獲得性魯登巴赫綜合征的外科治療_《中國胸心血管外科臨床雜志》

目的探討獲得性魯登巴赫綜合征的手術時機及治療方法，以改善預后。方法回顧性分析 2000年 1月至 2009年 12月北京安貞醫院 16例獲得性魯登巴赫綜合征患者手術治療的臨床資料，其中男 9例，女 7例；年齡（39.45±10.23）歲。所有患者在氣管內插管靜脈復合全身麻醉體外循環下施行手術，經胸骨正中切口徑路進胸行二尖瓣置換術＋房間隔缺損修補術。術后觀察圍術期死亡和并發癥發生情況。隨訪期間采用超聲心動圖觀察有否瓣周漏和房間隔水平殘余分流、心室的大小、肺動脈壓的改變和心功能的改善情況等。結果術后死亡 2例（12.5%），其中 1例死于腎功能衰竭，另 1例死于肺部感染。1例術中不能脫離體外循環，應用體外膜式氧合（ECMO）輔助 3 d，順利撤除，痊愈出院； 1例應用連續性腎臟替代治療（ CRRT）11 d，逐漸恢復； 1例術后 6 d做氣管切開，術后 23 d脫離呼吸機，自主呼吸，痊愈出院。隨訪 14例，隨訪率 100%，隨訪時間 6個月～ 5年，術后 6個月復查超聲心動圖提示：左心室舒張期末內徑（ 42.1±5.7）mm，無瓣周漏和房間隔水平殘余分流，射血分數 67.4%±6.7%，估測肺動脈壓（ 23.4±5.4）mm Hg。心功能分級（ NYHA）Ⅱ級 12例，Ⅲ級 2例。結論對手術時機的選擇，只要患者左心室舒張期末容積指數提示可以耐受手術，就應該盡早施行手術治療。對術前判定脫離體外循環困難的患者，建議直接應用 ECMO輔助。
