新鮮閉合跗中關節骨折脫位的治療_《中國修復重建外科雜志》

目的探討新鮮閉合跗中關節骨折脫位的治療方法及療效。方法2004年4月－2011年4月，收治73例（75足）新鮮閉合跗中關節骨折脫位，行閉合整復結合切開復位內固定治療。男56例（58足），女17例（17足）；年齡19～62歲，平均35.8歲。高處墜落傷35例，扭傷4例，機器皮帶絞傷5例，重物砸傷9例，交通事故傷20例。傷后至入院時間為1 h 30 min～48 h，平均4.5 h。跗中關節損傷根據Main等分型標準：縱向壓縮型6足，內側移位型33足，外側移位型17足，跖屈型9足，碾壓損傷型10足。合并中足骨折脫位34足，舟骨骨折6足，骰骨壓縮骨折18足，跟骨骨折8足，距骨骨折7足，脛距關節脫位2足，距下關節脫位2足，內踝骨折1足。合并足急性筋膜室綜合征3足。結果術后65例（67足）切口Ⅰ期愈合，8例（8足）Ⅱ期愈合。 62例（62足）獲隨訪，隨訪時間11個月～7年11個月，平均3年6個月。術后26足步行時足部疼痛，36足步行時自覺傷足僵硬或不適。X線片復查示，59足跗中關節骨折脫位及其合并傷均復位良好，無再脫位及骨折不愈合發生；3足發生足舟骨壞死，繼發扁平足，均行關節融合術。末次隨訪時，按照美國矯形足踝協會（AOFAS）標準評價足功能，為77～90分，平均88.6分。結論根據術前對損傷的評估，采用手法整復結合內固定（微型接骨板橋式支撐、空心螺釘聯合克氏針內固定）治療存在不同合并損傷的跗中關節骨折脫位療效良好。
