復雜巨大肝腫瘤的精確切除體會(附52例報道)_《中國普外基礎與臨床雜志》

目的總結復雜巨大肝腫瘤的精確切除經驗。方法2008年4月至2009年8月期間四川大學華西醫院肝膽胰外科行復雜巨大肝腫瘤精準切除52例，術前仔細進行肝臟儲備功能、肝臟影像學等評估，術中選擇適當的手術入路、血流阻斷方式、斷肝新技術、結合術中B超等方法。結果本組病例平均手術時間為350 min (210～440 min)，半肝血流阻斷時間平均為43 min (8～57 min)，術中平均出血量為370 ml (250～1 150 ml)，谷丙轉氨酶恢復正常平均時間為10 d (7～14 d)，總膽紅素恢復正常平均時間為4.5 d (3～10 d); 術后并發癥發生率低，僅6例出現輕度漏膽，無肝功能衰竭等重大并發癥，無死亡病例。結論精準肝切除治療復雜巨大肝腫瘤安全、可行。
