肺周邊局灶性炎癥超聲聲像圖特征分析_《中國呼吸與危重監護雜志》

目的探討肺周邊局灶性炎癥的聲像圖特征, 提高超聲對肺周邊局灶性病變的診斷與鑒別診斷能力。方法回顧性分析44 例肺周邊局灶性炎癥患者的聲像圖, 與其他病變聲像圖特征進行對比, 將病灶邊界清晰度、回聲類型、臟胸膜繼發改變、病灶內表面成角、病灶內支氣管氣相作為自變量, 病灶性質是否為周邊局灶性炎癥作為因變量進行Logistic 分析。病理結果經超聲引導穿刺活檢獲得。結果病灶內表面成角和病灶內支氣管氣相對診斷肺周邊局灶性炎癥有顯著影響( P lt;0. 01) 。以病灶內表面成銳角作為診斷肺周邊局灶性炎癥的標準, 與穿刺活檢結果對比, 超聲檢查對肺周邊局灶性炎癥診斷的準確率為82. 6%, 敏感度為72. 7% , 特異度為84. 7% , 陽性預測值為51. 0%, 陰性預測值為93. 4%。結論病灶內表面成銳角及病灶內支氣管氣相對診斷肺周邊局灶性炎癥有一定價值。床旁超聲可以診斷肺周邊局灶性炎癥。
