ObjectiveTo explore the prognostic value of stroke volume index (SVI) measured by transthoracic echocardiography (TTE) and cardiac troponin T (cTnT) in hemodynamically stable patients with acute pulmonary embolism (APE), and to establish a novel risk prediction model combining simplified pulmonary embolism severity index (sPESI), SVI and cTnT, and compare its performance with Bova score and modified FAST (mFAST) score. MethodsA total of 266 hemodynamically stable patients with confirmed APE from June 2019 to September 2021 were retrospectively included. All patients were followed up for 30 days via outpatient records or telephone. The primary endpoint was adverse outcomes. ResultsAdverse outcomes occurred in 31 (11.6%) of 266 patients within 30 days. Multivariate analysis showed that both SVI and cTnT were independent predictors of adverse events. The optimal cutoff values of SVI and cTnT were 31.8 mL/m2 and 16.5 ng/L, respectively. The new model divided patients into three risk groups with adverse event rates of 1.3%, 8.5% and 50.0%. The AUC of the new model was 0.80, superior to Bova score and mFAST score. ConclusionSVI and cTnT are independent predictors of adverse prognosis in non-high-risk APE patients. The new model integrating sPESI, SVI and cTnT provides effective risk stratification and has better predictive value than sPESI alone.
