探查供血動脈及引流靜脈的數目及走行,以確定動、靜脈畸形的位置、手術切除范圍及切除術后是否有畸形血管殘留;B組31例行常規手術,比較兩組之間的差異。 結果 術中彩色多普勒超聲能清晰的顯示動靜脈畸形的情況,兩組經手術完整切除動靜脈畸形率、再出血率、術后7 d Glasgow昏迷評分及預后差異均有統計學意義(Plt;0.05)。 結論 術中實時彩色多普勒超聲能對腦動靜脈畸形準確定位,并可判定腦動靜脈畸形的血供模式及殘余情況,減少腦組織損傷及降低手術并發癥的發生,提高了手術安全性及治療效果。【Abstract】 Objective To explore the application of the colored Doppler ultrasonography in the excision of intracranial arteriovenous malformation (AVM) to determine the location of the lesion and the completeness of the resection, and to evaluate the clinical value of intraoperative ultrasonography in the diagnosis, treatment and the relation to the prognosis of patients. Methods A total of 65 patients with brain AVM were randomly divided into group A and B. The microsurgery was perfromed on the patients. Intraoperative ultrasound was performed on the 34 patients in group A to the numbers, location, depth,size of different malformation vascular clumps relationship of the arteriovenous malformation with surrounding tissues. While the pateints in group B underwent the routine surgery. The difference between the two groups were compared. Results The condition of the AVM were clearly revealed by intraoperative ultrasonography. All of the intracranial AVM in group A were completely removed. The difference in rehaemorrhagia and postoperative Glasgow coma scale score of seven day between the two groups were statistically significant (Plt;0.05). Conclusions Intraoperative color Doppler ultrasounography in the resection of intracranial AVM could accurate localize the lesions, identify vascular anatomy of the AVM vascular balls and the completeness of resection. The parameters including resistance index of the vessels may identify the abnormal vessels and reduce the operative complications and brain tissue damage, which improve the safety and efficacy of the therapeutic effect.

        • RELATED FACTOR ANALYSIS OF CUBITAL TUNNEL SYNDROME CAUSED BY CUBITUS VALGUS DEFORMITY

          To explore related factors of cubital tunnel syndrome caused by cubitus valgus deformity so as to provide theoretical basis for the cl inical treatment. Methods Between June 2002 and September 2008, 40 patients with cubital tunnel syndrome caused by cubitus valgus deformity underwent anterior subcutaneous ulnar transposition. Related factors wasanalysed through logistic regression analysis using scoring standard recommended by Yokohama City University. Results All 40 patients were followed up 27.5 months on average (range, 12-75 months). The duration of cubitus valgus deformity, cubitus valgus deformity angle, and the duration of paraesthesia and muscular atrophy were identified as related factors for ulnar neuropathy and the odds ratios were 1.005 (P=0.045), 9.374 (P=0.000), and 4.358 (P=0.010), respectively. The related prognosis factors were duration of paraesthesia and muscular atrophy, deformity angle, and age at surgery, with odds ratios of 8.489 (P=0.000), 2.802 (P=0.030), and 4.611 (P=0.031), respectively. Conclusion Related factors for ulnar neuropathy are durations of cubitus valgus deformity, cubitus valgus deformity angle, and duration of paraesthesia and muscular atrophy. Related factors for prognosis include age at surgery, cubitus valgus deformity angle, and duration of muscular atrophy. Early anterior subcutaneous ulnar transposition should be performed in patients with cubital tunnel syndrome caused by cubitus valgus deformity

        • Development and application of a multidisciplinary nursing program for Prader-Willi syndrome with spinal deformity in perioperative period

