目的研究原發性肺肉瘤的臨床表現、影像學診斷、病理學診斷和治療,提高臨床醫生對肺肉瘤的診治水平。方法分析我院2005年3月至2015年1月間經病理診斷的30例肺肉瘤患者,其中男18例,女12例;平均年齡(55.6±10.2)歲,發病年齡40~75歲;初診癥狀為咳嗽、咳痰10例, 咯血2例,胸痛9例,胸悶、氣促8例,體檢意外發現1例。結果 30例患者均表現為孤立性占位病變。其中,28例行手術治療,2例行非手術治療。手術治療的28例中,右肺上葉切除12例,右肺中、下葉切除2例,右肺下葉切除4例,左肺上葉切除5例,左肺上葉部分切除2例,左肺下葉切除2例, 左胸膜全肺切除1例。免疫組織化學檢查結果為波形蛋白陽性24例,結蛋白陽性20例。病理類型:惡性纖維組織細胞瘤9例,滑膜肉瘤7例,脂肪肉瘤5例,平滑肌肉瘤3例,橫紋肌肉瘤3例,軟骨肉瘤2例,胸膜肺母細胞瘤1例。2例平滑肌肉瘤及1例橫紋肌肉瘤患者存活超過5年。全組患者1年生存率為40%,5年生存率為10%。結論原發性肺肉瘤主要表現為孤立性占位病變,其波形蛋白、結蛋白免疫組織化學檢查可呈陽性。外科治療為首選治療方式,但預后差。其預后與腫瘤組織學類型、大小和位置有關。
