目的分析早產兒初始持續氣道正壓通氣（continuous positive airway pressure，CPAP）失敗的相關危險因素，為早產兒選擇有效的呼吸支持方式提供臨床依據。方法選取 2014 年 1 月—12 月在新生兒科住院初始呼吸支持為 CPAP 的早產兒 152 例，對其臨床資料進行回顧性研究，分析 CPAP 失敗的相關危險因素。結果單因素分析顯示出生體質量、胎膜早破、臍帶異常、宮內窘迫、生后窒息、孕母妊娠期高血壓、孕母妊娠期糖尿病、產前使用激素、使用 CPAP 時日齡、初始吸入氧濃度、初始氧分壓這 11 項因素與 CPAP 失敗的關聯有統計學意義；logistic 回歸分析表明低出生體質量、宮內窘迫、使用 CPAP 較晚是影響 CPAP 失敗的主要危險因素。結論綜合分析出生體質量、宮內窘迫、使用 CPAP 日齡等圍生期因素是 CPAP 失敗的主要預測因素。
