目的研究不同類型（囊狀、梭狀型）先天性膽總管囊腫壁細胞凋亡與增殖的關系，了解細胞凋亡/增殖在膽總管囊腫中的作用。方法以膽總管囊狀擴張組32例、梭狀擴張組35例及膽總管結石致膽管擴張組（對照組）25例為研究對象。術中取部分膽總管壁送檢,觀察膽總管壁損傷情況。用免疫組織化學法檢測細胞凋亡的相關指標bcl2、bax及細胞增殖指標PCNA的表達；用TUNEL法檢測細胞凋亡情況。結果囊狀擴張組膽總管壁粘膜上皮細胞破壞嚴重，梭狀擴張組鏡下所見與囊狀擴張組相似，但程度較輕。對照組膽總管粘膜上皮細胞破壞最輕，其凋亡細胞呈散在分布，上皮細胞和成纖維細胞凋亡陽性率較低，分別為（2.74±1.00）%和（2.95±0.87）%； bcl2、bax表達率較低，水平相似（P gt;0.05）； PCNA表達率較高，分別為（3.74±1.00）%和（3.71±1.77）%。囊狀擴張組與梭狀擴張組囊壁殘留上皮中的bcl2和PCNA表達率低，分別為（0.99±0.51）%和（0.90±0.38）%； bax呈高表達，bcl2呈低表達，凋亡細胞陽性率較高，分別為（13.94±4.77）%和（7.51±3.46）%；成纖維細胞中的PCNA表達率較高，分別為（9.91±2.91）%和（9.70±3.18）%，bax呈低表達，bcl2呈高表達，凋亡細胞陽性率較低，分別為（3.74±2.12）%和（4.46±2.41）%。兩組之間上述各項指標比較無明顯差異（P gt;0.05），但兩組的bcl2和bax的表達與對照組比較則明顯升高(P＜0.05)。結論細胞凋亡在膽總管囊腫形成中有一定的促進作用。