          Objective To develop a multidisciplinary nursing program for Prader-Willi syndrome with spinal deformity and evaluate its effectiveness in clinical practice. Methods In July 2016, a multidisciplinary collaborative team was established before the treatment of children with Prader-Willi syndrome complicated with spinal deformity. For the nursing difficulties in the perioperative period, relevant literature was consulted, and a multidisciplinary collaborative nursing plan was formulated, which included nutrition management and blood glucose control, management of obstructive sleep apnea hypopnea syndrome, behavioral intervention related to mental change, and early identification and management of complications. The developed multidisciplinary collaborative nursing program was applied to three children with Prader-Willi syndrome complicated with spinal deformity in Peking Union Medical College Hospital from July 2016 to October 2018, and their postoperative recovery was evaluated. Results One child had inguinal skin ulceration when admitted to hospital, which was significantly improved after active treatment. The perioperative blood glucose level control of the three children was satisfactory, and there was no postoperative gastrointestinal dysfunction, deep wound infection, respiratory complications, internal fixation failure or other complications, and no asphyxia, fall, loss, or other adverse events. The follow-up compliance after discharge was 100%. Conclusions Multidisciplinary collaboration programs can escort patient’ safety and promote their recovery, improve the professional level of nursing staff, and reflect the nursing value. As a working mode, it can be further popularized and used for reference in the nursing of other difficult diseases.

        • Clinical Observation on Comprehensive Treatment for Body Surface Hemangioma and Vascular Malformation

          【摘要】 目的 觀察綜合治療體表血管瘤及脈管畸形的臨床療效。 方法 2008年1月-2010年9月,收治體表血管瘤和脈管畸形患者共205例。手術治療156例,其中單純手術治療103例,合并介入治療9例,硬化劑治療44例;非手術治療49例,其中介入治療1例,硬化劑治療38例,介入合并硬化劑治療10例。術后隨訪6個月~3年。 結果 15例手術治療患者于術后6個月~2年復發,其中單純手術治療患者10例(復發率9.71%),合并介入治療患者2例(復發率22.22%),合并硬化劑治療患者3例(復發率6.82%),均再次手術治療后痊愈。9例非手術治療患者于治療后3個月~2年復發,其中8例硬化劑治療患者,1例介入合并硬化劑治療患者,均經手術治療后痊愈。余患者未見復發。 結論 以手術治療為主的綜合治療,是體表血管瘤和脈管畸形治療的良好方法。【Abstract】 Objective To observe the clinical therapeutic efficacy of comprehensive treatment on body surface hemangioma and vascular malformations. Methods Between January 2008 and September 2010, 205 pateints with surface hemangioma and vascular malformation were treated. A total of 156 patients received surgical treatment, in whom 103 received surgery alone, 9 underwent combined interventional treatment, and 44 received combined sclerotherapy. There were 49 patients underwent non-surgical treatment, including intervention in 1, sclerotherapy in 38 involving 10 had combined sclerotherapy. The patients were followed up for 6 months to 3 years. Results A total of 15 patients who had received surgical treatment recurred 6 months to 2 years after the treatment, including 10 who had been treated with surgery alone (with the recurrence rate of 9.71%), 2 combined with interventional treatment (with the recurrence rate of 22.22%), and 3 combined with sclerotherapy (with the recurrence rate of 6.82%). They were all cured after reoperation. Nine cases of non-surgical treatment recurred 3 months to 2 years after the treatment, including 8 who had received sclerotherapy, and 1 combined with interventional treatment and sclerotherapy. Patients were all cured after surgery. The other patients had no recurrence. Conclusion Applying mainly on surgical treatment, the combined therapy is a good way for body surface hemangioma and vascular malformations.

        • TREATMENT OF CONGENITAL ADDUCTION FLEXION DEFORMITY OF THUMB

          Fom march Ⅰ98Ⅰ to June Ⅰ988, twenty-one hands in Ⅰ9 patients with congenital adduetion-flexion deformity of thumb were treated by various methods. The good appearance and function were obtained in Ⅰ8 cases. Appearance and function were improved in 3 cases. The author indicated that thorough soft tissue releasing, keeping stability of MP joint and deepening first web were the key points in achieving operation success.

