兩孔腹腔鏡膽囊切除術的探討_《中國普外基礎與臨床雜志》

目的　探討兩孔腹腔鏡膽囊切除術（LC）的安全性、可行性及優勢。
方法　回顧性分析2008年6月至2010年10月期間無錫市人民醫院開展的兩人兩孔LC（兩孔LC組，n=46）及同期完成的兩人三孔LC（三孔LC組，n=68）膽囊結石患者的臨床資料，比較2組的手術時間、術中出血量、術后進食時間、術后疼痛評分、術后住院時間及總住院費用。
結果　114例患者手術均獲成功，術中、術后無并發癥發生。兩孔LC組的手術時間明顯長于三孔LC組（P＜0.05）；2組的術中出血量、術后進食時間、術后疼痛評分及術后住院時間比較差異均無統計學意義（P＞0.05）；兩孔LC組的住院費用明顯低于三孔LC組（P＜0.05）。
結論　兩人兩孔LC在單純膽囊結石病例安全、可行，總住院費用低于兩人三孔LC；在膽囊結石合并急性膽囊炎病例，因視野及操作均受限，應謹慎開展。
