中國輸卵管積水不孕患者IVF-ET術前輸卵管切除預處理的必要性及安全性系統評價_《中國循證醫學雜志》

目的　系統評價中國輸卵管積水不孕患者體外受精-胚胎移植（IVF-ET）前輸卵管切除預處理的必要性及安全性。
方法　計算機檢索CBM、VIP、CNKI（均為2000～2010.12），查找中國輸卵管積水不孕患者IVF-ET術前行輸卵管切除術預處理的隨機對照試驗（RCT）。由兩位研究者按照納入與排除標準進行文獻篩選、資料提取和質量評價后，采用RevMan 5.0軟件進行Meta分析。
結果　共納入9個RCT，涉及輸卵管積水不孕患者687例、累計730個IVF-ET周期，9個RCT的質量評級均為B級。Meta分析結果顯示：中國輸卵管積水不孕患者在IVF-ET術前行輸卵管切除術預處理，① 會增加超排卵時Gn用量［WMD=1.23，95%CI（0.17，2.30），P=0.02］，但同時也增加了患者受精率［RR=1.07，95%CI（1.02，1.13），P=0.006］、卵裂率［RR=1.05，95%CI（1.00，1.09），P=0.03］及臨床妊娠率［RR=1.92，95%CI（1.41，2.61），P lt;0.000 1］；② 能降低流產率［RR=0.34，95%CI（0.13，0.86），P=0.002］；③ 不影響Gn的刺激天數［WMD= –0.27，95%CI（–0.59，0.06），P=0.11］、HCG日E2水平［WMD=59.15，95%CI（–9.61，127.91），P=0.09］、獲卵數［WMD= –0.27，95%CI（–0.44，0.99），P=0.46］、優質胚胎率［RR=1.02，95%CI（0.91，1.14），P=0.79］及異位妊娠率［RR=0.22，95%CI（0.03，1.82），P=0.16］。
結論　中國輸卵管積水不孕患者在行IVF-ET術前做輸卵管切除術預處理是必要且安全的。但由于納入試驗的方法學質量普遍偏低，因此期待更多設計合理的大樣本隨機雙盲對照試驗以提供更多高質量的證據。
