腹腔鏡闌尾切除術臨床價值再探討_《中國普外基礎與臨床雜志》

目的再次探討腹腔鏡闌尾切除術的手術價值。方法收集川北醫學院附屬醫院普通外科2009年4月至2010年12月期間收治的已行闌尾切除術的闌尾炎患者共200例的臨床資料，比較行腹腔鏡闌尾切除術(腹腔鏡組)和開腹闌尾切除術(開腹組)患者的手術時間、術后住院時間、住院費用、術后肛門排氣時間、術后疼痛得分、手術并發癥等。結果腹腔鏡組8例患者中轉開腹。腹腔鏡組住院費用高于開腹組(P lt;0.05); 慢性闌尾炎、急性單純性闌尾炎、發病36 h內的急性化膿性闌尾炎在腹腔鏡組較開腹組手術時間短、術后住院時間短、肛門排氣時間較早、術后疼痛輕(P lt;0.05); 超過36 h的急性化膿性闌尾炎及急性壞疽性闌尾炎2組在術后住院時間、肛門排氣時間、術后疼痛方面差異均無統計學意義(P gt;0.05); 急性壞疽性闌尾炎在腹腔鏡組手術時間較開腹組長(P lt;0.05)，但腹腔鏡組切口感染率較開腹組低(P lt;0.05)。結論慢性闌尾炎、急性單純性闌尾炎、發病36 h內的急性化膿性闌尾炎經腹腔鏡闌尾切除術治療效果確切，優點突出，應用價值明確; 而對于發病超過36 h的急性化膿性闌尾炎及急性壞疽性闌尾炎腹腔鏡闌尾切除術手術操作困難，中轉率高，術后恢復較開腹手術沒有明顯優勢，并且增加了醫療費用，應用價值不大。
