抗原B ELISA法對細粒棘球蚴病診斷價值的Meta分析_《中國循證醫學雜志》

目的　系統評價抗原B ELISA（AgB ELISA）法對細粒棘球蚴病的診斷價值。
方法　計算機檢索PubMed、EMbase、The Cochrane Library、EBSCO、CBM、CNKI、WanFang Data和超星Medalink數據庫，并手工檢索相關期刊，全面收集AgB ELISA法診斷細粒棘球蚴病的診斷性試驗，檢索時限從建庫至2012年7月5日。由2位研究者按納入與排除獨立篩選文獻、提取資料并按QUADAS標準評價納入研究的方法學質量后，采用Meta-Disc 1.4軟件進行Meta分析。
結果　最終納入8個研究，包括562例經金標準確診的細粒棘球蚴病患者，434例相似病例和303例健康人。8個研究間無閾值效應（靈敏度對數與1-特異度對數的Spearman相關系數=0.527，P=0.400 1），采用隨機效應模型進行Meta分析結果顯示，合并靈敏度、特異度、陽性似然比、陰性似然比及診斷比值比分別為0.76［95%CI（0.73，0.79）］、0.84［95%CI（0.82，0.86）］、5.20［95%CI（3.59，7.55）］、0.26［95%CI（0.18，0.35）］、23.93［95%CI（12.35，46.39）］，SROC曲線下面積（AUC）=0.889 7，Q=0.820 4。
結論　采用天然AgB和rAgB ELISA法檢測細粒棘球蚴病具有較大診斷價值。
