目的探討咬合導板及頜間牽引釘技術在游離腓骨瓣修復下頜骨缺損中的應用價值。方法2011年8月－12月，收治7例下頜骨腫瘤患者。男4例，女3例；年齡32～65歲，中位年齡50歲。下頜骨成釉細胞瘤5例，牙齦癌2例。術前制備咬合導板并確定頜間牽引釘植入位置；術中5例行下頜骨半側切除、2例行保留髁突的下頜骨半側切除后，切取長11～13 cm的腓骨瓣修復下頜骨缺損，腓骨瓣固定時采用咬合導板和頜間牽引釘技術恢復咬合關系。供區直接拉攏縫合。結果手術時間7～12 h，平均9.5 h。術后腓骨瓣均成活，供、受區切口均Ⅰ期愈合。患者均獲隨訪，隨訪時間10～14個月，平均12.3個月。患者面部對稱，咬合關系良好，張口度及下頜側向運動正常，雙側顳下頜關節區無疼痛等不適。全口曲面體層片顯示下頜骨缺損區域重建形態及固位良好。隨訪期間腫瘤無復發。結論游離腓骨瓣移植修復下頜骨缺損術中應用咬合導板和頜間牽引釘技術，能簡化手術操作，精確恢復患者面部外形和咬合關系，提高術后口腔舒適性。
