目的 研究COPD 患者機械通氣期間的多尿現象及其可能的機制。方法 分別應用酶聯免疫吸附法和放射免疫法檢測26 例COPD 患者在機械通氣前( 0 h) , 機械通氣24 h、48 h、72 h、1 周血漿、尿液胱抑素C( CyC) 濃度和血漿心鈉素( ANF) 、抗利尿激素( ADH) 及醛固酮( ALD) 濃度,并記錄患者每日尿量, 同時檢測30 例健康人標本作為正常對照組。結果 COPD 患者多尿現象多出現在機械通氣24 ～72 h; 機械通氣前的血、尿CyC 和血漿ANF、ADH、ALD 濃度分別為( 4.87 ±0. 51) mg/L、( 0.58 ±0. 13) mg/L、( 37. 02 ±4. 35) pmol /L、( 8.61 ±1. 43) pmol /L 和( 925. 4 ±142. 7) pmol /L, 與對照組相比均顯著升高( P lt; 0. 01) ; 機械通氣期間逐漸下降, 血CyC 和ANF 在24 h, ADH在72 h, 尿CyC 在1 周時的濃度與對照組無顯著差異( P gt;0. 05) , 但ALD 直到1 周時仍高于對照組( P lt;0. 05) 。結論 COPD 機械通氣期間有相當多的患者出現多尿現象。這可能主要與ADH 和腎素-血管緊張素-醛固酮系統在機械通氣期間的再調整與適應不良有關。
【摘要】 目的 探討多尿性梗阻性腎衰的診斷和治療。 方法 回顧性分析2004年5月-2007年5月收治的15例多尿性梗阻性腎衰患者的臨床資料。 結果 經治療后15例患者腎功能均恢復正常。 結論 多尿性梗阻性腎衰的治療關鍵在于診斷，診斷明確后可以用各種方法解除功能性孤立腎的梗阻，挽救腎臟功能。【Abstract】 Objective To investigate clinical experience of the diagnosis and therapy for the hyperdiuresis post-renal acute renal failure. Methods Clinical data of 15 patients with hyperdiuresis post-renal acute renal failure admitted from May 2004 to May 2007 were retrospectively analyzed. Results All of the patients were treated successfully without death and complications. Conclusion The core for the treatment of hyperdiuresis post-renal acute renal failure is the diagnosis. Several therapies can be used to relieve obstruction and save renal function after precise diagnosis.
