目的研究成人間活體肝移植（A-ALDLT）供體術中的麻醉管理及術中病理生理的變化，探討如何提高A-ALDLT的供體麻醉質量。方法回顧分析2005年9月至2007年1月華西醫院47例A-ALDLT供體術中基本生命體征、麻醉處理、肝功能、凝血功能及并發癥發生的情況。結果 47例供體術前一般狀況較好，ASA分級均為Ⅰ級。術中采用靜吸復合麻醉方法, 連續監測心電圖、脈搏血氧飽和度、體溫，有創動脈測壓并行中心靜脈置管，用血液回收機采集出血洗滌后回輸。47例供體術中出血量平均(603.13±317.00) ml，輸入自體血(381.25±171.15) ml，僅4例供體輸入異體血。術中心率和平均動脈壓平穩，插管后、切肝前、后及關腹前各時點差異無統計學意義(P gt;0.05)；切肝前控制中心靜脈壓(CVP)，切肝前、后CVP明顯低于插管后及關腹前(P＜0.05)。切肝后至術后第1 d，HGB及Alb明顯下降(P＜0.05)，ALT、AST及TBIL明顯升高(P＜0.05)； PT于切肝后升高(P＜0.05), APTT于術后第1 d開始明顯升高(P＜0.05)。47例供體均于術后第1 d在肝移植ICU拔氣管導管，蘇醒好。術后有3例(6.38%)發生并發癥，分別為漏膽、門靜脈血栓形成及大量胸腔積液生成，給予相應處理后好轉出院。結論供體肝葉切除術中采用丙泊酚、瑞芬太尼和異氟醚靜吸復合麻醉，降低麻醉藥物對肝臟的損害，保證肝臟充分的氧供，維持平穩的麻醉，采用有效的血液保護措施，包括降低CVP減少出血和血液回收避免異體血的輸入，是保證供體安全、減少并發癥的關鍵。
