目的　總結保留皮膚乳腺癌改良根治術后即時乳房再造方法的治療體會。
方法　筆者所在醫院2008年5月至2011年12月期間對10例早期乳腺癌患者行保留大部分皮膚的改良根治術，同時應用下腹部腹直肌帶蒂肌皮瓣（TRAM）行即時乳房再造，就該10例患者的臨床資料進行回顧性分析。
結果　10例患者中1例TRAM皮瓣部分壞死，1例乳頭部分壞死，1例再造乳房乳尾部脂肪部分壞死伴腋下傷口感染；無腹壁切口疝發生，無全部TRAM皮瓣壞死病例。術后患者對再造乳房形態評價非常滿意5例，比較滿意4例，不滿意1例。
結論　保留皮膚的乳腺癌改良根治術后即時TRAM皮瓣乳房再造方法的切口小，再造乳房形態好，是一種安全有效的治療早期乳腺癌患者的方法。
