        • Numerical study of the effect of geometrical parameters of straight impellers on the flow and hemolysis performance of centrifugal blood pumps

          Red blood cells are destroyed when the shear stress in the blood pump exceeds a threshold, which in turn triggers hemolysis in the patient. The impeller design of centrifugal blood pumps significantly influences the hydraulic characteristics and hemolytic properties of these devices. Based on this premise, the present study employs a multiphase flow approach to numerically simulate centrifugal blood pumps, investigating the performance of pumps with varying numbers of blades and blade deflection angles. This analysis encompassed the examination of flow field characteristics, hydraulic performance, and hemolytic potential. Numerical results indicated that the concentration of red blood cells and elevated shear stresses primarily occurred at the impeller and volute tongue, which drastically increased the risk of hemolysis in these areas. It was found that increasing the number of blades within a certain range enhanced the hydraulic performance of the pump but also raised the potential for hemolysis. Moreover, augmenting the blade deflection angle could improve the hemolytic performance, particularly in pumps with a higher number of blades. The findings from this study can provide valuable insights for the structural improvement and performance enhancement of centrifugal blood pumps.

        • Hemolysis of Xuesaitong Injection

          【摘要】 目的 對市場上流通使用的血塞通注射液在溶血性方面的狀況進行考察與研究。 方法 按《中國藥典》2005年版一部附錄ⅩⅧ B中藥注射劑安全性檢查法應用指導原則和中藥、天然藥物刺激性和溶血性研究的技術指導原則,對11 個廠家共計27 批血塞通注射液每批次樣品制備4 個濃度,進行溶血實驗研究,并采用分光光度法(545 λ/nm)測定計算溶血率,比較各批次樣品的溶血率。 結果 不同廠家甚至同一廠家不同批次的血塞通注射液溶血率存在一定差異。 結論 在臨床使用中應注意用量,過量使用可能導致溶血引起的不良反應;同時,應注意溶血引起的臨床不良反應的觀測。【Abstract】 Objective To explore the hemolysis of Xuesaitong injection. Methods According to the Guiding Principles of safety tests on traditional Chinese medicine injection in Annex ⅩⅧ B, Chinese Pharmacopoeia, 2005 Edition 1, and Technical Guidelines of studies on the irritability and hemolytic activity of traditional Chinese medicine and natural medicine, a total of 27 samples of Xuesaitong injections (each sample was diluted into four concentrations) produced by 11 manufacturers had been examined. Spectrophotometry (545 λ/nm) was used to calculate the hemolytic rate. Results There was a certain difference in the hemolytic rate among several samples of Xuesaitong injections produced by different manufacturers, or even different batches by the same manufacturer. Conclusion The dosage of Xuesaitong injections should be noted in clinical use. Excessive use may lead to adverse reactions caused by hemolysis; at the same time, clinical adverse reactions caused by hemolysis should be observed.

        • A case report of pulmonary tumor thrombotic microangiopathy and review of the literature

          ObjectiveTo investigate the clinical features, diagnosis, treatment and prognosis of pulmonary tumor thrombotic microangiopathy (PTTM).MethodsA patient with PTTM was reported. Literatures about PTTM searched by WanFang databases and PubMed were reviewed for its clinical characteristics.ResultsA 62-year-old female was admitted with chief complaint of dry cough, dyspnea and hemoptysis. Progressive dyspnea, pulmonary hypertension and hypoxemia occurred during hospitalization. Computed tomography angiography (CTA) of the lung excluded pulmonary embolism. Peripheral blood appearing a large number of late erythroblasts and erythrocyte debris and progressively decreasing platelets suggested that the patient suffer from thrombotic microvascular disease. CT showed widely metastatic lesions at the vertebrae and sternum. On the basis of above clinical characteristics, PTTM was diagnosed clinically. Although the patient accepted respiratory support therapy, anticoagulation therapy and resuscitation, she still died 5 days later after hospitalization. Literatures about PTTM with complete clinical information were reviewed. A total of 92 PTTM cases were reviewed and the main reasons of these patients admitted were progressive dyspnea and chronic cough. During hospitalization, they all suffered varying degrees of hypoxia, while radiological findings of the lungs lack specificity. No abnormal sighs were found by lung CTA. The results of ultrasonic cardiography or the Swan–Ganz catheter indicated varying degrees of pulmonary hypertension, some patients were proved with disseminated intravascular coagulation and/or microangiopathic hemolytic anemia. The definite diagnosis of PTTM depended on the histologic evidence which were often obtained from post-mortem examination, because many patients couldn’t tolerate the lung biopsy due to rapid aggravation. The treatment of PTTM included respiratory support therapy, anticoagulation therapy, antipulmonary hypertension and the chemotherapy of primary or metastatic tumour. The prognosis of PTTM was poor and almost all of the patients died in a short term, ranged from 48 hours to 3 months.ConclusionIf a patient with a history of cancer or evidence of cancer metastasis has hypoxemia and pulmonary hypertension but without abnormal lung CTA signs, PTTM should be considered.

