ObjectiveTo investigate the relationships between the expression of integrin β1 and activated cells in a partial-thickness articular cartilage injury model of adult rats. MethodForty-five male Sprague Dawley rats (aged 10 weeks and weighing 300-400 g) were randomly divided into operated group (n=15) , sham-operated group (n=15) , and control group (n=15) . Partial-thickness articular cartilage injury model was made by scarification in operated group, direct suture after opening of the knee joint was performed in sham-operated group, and no operation was done in control group. Five rats were sacrificed at 1, 7, and 14 days after operation respectively for macroscopic evaluation, HE staining, Safranin O staining, CD105, BrdU, CD105/integrin β1 immunofluorescence and double labeling staining. The histological score of HE staining, gray value of Safranin O staining and CD105-positive cells count were compared among groups at each time point. ResultsMacroscopic evaluation showed chondromalacia and cartilage fibrosis around the linear injury with aggravating tendency with time in operated group, but no chondromalacia and cartilage fibrosis in sham-operated and control groups. HE staining demonstrated a number of activated cells accumulating around the linear injury with nonuniform distribution in operated group, and uniform size and distribution in sham-operated and control groups. The histological scores at each time point in operated group were significantly higher than those in sham-operated group and control group (P<0.05) , but no significant difference was found between different time points in 3 groups (P>0.05) . Safranin O staining was nonuniform with hypochromasia around linear injury in operated group, but the staining was uniform in sham-operated group and control group. Gray value of Safranin O staining had no significant difference among groups and among different time points in the same group (P>0.05) . BrdU-positive and CD105-positive cells distributed unevenly around the linear injury in operated group, uniform distribution was observed in sham-operated group and control group. CD105-positive cells count in operated group was significantly higher than those in sham-operated group and control group at each time point (P<0.05) ; CD105-positive cells increased significantly with time in operated group (P<0.05) . CD105/integrinβ1-positive cells were observed around the linear injury in operated group, but was not observed in sham-operated group and control group. ConclusionsThe partial-thickness articular cartilage injury model is successfully established in rats, and cartilage injury could not be repaired completely in the model. The activated cells aggregation around the linear injury can be observed, but there is no obvious relationships between activated cells and cartilage matrix. These activated cells are in proliferation and could express both CD105 and integrin β1.

        • The theoretical and practical research and effect evaluation at home and abroad: a review of integrated care

ObjectivesTo analyze the theoretical and practical research and effect evaluation of integrated care at home and abroad, so as to provide evidence for the development of integrated care in China.MethodsPubMed, CNKI and WanFang Data databases and government documents, research reports were searched to collect studies on integrated care, and then literature review was then performed.ResultsForeign integrated care was dominated at government level. The integration involved numerous departments such as hospital and insurance institutions. The degree of integration was high and funds were sufficient. The theoretical framework was studied from the macro, meso and micro perspectives, and people-centered, value-based integration was proposed. However, in China the integrated care was primarily guided by the government nationally. Local government was responsible for specific integration practices. The degree of integration was low and funds were insufficient. The theoretical research mostly focused on cooperation, interest mechanisms and so on. At home and abroad, researchers focused on the evaluation of health service and quality. Foreign countries paid more attention to medical costs, while in China, due to the imbalance of interest mechanisms, researchers paid more attention to medical expenses and performance evaluation.ConclusionThere is no unified theoretical framework and method for integrated medical care. Researchers and policy makers should combine the successful experience and characteristics at home and abroad, consider the complex health policy context of the country to implement integrated care.

        • Association between ADAM33 Gene Polymorphisms and Susceptibility to Asthma in Chinese Han Population: A Meta-analysis

ObjectiveTo systematically review the association between ADAM33 (a disintegrin and metalloproteinase domain33) T2 (rs2280090), S2 (rs528557), and V4 (rs2787094) polymorphisms and asthma in Chinese Han population. MethodsWe electronically searched databases including PubMed, Web of Science, CNKI, WanFang Data and VIP from inception to December 2014, to collect case-control studies about the association between ADAM33 polymorphisms and asthma in Chinese Han population. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies. Then, meta-analysis was performed using Stata 12.1 software. ResultsA total of 8 case-control studies including 1651 asthma patients and 1639 controls were included. The results of meta-analysis showed that: ADAM33 T2 polymorphism was associated with decreased risk of asthma (AA+AG vs. GG: OR=0.316, 95%CI 0.151 to 0.659, P=0.002), and the S2 polymorphism was associated with increased risk of asthma (GG+GC vs. CC: OR=1.271, 95%CI 1.023 to 1.578, P=0.030). However, no significant association was found between asthma risk and V4 polymorphism (GG+GC vs. CC: OR=1.561, 95%CI 0.638 to 3.823, P=0.330). Subgroup analysis by age indicated that the T2 polymorphism was associated with decreased risk of asthma in both adults and children, the S2 polymorphism was only associated with increased risk of asthma in adults, however, no significant association was found between asthma and the ADAM33 V4 polymorphism in both adults and children subgroups. ConclusionCurrent evidence shows that the ADAM33 T2 and S2 polymorphisms are associated with risk of asthma in Chinese Han population. Due to the limited quantity and quality of the studies, the above conclusion still need to be verified by further more high-quality studies.
