胃癌根治術中No.12a 淋巴結正確清掃的意義和技巧_《中國普外基礎與臨床雜志》

目的　探討胃癌根治術中No.12a淋巴結清掃的意義和技巧。
方法　回顧性分析筆者所在科室行No.12a淋巴結清掃的68例Ⅳ期胃癌患者的臨床資料，結合筆者的手術實踐提出在胃癌根治術時，清掃該組淋巴結的經驗和體會。
結果　68例Ⅳ期胃癌中共檢出No.12a淋巴結556枚（5～11枚/例），平均8.17枚/例，No.12a淋巴結轉移率為33.27% （185/556）。本組病例術后無吻合口漏、淋巴管瘺、出血等并發癥發生。
結論　熟悉肝固有動脈周圍解剖以及鞘內淋巴結清掃，胃左靜脈和胃右動脈自根部結扎以及門靜脈前壁和胃十二指腸動脈的充分顯露是徹底清掃No.12a淋巴結的關鍵。
