全螺紋加壓無頭空心螺釘經皮固定骨盆骨折_《中國修復重建外科雜志》

【摘要】目的探討全螺紋加壓無頭空心螺釘經皮固定治療骨盆骨折的療效。方法 2008年3月－2010年4月對31例骨盆骨折患者行全螺紋加壓無頭空心螺釘經皮內固定（手術組），并與同期行保守治療的14例骨盆骨折患者（對照組）臨床療效進行比較。兩組患者年齡、性別構成比比較差異無統計學意義（P gt; 0.05），骨折Tile分型對照組輕于手術組。傷后近期恢復以床上可主動翻身和至下床助行器輔助下行走達到5步為標準。遠期療效隨訪根據疼痛、步態、骨折愈合和功能恢復四方面評價。結果手術組除1例術前伴骶叢神經損傷患者外，其余患者于傷后1～4 d達近期恢復標準；對照組A2型、B2型骨折，翻身時間分別為傷后9～13 d與17～25 d，行走達5步時間分別為傷后17～21 d與45～57 d。兩組患者均獲隨訪，隨訪時間6～19個月。骨折均在2～6個月內愈合。術后6個月手術組患者療效獲優29例，良2例；對照組獲優8例，良6例，與手術組比較差異有統計學意義（χ2=6.636，P=0.017）。結論正確的經皮內固定技術結合全螺紋加壓無頭空心螺釘，能夠快速減輕患者痛苦、提高生活質量，并有較好的治療效果。