        • Data analysis of 8 113 cases of limb deformities corrected by external fixation

          Objective To analyze the data of external fixation instruments (including Ilizarov instruments) used by QIN Sihe orthopaedic surgical team in the treatment of limb deformities in the past 30 years, and to explore the indications for the application of modern external fixation techniques in the correction of limb deformities and individual device configuration selection strategy. Methods According to QIN Sihe orthopaedic surgical team, the use of external fixator between January 1988 and December 2017 was analyzed retrospectively. The total use of external fixation and the proportion of different external fixators were analyzed in gender, different operation time, different age, different parts, and different diseases. Results External fixators were used in 8 113 patients, 69 of them were used simultaneously in both lower extremity surgery, so 8 182 external fixators were used. Among them, there were 4 725 (57.74%) combined external fixators, 3 388 (41.41%) Ilizarov circle fixators, 64 (0.78%) single arm external fixators (including Orthofix), 5 (0.06%) Taylor space external fixators. There were 4 487 males (55.31%) and 3 626 females (44.69%). According to the analysis of different time periods, the number of external fixators increased year by year, and the number of applications increased after 2000. The main age of the patients was 11-30 years old, of which 1 819 sets (22.23%) were used at the age of 21-25 years. The use of the external fixator covered almost all parts of the limbs, with the ankle and toe areas being the most common, reaching 4 664 sets (57.00%), and the upper extremities the least, with 152 sets (1.86%). The 8 113 cases covered more than a dozen disciplines and more than 150 kinds of diseases. The top 5 diseases were poliomyelitis sequelae, cerebral palsy, deformity of lower extremity after spina bifida, traumatic sequelae, and congenital equinovarus foot. Conclusion Ilizarov technique has been widely used in extremity deformity, disability, and complicated orthopedic diseases caused by vascular, lymphoid, nerve, skin, endocrine, and other diseases. The indication of operation is far beyond the scope of orthopedics. The domestic external fixator and its mounting tools can basically meet the requirements of various treatments. The technique of external fixation has entered a new era of tension tissue regeneration under stress control, natural repair of tissue trauma and deformity, and reconstruction of limb function.

        • Cross-sectional study of nutritional status and nutritional support in patients with spinal deformity during orthopedic perioperative period

          Objective To investigate the status of nutrition, nutritional support, and postoperative nutrition-related complications in patients with spinal deformity during orthopedic perioperative period. Methods From February to August 2021, patients who underwent spinal orthopedic surgery for spinal deformity in Peking University Third Hospital were collected. A survey was conducted using self-designed questionnaire, including patient’s general information (gender, age, and type of spinal deformity), nutrition-related information (risks of malnutrition, forms of nutritional support), and postoperative nutrition-related complications. Results A total of 39 patients with spinal deformity were enrolled, and 46.2% (18 cases) were at risk of malnutrition. There were a total of 174 times of nutritional support, and 96.0% (167 times) were parenteral nutrition. The patients’ energy intake from nutritional support accounted for 34.41% of the daily goal energy intake averagely. At admission, one day after operation, and at discharge, the prealbumin was (215.51±34.69), (172.85±31.85), and (163.67±29.15) mg/L, respectively, and the hemoglobin was (138.08±15.67), (119.92±18.01), and (117.69±14.76) g/L, respectively, which were significantly lower one day after operation and at discharge than those at admission (P<0.01). The incidence of postoperative nutrition-related complications during hospitalization was 5.1% (2/39). Conclusions Patients undergoing spinal deformity orthopedics are at high risk of overall malnutrition during the perioperative period, their clinical nutritional interventions are mostly based on parenteral nutrition with a single infusion of nutritional preparations, and the nutritional status is not significantly improved. However, the incidence of postoperative nutrition-related complications is low. Standardized nutrition interventions should be strengthened in the future.
