主動脈瓣置換術患者合并冠心病的臨床特點_《中國胸心血管外科臨床雜志》

目的　探索主動脈瓣病變合并冠心病患者的臨床特點，以提高臨床治療的針對性。　方法　回顧性分析沈陽軍區總醫院2010年1～10月期間78例50歲以上行主動脈瓣置換術患者的臨床資料，根據是否合并冠心病，將患者分為兩組：合并冠心病組（病例組），20例，男19例、女1例，年齡（64.6±8.3）歲；單純主動脈瓣置換術組（對照組），58例，其中男28例、女30例，年齡（58.7±6.2）歲。通過單因素分析和多因素分析，探索主動脈瓣置換術患者合并冠心病的臨床特點。結果單因素分析結果顯示，病例組年齡（P=0.000）、男性（P=0.004）和糖尿病史（P=0.004）、主動脈瓣反流（P=0.034）、心瓣膜病變（P=0.028）、呼吸機輔助時間（P=0.008）均明顯高于對照組，且差異有統計學意義。多因素分析結果顯示，主動脈瓣置換術患者合并冠心病的獨立臨床特點包括男性、年齡較大、術后呼吸機輔助時間明顯延長、術后6個月時的肺動脈收縮壓（PASP）較對照組明顯下降、術前右心室舒張功能明顯減低。結論單純主動脈瓣病變男性患者若術前伴有右心室舒張功能降低和/或主動脈瓣呈三葉的退行性病變，應將冠狀動脈病變篩選年齡適當放寬；單純行主動脈瓣置換術的患者應較長時間地服用逆轉左心室重構的藥物，以避免惡性心律失常的發生。
