單切口椎體釘棒聯合同側椎弓根釘棒固定治療胸腰椎結核_《中國修復重建外科雜志》

目的探討病灶清除加單切口椎體釘棒聯合同側椎弓根釘棒固定植骨融合術治療胸腰椎結核的療效。方法2008年1月－2010年10月，對22例胸腰椎結核患者行病灶清除加單切口椎體釘棒聯合同側椎弓根釘棒固定植骨融合術，術后繼續行抗結核治療。男14例，女8例；年齡18～66歲，平均42歲。病程2～16個月，平均6個月。雙節段16例，其中T7、8 3例、T8、9 1例、T9、10 3例、T11、12 2例、L1、2 4例、L3、4 3例；三節段6例，其中T7～9 2例、T11～L1 1 例、L2～4 3例。術前疼痛視覺模擬評分（VAS）為（7.50 ± 0.63）分；神經功能按美國脊髓損傷學會（ASIA）修訂的Frankel分級：B級2例，C級4例，D級9例，E級7例。結果術后22例均獲隨訪，隨訪時間15～36個月，平均25.2個月。1例于術后1個半月發生切口感染，經相應處理后愈合；其余患者切口均Ⅰ期愈合。未發現內固定物松動、斷裂，無脊髓損傷加重及腦積液漏等并發癥發生。X線片及CT示患者椎間隙均達骨性融合，融合時間3～6個月，平均5.2個月。患者術后紅細胞沉降率較術前明顯降低，差異有統計學意義（P lt; 0.05）。術后2周及末次隨訪時VAS評分分別為（2.90 ± 1.00）分及（2.60 ± 0.81）分，均較術前顯著改善（P lt; 0.05）。末次隨訪時患者神經功能Frankel分級為C級2例，D級5例，E級15例。結論單切口椎體釘棒聯合同側椎弓根釘棒固定治療胸腰椎結核創傷小，能有效恢復脊柱穩定，但遠期療效需進一步觀察。
