自身免疫性胰腺炎術后激素治療初探△_《中國普外基礎與臨床雜志》

目的　分析自身免疫性胰腺炎（AIP）手術治療后的生化改變，并初步探討術后激素治療策略。
方法　回顧性分析2007年1月至2011年10月期間中山醫院普外科接受手術治療的6例AIP患者，其中4例患者因術前誤診為惡性腫瘤行切除術，2例術中穿刺證實為AIP行膽腸內引流術。2例行內引流術的AIP患者及1例行切除術的AIP患者術后予以潑尼松治療。所有病例檢查術前CA19-9水平；檢測圍手術期（術前1d，術后1d、4d及7d）肝功能，包括總膽紅素（TB）、直接膽紅素（DB）、γ-谷氨酰轉移酶（γ-GT）及γ-球蛋白（γ-GLB）；部分病例在術后檢測了IgG或IgG4；隨訪CT或MRI檢查評價胰腺病變緩解情況。
結果　6例AIP患者術前的CA19-9水平正常或輕度升高。5例主要病變在胰頭的AIP患者，術前γ-GT均明顯升高。1例胰體尾局限性AIP，由于膽總管未累及，圍手術期的TB/DB、γ-GT均在正常范圍；另外5例AIP患者，病變主要集中在胰頭，不管是行胰十二指腸切除術或者行內引流術，雖在術后1d或術后4d可能會出現一過性的TB/DB升高，但在術后7d可降到術前水平以下或正常水平；而γ-GT也表現為下降的趨勢。6例AIP患者術前γ-GLB均升高；4例行切除術的患者，術后γ-GLB呈現一種下降趨勢，而2例僅行內引流術的AIP患者，術后γ-GLB始終高于術前水平。4例AIP患者檢測了IgG，僅1例AIP患者檢測了IgG4，均高于正常。CT復查可發現胰腺腫脹程度較治療前明顯緩解。
結論　AIP患者施行手術后是否采取激素治療，要綜合參考γ-GLB、手術方式及有無胰外表現，CT或MRI等影像學改變是評估激素治療后效果的客觀指標之一。
