改良雙葉瓣修復顴面部皮膚癌術后缺損的療效_《中國修復重建外科雜志》

目的探討改良雙葉瓣修復顴面部皮膚癌切除術后缺損的療效。方法2009年8月－2011年10月，收治顴面部皮膚癌患者15例。男12例，女3例；年齡52～78歲，平均64.1歲。病程1～14個月，平均4.6個月。基底細胞癌11例，鱗狀細胞癌4例。術前1例腫瘤病灶合并感染，余患者皮膚均無破潰；腫瘤均累及皮膚層。TNM分期：T2N0M2 13例，T3N0M3患者2例。腫瘤病灶切除后遺留缺損范圍為4.0 cm × 2.5 cm～6.5 cm × 4.0 cm。采用改良的耳前、耳后雙葉瓣修復缺損，切取范圍第1葉皮瓣4.0 cm × 2.5 cm～6.5 cm × 4.0 cm，第2葉皮瓣3.0 cm × 2.0 cm～5.0 cm × 3.0 cm。結果術后1例切口部分裂開，經換藥后痊愈；余患者皮瓣均順利成活，切口均Ⅰ期愈合。14例獲隨訪，隨訪時間12～24個月，平均18.7個月。患者面部無明顯不對稱，眼瞼閉合無影響，無面神經損傷表現。隨訪期間均無腫瘤復發。末次隨訪時患者滿意度評分：不滿意1例，可以接受1例，比較滿意7例，非常滿意5例。結論使用改良耳前、耳后雙葉瓣修復顴面部皮膚癌術后缺損可獲得較好效果，且并發癥少。
