CT圖像上結直腸癌肝轉移灶的分布特點_《中國普外基礎與臨床雜志》

目的　探討結直腸癌肝轉移灶的分布特點及其與原發腫瘤部位的關系。
方法　回顧性分析我院2010年1月至2012年4月期間的105例結直腸癌伴肝轉移患者的多排螺旋CT圖像及臨床資料，記錄腫瘤原發部位、CT圖像中轉移灶數目、位置及腸系膜下靜脈（IMV）匯入門靜脈的解剖位置。
結果　①腫瘤原發灶位于右半結腸組及左半結腸組，轉移灶位于肝右葉與肝左葉的比例分別約為2∶1、1.2∶1，2組間構成比差異有統計學意義（χ2=8.709，P=0.003）。②腫瘤原發灶位于左半結腸組中，其IMV匯入脾靜脈（SpV）組、匯入腸系膜上靜脈（SMV）組及匯入SMV與SpV匯合處組，轉移灶位于肝右葉與肝左葉的比例分別為65∶98、116∶52及13∶15。3組間構成比差異有統計學意義（χ2=28.575，P=0.000），進一步比較前2組間構成比，差異有統計學意義（χ2=27.951，P=0.000）。③IMV匯入SpV組中，76.0% （19/25）的病例其轉移灶主要分布在肝左葉（P=0.001）；IMV匯入SMV組中，73.5% （25/34）的病例其轉移灶主要位于肝右葉（P=0.000）。
結論　右半結腸癌轉移灶主要分布在肝右葉，而左半結腸癌中其IMV匯入SpV者轉移灶主要分布在肝左葉，匯入SMV者轉移灶主要分布在肝右葉。該分布規律，一方面對肝轉移癌患者尋找腸道原發病變有指導意義，另一方面也有助于在初診及隨訪時提高結直腸癌肝轉移的檢出率。
