腹腔鏡膽總管探查取石術后膽管一期縫合與T管引流比較的系統評價_《中國循證醫學雜志》

目的　系統評價腹腔鏡下膽總管探查取石后膽管一期縫合（LBEPS）與腹腔鏡下膽總管探查取石后T管引流（LCHTD）治療膽總管結石的效果，為醫患雙方合理選擇術式提供證據。
方法　計算機檢索Cochrane 圖書館臨床對照試驗數據庫（2010 年第2 期）、PubMed（1978 ～ 2010）、EMbase（1966 ～ 2010）、CBM （1978 ～ 2010）、CNKI（1979 ～ 2010）和中華醫學會數字化期刊系統（1990 ～ 2010），并手工檢索中、英文已發表的資料和會議論文并追索納入文獻的參考文獻，查找比較LBEPS與LCHTD治療膽總管結石的隨機或半隨機對照試驗。在對納入研究進行方法學質量評價后，采用RevMan 5.0軟件進行Meta分析。
結果　共納入3個隨機對照試驗和1個半隨機對照試驗，合計274例患者。Meta分析結果表明：與LCHTD相比，LBEPS能縮短手術時間［WMD= –17.11，95%CI（–25.86，–8.36）］、腹腔引流時間［WMD= –0.74，95%CI（–1.39，–0.10）］、術后住院天數［WMD= –3.30，95%CI（–3.67，–2.92）］和減少住院費用［WMD= –2 998.75，95%CI （–4 396.24，–1 601.26）］，而且還能降低由放置T管引起的脫管及拔管后膽漏、膽汁性腹膜炎等并發癥的發生［RR=0.56，95%CI（0.29，1.09）］。
結論　LBEPS治療膽總管結石總體療效優于與LCHTD，其前提是需嚴格掌握手術適應癥。由于納入研究質量普遍較低，上述結論尚需開展更多設計合理、執行嚴格的多中心大樣本且隨訪時間足夠的隨機對照試驗加以驗證。
