外傷性脾破裂患者及肝硬變患者脾切除術后血小板變化情況研究_《中國普外基礎與臨床雜志》

目的　比較外傷性脾破裂患者與肝硬變門靜脈高壓癥患者脾切除術后血小板變化的差異，分析可能的原因及其臨床意義。
方法　統計我院2008年7月至2009年12月期間47例外傷性脾破裂患者及36例肝硬變患者行脾切除術前后的血小板計數，比較脾切除術后2組患者血小板計數及其變化趨勢的差異。
結果　脾破裂組47例患者術后均出現血小板異常升高，血小板計數最高值300×109/L～600×109/L者6例，600×109/L～900×109/L
者21例，＞900×109/L者20例；肝硬變組36例患者中有26例術后血小板最高值超過300×109/L，8例在100×109/L～300×109/L之間，2例仍低于100×109/L。2組患者術后血小板最高值分布的差異具有統計學意義（P=0.00）。脾破裂組患者與肝硬變組患者相比，術后血小板升高更明顯，下降更緩慢（P＜0.05）。Child A級患者血小板異常的持續時間及血小板升高幅度均超過Child B、C級患者（P=0.006，P=0.002）。
結論　脾破裂和肝硬變患者，因肝功能及全身狀況的差異，術后血小板數量的變化有明顯的不同。術后應根據血小板計數及凝血功能情況采取相應的治療，方可取得良好的治療效果。
