關節鏡下治療膝關節外側盤狀半月板損傷的臨床療效_《中國修復重建外科雜志》

目的探討關節鏡下治療膝關節外側盤狀半月板損傷的方法及近期療效。方法2010年1月－2011年5月，收治38例（42膝）外側盤狀半月板損傷患者。男23例（24膝），女15例（18膝）；年齡7～62歲，中位年齡32.8歲。病程7 d～40年，中位病程8.6個月。根據Watanabe盤狀半月板分型標準，完全型22膝，不完全型19膝，Wrisberg型1膝。于關節鏡下行半月板成形聯合縫合術25例（28膝），半月板次全切除術12例（13膝），半月板全切除術1例（1膝）。術后早期開始肌力訓練和關節活動練習。結果術后切口均Ⅰ期愈合。患者3～4周基本恢復正常活動。38例均獲隨訪，隨訪時間12～18個月，平均14.3個月。無關節交鎖及彈響癥狀；未出現再撕裂或因癥狀復發再手術的患者。術后3、6個月及1年膝關節屈曲及伸直活動范圍均較術前顯著改善（P lt; 0.05）；術后1年手術療效按照Ikeuchi的評價方法，獲優22膝，良16膝，可4膝，優良率為90.4%。術前及術后即刻、3、6個月、1年的Lysholm評分分別為（69.38 ± 4.59）、（88.57 ± 2.95）、（91.02 ± 4.17）、（92.90 ± 3.36）、（94.74 ± 3.52）分，術后各時間點評分均較術前顯著改善（P lt; 0.05）。結論關節鏡下治療外側盤狀半月板損傷創傷小，能夠精確切除或縫合失穩及破裂的半月板，最大限度保留半月板功能，延緩膝關節退變，術后配合正規康復訓練可獲得良好近期療效。
