        • Perioperative management and outcomes of 22 patients treated with left ventricular assist device implantation

          Objective To evaluate the perioperative management strategies and clinical outcomes in a cohort of 22 patients with end-stage heart failure undergoing implantation of left ventricular assist device (LVAD). Methods A retrospective analysis was conducted on the clinical records of patients with end-stage heart failure treated with LVAD at West China Hospital of Sichuan University between January 2021 and June 2025. Surgical outcomes were assessed using perioperative variables, including operative duration, cardiopulmonary bypass and aortic cross-clamp time, length of ICU stay, incidence of adverse events, as well as cardiac function and hemodynamic status at the time of discharge. ResultsThe study included 22 patients (19 male, 3 female) with a mean age of (47.9±12.6) years (range: 27-67 years). Preoperatively, all patients were classified as New York Heart Association (NYHA) functional class Ⅳ. All implantations were performed successfully; however, there were two perioperative mortalities due to gastrointestinal bleeding. Post-implantation, significant improvements in cardiac function and hemodynamics were observed: left ventricular ejection fraction increased from a baseline of 24.0%±5.0% to 31.0%±7.0% (P=0.002), tricuspid annular plane systolic excursion improved from (10.0±2.5) mm to (15.0±3.0) mm (P<0.001), fractional area change increased from 22.5%±6.0% to 30.0%±6.5% (P<0.001), and cardiac index rose from (1.9±0.4) L/(min·m2) to (2.8±0.5) L/(min·m2) (P<0.001). The average length of stay in the ICU was 7 (5, 10) days. No device malfunctions or bleeding events necessitating surgical re-exploration were recorded. Conclusion LVAD therapy provides effective cardiac unloading and enhances systemic perfusion in the perioperative setting, achieving a high rate of survival with the device. Clinical complications are generally manageable. Strengthening right heart evaluation and protection, alongside optimizing perioperative management protocols, holds the potential to further enhance patient outcomes.

        • An electroencephalogram-based emotion recognition method using multi-branch convolutional neural networks and Transformer

          Electroencephalogram (EEG)-based emotion recognition is an important research area in affective computing and mental health assessment. To address the insufficient modeling of long-term dependencies in EEG signals, this paper proposes an EEG emotion recognition method based on multi-branch convolutional neural networks (CNN) and Transformer (MCT). The proposed method employs a multi-scale CNN to extract local temporal features from EEG signals and constructs a parallel CNN-Transformer architecture to capture both short-term variations and long-term dependencies, thereby enabling temporal feature modeling at different time scales. Furthermore, a dual-branch convolutional structure is utilized to learn both global and local spatial channel features of EEG signals. A convolutional block attention module (CBAM) is then introduced to fuse the spatio-temporal features of EEG signals. Experimental results show that the proposed MCT model achieves a classification accuracy of 83.83% on the Shanghai Jiao Tong University emotion EEG dataset (SEED). On the music emotion EEG dataset (MEEG), it attains accuracies of 90.00% and 92.62% for the arousal and valence dimensions, respectively. On the database for emotion analysis using physiological signals (DEAP), it attains accuracies of 61.30% and 61.04% for the arousal and valence dimensions. The accuracy results on all three datasets outperform those of the best-performing baseline models. These findings indicate that MCT can effectively learn discriminative features associated with emotional states, providing a new perspective for EEG-based emotion recognition research.

        • Effects of removing superficial layer of cartilage on the surface morphology and mechanical behavior of cartilage

          Superficial cartilage defect is an important factor that causes osteoarthritis. Therefore, it is very important to investigate the influence of superficial cartilage defects on its surface morphology and mechanical properties. In this study, the knee joint cartilage samples of adult pig were prepared, which were treated by enzymolysis with chymotrypsin and physical removal with electric friction pen, respectively. Normal cartilage and surface treated cartilage were divided into five groups: control group (normal cartilage group), chymotrypsin immersion group, chymotrypsin wiping group, removal 10% group with electric friction pen, and removal 20% group with electric friction pen. The surface morphology and structure of five groups of samples were characterized by laser spectrum confocal microscopy and environmental field scanning electron microscopy, and the mechanical properties of each group of samples were evaluated by tensile tests. The results show that the surface arithmetic mean height and fracture strength of the control group were the smallest, and the fracture strain was the largest. The surface arithmetic mean height and fracture strength of the removal 20% group with electric friction pen were the largest, and the fracture strain was the smallest. The surface arithmetic mean height, fracture strength and fracture strain values of the other three groups were all between the above two groups, but the surface arithmetic mean height and fracture strength of the removal 10% group with electric friction pen, the chymotrypsin wiping group and the chymotrypsin soaking group decreased successively, and the fracture strain increased successively. In addition, we carried out a study on the elastic modulus of different groups, and the results showed that the elastic modulus of the control group was the smallest, and the elastic modulus of the removal 20% group with electric friction pen was the largest. The above study revealed that the defect of the superficial area of cartilage changed its surface morphology and structure, and reduced its mechanical properties. The research results are of great significance for the prevention and repair of cartilage injury.

        • Practical exploration of improving submission rate of pathogen examination in counterpart supported high-altitude county hospitals based on failure mode and effect analysis

          Objective To explore clinical effect of failure mode and effect analysis in improving the submission rate of pathogen examination in counterpart supported high-altitude county hospitals, and formulate practical measures and methods suitable for high-altitude county hospitals to improve the submission rate of pathogen examination. Methods Patients admitted to the People’s Hospital of Ganzi County between January and December 2024 were selected. The data of hospitalized patients between January and June 2024 were as the control group, and the data of hospitalized patients between July and December 2024 were as the intervention group. The study analyzed and compared the submission rate of pathogen testing and the pass rate of microbiological test specimens before antimicrobial treatment between the two groups. Results A total of 3 984 patients were included. Among them, there were 1 748 cases in the control group and 2 236 cases in the intervention group. A total of 10 risk factors and 2 high-risk points were identified. There were statistically significant differences in the submission rate of pathogen specimens before antibiotic treatment [36.21% (633/1 748) vs. 49.33% (1 103/2 236); χ2=68.646, P<0.001] and the qualified rate of microbiological test specimens [26.75% (122/456) vs. 36.45% (261/716); χ2=11.910, P=0.001] between the control group and the intervention group. Conclusions Failure mode and effect analysis can effectively find out the weak points in low pathogen examination submission rate in high-altitude county hospitals. According to the high-risk points to guide the formulation of relevant measures, the pathogen submission rate in the region can be effectively improved.
