改良Carpentier法矯治三尖瓣下移畸形_《中國胸心血管外科臨床雜志》

目的探討改良 Carpentier法矯治三尖瓣下移畸形（ Ebstein畸形）的手術方法，總結其臨床經驗。方法回顧性分析 2006年 6月至 2010年 8月安徽醫科大學第一附屬醫院 13例 Ebstein畸形患者手術治療的臨床資料，其
中男 6例，女 7例；年齡（ 26.8±13.5）歲。所有患者均采用改良 Carpentier法矯治，手術方法包括：切除部分房化右心室，折疊環縮三尖瓣瓣環至適當的大小，用自體心包片擴大后瓣葉 /隔瓣葉，移位縫合切下的部分前瓣葉（向內旋轉后）、后瓣及隔瓣葉縫至正常的瓣環水平，移位相應的乳頭肌、腱索，擴大修復瓣葉，加固瓣環，并矯正其他合并的心血管畸形。結果所有患者術后均恢復順利，無死亡。超聲心動圖提示：三尖瓣功能良好，三尖瓣瓣葉均在正常位置；三尖瓣有輕度至中度反流 3例，無反流或有輕微反流 10例。心功能分級（ NYHA）I～Ⅱ級。所有患者均得到隨訪，隨訪時間 3～15個月，平均 8個月。術后 3個月和 1年分別復查超聲心動圖提示：三尖瓣啟閉及右心室功能良好，三尖瓣無明顯反流 12例，1例出院時三尖瓣有中度反流患者轉為輕度反流。所有患者恢復正常的生活或工作。結論改良 Carpentier法矯治 Ebstein畸形有較好的近期療效，該手術方法切除無功能的房化右心室后重新縫合塑形，有利于右心室形態重建和功能恢復，通過自體心包片擴大瓣葉、充分的瓣葉移位及后乳頭肌、腱索移位重建可達到良好的瓣膜成形效果。
