全胸腔鏡肺葉切除術治療周圍型肺癌的臨床分析_《中國胸心血管外科臨床雜志》

目的　探討全胸腔鏡肺葉切除術在治療周圍型肺癌患者中的應用。　方法　回顧性分析2009年7月至2011年12月安徽醫科大學附屬省立醫院應用全胸腔鏡肺葉切除術治療90例周圍型肺癌患者的臨床資料，其中男55例，女35 例；年齡33～79 （62.5±11.5）歲。觀察術中淋巴結清掃組數、手術時間、術中出血量、術后胸腔引流時間、術后住院時間、并發癥發生率和疼痛評分。　結果　圍術期無死亡。手術時間（135.0±32.5） min，術中出血量（230.0±80.4） ml，術后胸腔引流時間（4.8±2.1） d，術后第3 d疼痛評分（5.3±1.2）分。共清掃淋巴結520組，1 568枚，5.8組/例、17.4枚/例；淋巴結有轉移71組，陽性率13.7% （71/520）。術后發生聲音嘶啞2例；乳糜胸3例，經相應的治療均治愈。隨訪90例，隨訪時間1～24個月，隨訪期間因腫瘤轉移死亡4例；其余生存患者生活質量良好。　結論　對周圍型肺癌患者采用全胸腔鏡肺葉切除術治療，具有創傷小、恢復快和疼痛輕等優點。手術安全性、根治性與開胸手術相似，可作為治療周圍型肺癌患者的手術方式。
