經腓骨小頭上入路治療脛骨平臺后外側髁骨折的臨床研究_《中國修復重建外科雜志》

目的探討經腓骨小頭上入路治療脛骨平臺后外側髁骨折的療效。方法2010年6月－2012年2 月，收治20例脛骨平臺后外側髁骨折患者。男12例，女8例；年齡28～58歲，平均42.2歲。致傷原因：交通事故傷14例，高處墜落傷4例，扭傷2例。均為閉合性骨折。單純后外側髁骨折1例，后外側髁骨折合并其他部位骨折19例。合并外側半月板損傷5例，內側半月板損傷2例，前交叉韌帶損傷1例。患者受傷至入院時間90 min～32 h，中位時間4.5 h。入院至手術時間5～12 d，平均7.8 d。取腓骨小頭上入路，骨折復位Pilon鋼板固定；一期處理合并骨折及半月板損傷。結果術后切口均Ⅰ期愈合。患者均獲隨訪，隨防時間6～26個月，平均19.1個月。術后無肢體麻木、關節不穩等并發癥發生。X線片復查示，骨折均愈合，愈合時間8～12周，平均10.2周；隨訪期間均無內固定物松動及關節面丟失發生。末次隨訪時按Rasmussen膝關節功能評分標準，獲18～30分，平均27.9分；其中優16例，良3例，中1例，優良率95%。結論經腓骨小頭上入路治療脛骨平臺后外側髁骨折手術操作簡便，安全有效，可獲得較好療效。
