目的探討鎖定鋼板治療Neer三部分及四部分肱骨近端骨折的療效。方法回顧分析2008年7月－2011年5月，采用切開復位、鎖定鋼板內固定治療并獲12個月以上隨訪的77例Neer三部分及四部分肱骨近端骨折患者的臨床資料。男39例，女38例；年齡18～81歲，平均54.2歲。致傷原因：跌傷47例，交通事故傷16例，高處墜落傷4 例，運動傷5例，其他原因5例。受傷至手術時間2～16 d，平均4.5 d。骨折根據Neer分型標準：三部分骨折54例，四部分骨折23例。隨訪評估患者肩關節功能Constant評分、疼痛視覺模擬評分（VAS）及相關并發癥。結果術后76例切口Ⅰ期愈合，1例Ⅱ期愈合。患者均獲隨訪，隨訪時間12～36個月，平均18.5個月。末次隨訪時，Constant評分為（71.1 ± 11.9）分；獲優18例，良24例，中25例，差10例，優良率為54.5%。VAS評分為（2.8 ± 2.2）分。2例患者發生骨不連；余患者骨折均愈合，愈合時間2～6個月，平均3.2個月。術后25例患者發生并發癥，發生率為32.5%；15例行二次手術，二次手術率為19.5%。結論對于Neer三部分及四部分肱骨近端骨折應爭取解剖復位、穩定固定、減少肱骨頭血供破壞，以獲得良好療效。
