早期直腸癌中p53和hMLH1的表達_《中國普外基礎與臨床雜志》

目的探討p53基因和hMLH1基因的突變是否是直腸癌發生的早期事件之一。
方法用PV-9000二步法免疫組化檢測技術對我院2003年2月至2009年7月期間的32例早期直腸癌、32例直腸腺瘤及30例正常直腸黏膜組織石蠟切片進行p53和hMLH1基因蛋白表達的檢測。
結果 ①正常直腸黏膜組織、直腸腺瘤組織及早期直腸癌組織中p53蛋白表達陽性率分別為0(0/30)、59.38%(19/32)及68.75%(22/32),其在直腸腺瘤組織和早期直腸癌組織中的p53蛋白表達陽性率差異無統計學意義(P gt;0.05),但二者均明顯高于其在正常直腸黏膜組織中的表達(P lt;0.01); hMLH1蛋白在前述三種組織中的表達陰性率分別為0(30/30)、12.50%(4/32)和50.00%(16/32)，其在早期直腸癌組織中的表達陰性率明顯高于在直腸腺瘤組織和正常直腸黏膜組織中的表達（P lt;0.01）。②在直腸腺瘤組織和早期直腸癌組織中p53陽性表達同時合并hMLH1陰性表達者分別占9.38%(3/32)及37.50%(12/32)，二者間差異有統計學意義(P=0.008)。③p53陽性表達和hMLH1陰性表達均與早期直腸癌的分化程度有關(P lt;0.05)，而均與患者的性別、年齡、腫瘤最大徑、浸潤深度及大便潛血均無關(P gt;0.05)。④p53陽性表達與腺瘤的增生程度有關(P=0.009)，hMLH1陰性表達與腺瘤的增生程度無關(P gt;0.05)。
結論 p53基因和hMLH1基因的同時突變致癌作用明顯，可能是直腸癌發生的早期事件之一。
