        • Progress in diagnosis and treatment strategies for multiple primary lung cancer

Multiple primary lung cancer is a special type of lung cancer. Its detection rate is increasing year by year, and there is no clear diagnosis and treatment strategy, which makes the diagnosis and treatment become a hotspot in clinical work. The molecular genetics is gradually changing the status quo of relying only on imaging and tumor-free interval to distinguish lung metastasis from multiple primary lung cancer, and it is an effective method for differential diagnosis and prediction of biological behavior of lung cancer. Based on our experience and other studies, it is recommended that surgical treatment should be preferred when there is no contraindication. The advantages and disadvantages of bilateral thoracoscopic surgery for bilateral multiple primary lung cancer during the same period are discussed, and its feasibility and safety are confirmed. For the lesions that cannot be completely resected, active surgical local treatment is recommended. The diagnosis and treatment of multiple primary lung cancer is still a clinical difficulty, and we hope that our research can provide theoretical and practical guidance for clinicians.

        • Chinese expert consensus on preoperative assisted positioning technology of small pulmonary nodules (2025 edition)

How to accurately locate pulmonary nodules in thoracic surgery is a key challenge in surgery. "Chinese expert consensus on preoperative assisted positioning technology of small pulmonary nodules (2025 edition)" (hereafter referred to as the consensus) is an update version incorporating the latest evidence and clinical practice based on the 2019 edition, aiming to provide reference for clinical practice. The consensus summarizes commonly used preoperative assisted positioning technologies, such as percutaneous lung puncture positioning (Hookwire positioning needles, metal spring coils, etc.), transbronchial puncture positioning (electromagnetic navigation bronchoscopes, etc.), and virtual reality and 3D printing positioning, and introduces the application indications, operating specifications and potential complications of different positioning technologies, analyzes the existing technical advantages and shortcomings, and emphasizes the selection of the most appropriate positioning method according to the specific situation of the patient, so as to improve the success rate and safety of the surgery, and promotes the minimally invasive and precise development of thoracic surgery.

        • Guidelines for the perioperative diagnosis and treatment of oncogene-driven non-small cell lung cancer (2026)

Lung cancer constitutes the most prevalent and lethal malignant tumor in China. Approximately 85% of lung cancer diagnoses correspond to the non-small cell histological subtype [non-small cell lung cancer (NSCLC)]. Despite surgery being the mainstay for early-stage disease, postoperative recurrence remains high and adjuvant chemotherapy offers limited benefit. In recent years, targeted therapy has demonstrated substantial advantages in driver mutation-positive NSCLC. To this end, the Lung Cancer Medical Education Committee of the Chinese Medical Education Association developed guidelines based on a systematic review of evidence through November 2025, using the Grading of Recommendations, Assessment, Development and Evaluations (GRADE) approach and a modified Delphi method. Focusing on epidermal growth factor receptor (EGFR) and anaplastic lymphoma kinase (ALK), and addressing ROS proto-oncogene 1 (ROS1), B-Raf proto-oncogene serine/threonine kinase (BRAF) V600E mutation, and mesenchymal-epithelial transition factor (MET) exon 14 (METex14) skipping, the guideline covers molecular testing, neoadjuvant/adjuvant therapy, perioperative strategies, minimal residual disease monitoring, and postoperative surveillance. It defines testing requirements, specifies stage-directed and subtype-specific treatments, and standardizes minimal residual disease monitoring. These recommendations emphasize precision and feasibility to improve survival and quality of life.
