        • Retrospective analysis of 240 surgery-related near misses

          Objective To summarize the characteristics of surgery-related near misses including events composition, cause of incident, specialty category, personnel allocation etc, and to provide experience of feedforward control for the nurses in operating room and a clinical basis of safety standards for the management of operating rooms. Method The 240 surgery-related near misses occurred between July 2014 and July 2016 were retrospectively analyzed, using frequencies and percentiles to describe the count data. Results The 240 surgery-related near misses were mainly associated with surgical stitches (91 cases, 37.9%), surgical dressings (52 cases, 21.7%) and surgical instruments (45 cases, 18.8%). The main features of the 91 cases of surgical stitching included loss of suture needles (40.7%, 37/91) and fracture events (37.4%, 34/91). Among the 52 cases of surgical dressings, the most commonly were gauze dressing events (43 cases, 82.7%), in which 19 were with unclear numbers of retained gauzes in the reoperation patient’s body, and 15 were postoperative counting anomalies. Among the 45 cases of surgical instruments, the fracture and defect were the most common (21 cases, 46.7%). Conclusion The operation nurses should focus on the prevention of suture needle loss, the surgical dressings loss and the fracture and defect of surgical instruments, etc, to reduce or avoid the surgery-related near misses.

        • SURGICAL TREATMENT OF PERINEAL PAGET’S DISEASE

          Objective To investigate the surgical methods and effects of perineal Paget’s disease. Methods From January 1998 to September 2004, 20 perineal Paget’s disease cases were treated with several surgical methods. All the patients were male. Their ages ranged from 47 to 80 years. The courses of diseases ranged from 1 year to 9 years (4.7 years on average). Among them, 14 cases involved scrotums purely, 5 cases involved scrotums and penises, 1 cases involved scrotum, penis and hypogastric zone. Fifteen cases were diagnosed as having Paget’s disease through pathological exam. No metastasis was found before operation. ResultsThe wounds healed by first intention in 19 cases, the free skin graft necrosed partially in 1 case and healed by second intention after dress changing. All the patients were followed up 2 to 5 years, the contours of scrotums and penises were good. Two cases had tumor recurrence and received surgery again. Recurvation occurred in 2 cases during penis erecting,and was corrected by regional flap transplantation Conclusion The lesions like eczema in perineal reigon should be suspected as Paget’s disease, and should be biopsied earlier. Complete removal by surgery is the only effective treatment. Perineal Padget’s disease should be diagnosed and treated earlier. If the raw surfaces could not be repaired completely, flap transplantation and free skin graft should be used .The patients should be followed up closely.

        • A NEW PROCEDURE IN MAKING RELIABLE EXPERIMENTAL MODELS OF GASTROESOPHAGEAL REFLUX

          Objective To provide a reliable experimental model for gastroesophageal reflux (GER) study. Methods Twenty Japan 5-month-old male rabbits wererandomly divided into two groups: group cardiomyotomy(n=10), group partial cardiomyectomy(n=10). The operations of cardiomyotomy and parital cardiomyectomy were performed in 2 groups respectively. All the animals underwent intraesophagealpH detection 1 week before operation and 4 weeks after operation. The mean changes of reflux ratios were compared between before operation and after operation.Results In gastroesophageal reflux ratio between before operation and after operation, there was no significant difference in group cardiomyotomy (1.98%±1.52% and 4.32%±2.39%, Pgt;0.05) and there was significant difference in group partialcardiomyectomy(1.56%±1.57% and 13.56%±3.27%, Plt;0.05). Conclusion The reliable experimental model of GER can be made with procedure of partial cardiomyectomy. It can be used in estimating the operative procedure of antireflux and is conducive to dynamic observation and study of esophagitis.

        • CLINICAL ANALYSIS OF THE SURGICAL OPERATION ON 138 CASES WITH PAPILIARY ADENOCARCINOMA OF THYROID

          From Jan. 1980 to Dec. 1996, 138 cases of papillary adenocarcinoma of thyroid gland were surgically treated. To minimize the local recurrence and complication, resection of the involved lobe and the isthmus is an ideal surgical operation. Modified neck lymph node dissection should be performed, if the diameter of primary tumor is larger than 1.5 cm; whether the lymph node is palpable or not. Functional or classical radical neck lymph node excision should be taken, if the neck lymph node can be palpable.

        • REDUCED SIZED LIVER TRANSPLANTATION (REPORT OF 1 CASE)

          Objective To investigate the surgical technique of reduced sized liver transplantation. Methods A reduced size liver transplantation was successfully performed on a 11-year old girl with incurable caroli′s disease. Results The recovery of liver graft function was good after the operation in this patients without complications. Conclusion Reduced size liver transplantation is a safe and effective technique for pediatric liver transplantation to provide liver graft. Authors introduced their experiences of surgical technique in this patient.

