目的總結經皮內鏡下病灶清除與藥物灌洗治療脊柱結核的臨床療效。方法回顧分析 2018 年 1 月—2019 年 12 月采用經皮內鏡下病灶清除與藥物灌洗治療的 25 例脊柱結核患者臨床資料。其中男 18 例，女 7 例；年齡 29～85 歲，平均 59.4 歲。病程 1～120 個月，中位時間 17.5 個月。結合手術前后患者紅細胞沉降率（erythrocyte sedimentation rate，ESR）、C 反應蛋白（C reactive protein，CRP）、疼痛視覺模擬評分（VAS）、Oswestry 功能障礙指數（ODI）、神經功能美國脊髓損傷協會（ASIA）分級及影像學資料，綜合評估患者術后功能恢復和脊柱結核治愈情況。結果所有患者順利完成手術，手術時間（103.5±33.9）min，術中出血量（27.5±17.5）mL。術后切口均 Ⅰ 期愈合。25 例均獲隨訪，隨訪時間 12～22 個月，平均 15 個月。末次隨訪時 ESR、CRP 及 VAS 評分、ODI 均較術前顯著改善（P<0.05）；ASIA 分級除 1 例 D 級外，其余 24 例均為 E 級；CT 及 MRI 示椎體病灶愈合良好，未見明顯塌陷，無結核復發。結論經皮內鏡下病灶清除與藥物灌洗是一種治療脊柱結核較為安全、有效、微創的方式。
