目的　探討二尖瓣置換術（MVR）后左心室后壁早期破裂的原因、治療及預防措施。　方法　回顧性分析1981年5月至2010年11月期間沈陽軍區總醫院施行二尖瓣置換術2 638例中發生左心室后壁早期破裂7例的臨床資料，其中男2例，女 5例；年齡28～71歲。術前心功能分級（NYHA）Ⅱ級1例，Ⅲ級6例。心臟彩色超聲心動圖提示左心室射血分數（LVEF） 49%～ 60%，左心室舒張期末容積（LVEDV） 29～42 ml。均在中度低溫、全身麻醉、體外循環下行人工心瓣膜置換術。　結果　7例左心室后壁早期破裂均經手術修補，其中Ⅰ型2例，Ⅱ型4例，Ⅲ型1例。死亡4例，均死于失血性休克或低心排血量綜合征。手術搶救成功3例，術后7～15 d順利出院。3例生存患者隨訪4.5～18.0年，一般狀況良好，生活質量滿意，無晚期假性動脈瘤形成。　結論　左心室后壁早期破裂修補困難，死亡率較高，應以預防為主；術中一旦發生破裂，應該盡快恢復體外循環，根據實際情況在心腔內、外聯合修補較為確實、可靠，有可能挽救患者的生命。