        • Clinical Significance of the Detection of CD55, CD59 Expression Deletion in Patients with Cytopenia

          目的 檢測血細胞減少患者外周血紅細胞和中性粒細胞細胞膜糖基磷脂酰肌醇(GPI)連接的補體調節蛋白衰變加速因子(CD55)和膜反應性溶血抑制物(CD59)表達情況,并探討其臨床意義。 方法 2006年7月-2011年3月,采用直接免疫熒光標記法流式細胞儀檢測182例血細胞減少患者外周血CD55及CD59表達情況,其中陣發性睡眠性血紅蛋白尿(PNH)9例,再生障礙性貧血(AA)-PNH綜合征8例,AA 83例,骨髓增生異常綜合征51例,自身免疫性溶血性貧血11例,造血功能停滯6例,缺鐵性貧血7例,巨幼細胞性貧血4例,脾功能亢進3例。 結果 PNH及AA-PNH患者CD55、CD59抗原缺失率均較其他血細胞減少者明顯增高。 結論 流式細胞儀檢測外周血中紅細胞和中性粒細胞膜CD55和CD59抗原表達缺失率是目前診斷PNH可靠和敏感的方法,也是對PNH、AA-PNH早期診斷敏感指標,并且PNH克隆檢測還能為診斷疾病提供鑒別診斷依據。

        • Computational Evaluation of the Fluid Dynamics of a Disk Blood Pump

          Objective To optimize the hemodynamics of a disk blood pump in children. Method We used the computational fluid dynamics technology to simulate the flow in a pediatric blood pump numerically, and finally analyzed the results for deep study about the thrombosis and hemolysis produced in it, to improve the design according to the results of the flow field analysis. Results We calculated results between the flow rate and the pressure elevation at different rotational speed: 2 500 rpm, 3 000 rpm, and 4 000 rpm, respectively. Under each rotational speed, it was selected five different discharge outlet boundary conditions. The simulation results conformed to the experimental data. The increased pressure of the blood pump was effective. But the phenomenon of flow separation was increased the at blade surface in the low speed region. The maximum wall shear stress was maintained within 100 Pa. Conclusion The design of disc blood pump has a good fluid dynamic performance. And the flow line is fluent, the probability of thrombosis and hemolysis occurred is in the range of control. But the phenomenon of flow separation is appeared. There is a room to improve.

        • Progress in the analysis of hemolysis and coagulation models for interventional micro-axial flow blood pumps

          Interventional micro-axial flow blood pump is widely used as an effective treatment for patients with cardiogenic shock. Hemolysis and coagulation are vital concerns in the clinical application of interventional micro-axial flow pumps. This paper reviewed hemolysis and coagulation models for micro-axial flow blood pumps. Firstly, the structural characteristics of commercial interventional micro-axial flow blood pumps and issues related to clinical applications were introduced. Then the basic mechanisms of hemolysis and coagulation were used to study the factors affecting erythrocyte damage and platelet activation in interventional micro-axial flow blood pumps, focusing on the current models of hemolysis and coagulation on different scales (macroscopic, mesoscopic, and microscopic). Since models at different scales have different perspectives on the study of hemolysis and coagulation, a comprehensive analysis combined with multi-scale models is required to fully consider the influence of complex factors of interventional pumps on hemolysis and coagulation.

        • 心臟瓣膜手術后并發溶血性貧血的治療

          目的 探討心臟瓣膜手術后并發溶血性貧血的外科治療方法及效果。方法 1998年1月至2007年12月,廣東省人民醫院廣東省心血管病研究所共治療11例心瓣膜手術后并發溶血性貧血患者,男9例,女2例;年齡15~57歲(40±14歲)。風濕性心瓣膜病6例,退行性心瓣膜病2例,先天性心瓣膜病2例,感染性心內膜炎1例。第一次手術:行二尖瓣置換術(MVR)2例,雙瓣膜置換術(DVR)4例,二尖瓣人工瓣環成形術5例。第二次入院血紅蛋白55~92 g/L(76±14 g/L),紅細胞壓積0.19~0.31(0.25±0.04),網織紅細胞百分比0.08~0.17(0.12±0.04),總膽紅素34.70~91.50 μmol/L(56.00±19.10 μmol/L),非結合膽紅素23.40~54.90 μmol/L(38.60±12.30 μmol/L)。所有患者均先給予內科治療,10例患者經內科治療無效后再次行心瓣膜置換術或心瓣膜成形術治療,另1例給予內科保守治療。結果 再次手術治療10例,術后死亡1例,死于溶血性貧血、腎功能衰竭和多器官功能衰竭;其他9例經再次手術治療后痊愈出院,貧血癥狀消失,血常規檢查正常。內科保守治療治愈1例。隨訪10例,隨訪時間2個月至11年,心功能Ⅰ級8例,Ⅱ級2例,無晚期死亡。1例感染性心內膜炎患者出院時仍有輕度腎功能不全,隨訪3個月時腎功能完全恢復正常,隨訪1年心功能良好,無溶血癥狀。 結論 心瓣膜手術后并發溶血性貧血,如經內科治療無效應盡早行再次心瓣膜手術治療,可獲得良好的效果。