        • ADVANCES AND CURRENT STATUS IN THE STUDY ON QUALITY OF LIFE AFTER OPERATION OF RECTAL CANCER

          Objective To summarize the advances in the clinical application of the quality of life(QOL) assessment after surgery of rectal cancer.MethodsThe recent literatures on the advances in the studies on the QOL assessment following the treatment of rectal cancer were reviewed. Results After surgery of rectal cancer, the patients might suffer from short and/or long term of various complications affecting the physical, social and psychological aspects of their well-being. The QOL assessment included many aspects of the studies. Global QOL scores as measured by the specific questionnaires improved significantly after surgery. It had provided clinical evidences of therapeutic methods, operative treatment of choices and bowel reconstructed procedures for the patients with rectal cancer.Conclusion The QOL studies after surgery for rectal cancer are of increasing importance. Apart from the improvements of the survival rates and the recurrencefree period, it is essential to provide an acceptable QOL postoperatively. The QOL assessment for the guidance of clinical treatment should be discussed routinely for a patient with potentially curable rectal carcinoma so that the excellent outcome could be achieved.

        • ANALYSIS OF THE EARLY POSTOPERATIVE COMPLICATION OF GASTRIC CARCINOMA

          One thousand four hundred and fifty-four cases of operations for gastric carcinoma in this hospital from 1983 to 1994 are reviewed. 121 out of 1454 patients sustained 168 occurrences of early postoperative complications. The patiets involvement rate was 8.32% and the occurrence rate of complications was 11.55%. Complications could be divided into two groups, the general complication after surery (8.25%) and complication relavent to gastrointestinal reconstruction (3.30%). Most common complications were wound infection, pulmonary infection, anastomotic obstruction or leakage. The authors stress the prevention of surgical complications that would furhter improve the therapeutic result of gastric cancer.

        • Comparison Between Interventional Treatment and Operation Drainage for Malignant Obstructive Jaundice

          Objective To compare the therapeutic effect of percutaneous transhepatic cholangial drainage (PTCD) and operation drainage for the patients with inoperable malignant obstructive jaundice. Methods A total of 131 patients with inoperable malignant obstructive jaundice were treated in this hospital, in which 102 patients had PTCD by placement of metallic stent and (or) plastic tubes to remove obstruction of bile duct (interventional treatment group). Simultaneously 29 patients were selected for operation by intraexternal drainage (operation drainage group). The patients were followed up for comparison of the serum level of total bilirubin, postoperative complications, average length of hospitalization and average cost between the two groups. Results PTCD was successfully performed in all the patients of the interventional treatment group. There were no significant differences of 50% decrease period of average total bilirubin level or postoperative complications between the two groups (Pgt;0.05). The average length of hospitalization and average cost of interventional treatment group were less than those of operation drainage group (Plt;0.05). Conclusions Compare with operation drainage, interventional treatment can reduce average length of hospitalization and average cost, without increase of postoperative complications, which is a main chance of treatment for malignant obstructive jaundice.

        • TREATMENT OF MALLET FINGER ACCOMPANIED BY AVULSIONAL FRACTURE WITH ISHIGURO METHOD

          Objective To evaluate the clinical results of mallet finger accompanied by avulsional fracture treated with Ishiguro method. Methods Twenty-seven patients suffering from mallet finger accompanied by avulsional fracture weretreated with Ishiguro method or its modified method. During the operation, a blocking pin was inserted percutaneously with DIP flexed, then the DIP was fixed at a position of full extension with another Kirschner wire. When the fracture fragment was big with rotation displacement, a third Kirschner wire could be used to correct the displacement and fix fracture. Results The patients were followed up 2 months to 6 years and 6 months with an average of 9.3 months postoperatively. Andthe clinical results were evaluated with the modified Crawford’s criteria, including DIP pain, range of motion and pinch power. The rate of bone union was 100%. The results were excellent in 7 patients, good in 11 patients, moderate in 8 patients and poor in 1 patient. After operation, the range of flexion was 54.19°±14.45° , while the range of extension was -4.96°±9.27°. The X-ray sign of slight osteoarthritis could be seen in one patient. [WTHZ]Conclusion Ishiguro method is simple, effective and less invasive. It is suitable for treatment of mallet finger accompanied by avulsional fracture.

        • Method of Establishing The Model of Thymectomy in Adult Rats

          Objective  To establish model of thymectomy in adult rats. Methods The animal models were built by resection of the thymus and simultaneously emptying the air under xiphoid in the rats underwent thoracotomy. Results Of 30 rats, 1 died of postoperative atelectasis, 1 died of excessive bleeding because of puncturing the pulmonary vein by mistake during the operation. Twenty-eight rats survived more than 30 days. A successful rate of 93.3% was achieved in the making of thymectomy model. Conclusion The results show that the model is easy to operate and the success rate is very high, and can be used in the experiment of thymectomy in the rats.
