        • Effect of delayed treatment on survival and prognosis of papillary thyroid microcarcinoma: A large sample real world study based on SEER database

          ObjectiveTo use real-world data from a large sample of papillary thyroid microcarcinoma (PTMC) in the SEER database to investigate the impact of delayed treatment on survival outcomes. MethodsA total of 40 761 patients with PTMC eligible for the study from the SEER database of the National Cancer Institute of the United States during 2000–2019 were selected as the study objects and divided into 3 groups according to the different delayed treatment time (0, 0–6 months, >6 months). Kaplan-Meir method was used to plot the survival curve and calculate 5-year cumulative disease-specific survival (DSS) rate and overall survival (OS) rate. Cox proportional hazard regression model was used to analyze the relationship between delayed treatment time, DSS and OS in PTMC patients and the influencing factors of prognosis. ResultsAmong the 40 761 patients, 7 575 (18.58%) were males and 33 186 (81.42%) were females, most of whom were females. The patients ranged in age from 3 to 97 years old [ (51.1±13.9) years old], of which 24 043 (58.99%) were <55 years old and 16 718 (41.01%) were ≥55 years old. Received treatment immediately after diagnosis in 30 823 patients (75.62%), 9 734 patients (23.88%) received treatment within 6 months after diagnosis, 204 patients (0.50%) received treatment 6 months after diagnosis. There were significant differences in age, sex, race, lymph node stage, radiotherapy, surgical method, number of lesions and invasion of thyroid capsule among the 3 groups (P< 0.001). The survival analysis results of the 3 groups showed that the delayed treatment time had no effect on DSS and OS of PTMC patients (P>0.05). The multivariate Cox proportional hazard regression model analysis results showed that the patient’s age ≥55 years old, male, married, lymph node metastasis, radiotherapy, total thyroidectomy and thyroid capsule invasion were the risk factors affecting DSS and OS in PTMC patients (P<0.05), while delayed treatment was not risk factors for DSS and OS in PTMC patients (P>0.05). ConclusionDelayed treatment is not an independent risk factor for DSS and OS in patients with PTMC, and active monitoring is a safe alternative to surgery for some PTMCS.

        • Musculoskeletal biomechanical analysis of the three-dimensional gastrocnemius and soleus muscles

          Musculoskeletal models based on multibody dynamics have been widely applied in human biomechanics research; however, conventional one-dimensional (1D) muscle models are limited in their ability to represent realistic muscle geometry and fiber architecture, resulting in restricted biomechanical predictions. Although three-dimensional (3D) finite element (FE) muscle models can provide more detailed mechanical information, high computational cost limits their application at the whole lower-limb scale. To address these limitations, this study develops a unilateral lower-limb musculoskeletal model with hybrid muscles based on the TLEM2.0 database, in which the soleus-gastrocnemius muscle group was represented using 3D FE models, while the remaining muscles were modeled using 1D Hill-type formulations. By coupling FE analysis with muscle recruitment optimization, muscle activation patterns of the right lower limb during the stance phase of level walking were obtained. Model predictions were validated against in vivo knee joint reaction forces and surface electromyography (sEMG) data. The results demonstrated good agreement between the predicted and experimentally measured knee joint reaction forces, and the predicted force patterns of the medial and lateral gastrocnemius muscles corresponded well with sEMG trends. In addition, the model enabled detailed observation of muscle deformation as well as internal tensile stress and strain distributions in the 3D muscles during gait. This hybrid musculoskeletal modeling framework achieves a balance between computational efficiency and biomechanical fidelity, providing an effective platform for investigating lower-limb muscle biomechanics.

        • Comparison between layered anastomosis and mechanical anastomosis of tubular stomach and cervical esophagus in esophagectomy

          Objective To compare the outcome of tubular stomach and cervical esophagus laminated anastomosis and mechanical anastomosis. Methods A total of 128 patients with middle and upper esophageal cancer in our hospital from January 2013 to January 2016 were randomly divided into two groups, 64 patients in each group and all patients underwent esophagectomy. In the group A, there were 46 males and 18 females with age of 40–75 years, treated with tubular stomach and cervical esophagus layered anastomosis. There were 51 males and 13 females with age of 43–71 years in the group B receiving mechanical anastomosis. We observed the short-term and long-term efficacy as well as complications. Results In the group A, there was cervical anastomotic fistula in 1 patient, anastomotic stenosis in 2 patients and reflux esophagitis in 12 patients. In the group B, 1 patient suffered chylothorax, 8 patients cervical anastomotic fistula, 4 patients anastomotic stenosis and 14 patients reflux esophagitis. Conclusion Layered anastomosis can effectively reduce the incidence of anastomotic fistula and stenosis. Tubular stomach can effectively prevent and reduce the reflux esophagitis, and the high long-term quality of life will be achieved after the operation.
