目的 探討新型玻璃化冷凍法對人卵巢組織超微結構的保存效果，尤其是對卵巢間質細胞的保存效果。 方法 收集2007年6月－2009年1月在我院行婦科手術的患者卵巢組織共8例，使用新型玻璃化冷凍法和慢速冷凍法保存，比較兩種冷凍方法對于始基卵泡卵母細胞、顆粒細胞、卵泡周圍間質細胞超微結構保存效果。 結果 對于始基卵泡卵母細胞和顆粒細胞，發現兩種冷凍方法之間無顯著差異（P＞0.05）；在間質細胞保存中，新型玻璃化冷凍法與慢速冷凍法相比較，正常間質細胞百分率增加（P＜0.05）。 結論 新型玻璃化冷凍法與慢速冷凍法相比能更好地保存卵泡周圍間質細胞，在深低溫保存人卵巢組織中具有較廣闊的應用前景。
目的 通過對腹部手術后自控靜脈鎮痛（PCIA）不同藥物配方的研究，探討酒石酸布托啡諾與舒芬太尼用于術后PCIA臨床效果。 方法 將2012年2月－8月收治的60例麻醉分級為Ⅰ～Ⅲ級需術后鎮痛的腹部手術患者（均無心、肺、肝、腎、腦、內分泌疾病及過敏史）隨機分成兩組：酒石酸布托啡諾組（N組，n=30），舒芬太尼組（S組，n=30）。觀察鎮痛效果和不良反應發生率。 結果 兩組鎮痛效果差異無統計學意義（P＞0.05），不良反應（包括惡心、嘔吐、頭暈、嗜睡、皮膚瘙癢、呼吸抑制、尿潴留等），N組發生率均低于S組（P＜0.05）。 結論 酒石酸布托啡諾用于PCIA安全、有效，不良反應少。
With the accelerating population aging process, increasing attention has been paid to the nutritional risks among elderly patients in the intensive care unit (ICU). Elderly patients are often complicated by decreased physiological reserve, reduced muscle mass, multiple comorbidities, and critical illness-related hypercatabolism. Therefore, nutritional assessment and supportive therapy are featured by substantial complexity and individual heterogeneity. Focusing on the association between malnutrition and clinical outcomes in elderly ICU patients, this article reviews commonly-used nutritional risk screening and assessment tools, compares the indications and initiation timing of oral nutritional supplements, enteral nutrition and parenteral nutrition, and summarizes the principles for supplying energy, protein, fat, vitamins, trace elements and electrolytes as well as key points for therapeutic monitoring. It aims to provide references for individualized and dynamic nutritional management of elderly critically ill patients.
