低位腹主動脈內球囊阻斷技術在骨盆和骶骨腫瘤切除術中的應用_《中國普外基礎與臨床雜志》

目的探討應用低位腹主動脈內球囊阻斷技術在骨盆或骶骨腫瘤切除術中的臨床應用價值。方法 2006年4月至2010年4月期間我院應用低位腹主動脈內球囊阻斷技術控制24例骨盆和骶骨腫瘤手術過程中的出血。術中先用球囊導管阻斷低位腹主動脈，每次阻斷腹主動脈60 min，若超過60 min間隔10～15 min后再阻斷，然后再行腫瘤切除。結果本組患者平均手術時間為153 min (40~245 min); 阻斷低位腹主動脈后，手術中平均出血為310 ml (200~650 ml)，手術野干凈，解剖清晰，腫瘤周圍的血管、神經以及盆腔器官明確，腫瘤邊界容易確定，腫瘤被完整切除，腫瘤的術野污染小。手術全過程中，患者血壓穩定，術后患者腎功能及盆腔器官均正常。1例患者發生穿刺側髂動脈血栓形成，通過Fogarty取栓導管取出血栓后下肢血供恢復，術后無缺血癥狀，其余患者雙下肢均正常，無并發癥發生。結論應用低位腹主動脈內球囊阻斷技術切除骨盆和骶骨腫瘤可有效減少術中出血，提高手術的安全性和完整切除率，是一項具有實用價值的臨床技術。
